Klinisch onderzoek werd uitgevoerd op de afdeling Kaakchirurgie van de tandheelkundige kliniek van Vojvodina, Novi Sad, Servië, waarbij gebruik werd gemaakt van het dubbelblinde prospectieve klinische onderzoeksontwerp. Microbiologische studies werden uitgevoerd in het Centrum voor Microbiologie van het Instituut voor Volksgezondheid van Vojvodina, Novi Sad, Servië, op uitstrijkjes verkregen uit de chirurgische wonden van patiënten met postoperatieve infectie of het exsudaat (pus) verkregen uit de peri-coronale ruimte van patiënten met pericoronitis of een voorgeschiedenis van pericoronitis. De gevoeligheid van geïsoleerde micro-organismen voor moxifloxacine, cefixime en andere antibiotica die gewoonlijk worden gebruikt bij orale en maxillofaciale chirurgie (penicilline en zijn derivaten, erytromycine, azitromycine, clindamycine en tetracycline) werd microbiologisch getest. Patiënten ≥18 jaar, geïndiceerd voor chirurgische verwijdering van de geïmpacteerde derde molaren in de onderkaak, met een goede systemische gezondheid - patiënten geclassificeerd als American Society of Anesthesiologists (ASA) classificatie van fysieke gezondheid is een classificatiesysteem voor preoperatieve gezondheid van de chirurgische patiënten - ASA I en ASA II werden in de studie opgenomen. Evaluatie van de effecten van de toegepaste medicatie vond plaats op basis van postoperatieve gevolgen of complicaties. Complicaties, waaronder droge kom / alveolaire osteïtis (AO) en postoperatieve infectie op de operatieplaats (SSI), en inflammatoire gevolgen - pijn, zwelling en trismus (beperkte mondopening), werden postoperatief geëvalueerd.
In het geval van pericoronitis of een voorgeschiedenis van pericoronitis, met de aanwezigheid van exsudaat of pus, evenals in het geval van postoperatieve infectie, werd een uitstrijkje genomen uit de operatiewond of pericoronaire ruimte en werd het punctaat van exsudaat of pus verzonden voor microbiologische analyse. Het belangrijkste doel van deze analyse was om de gevoeligheid van micro-organismen vast te stellen, niet alleen voor antibiotica die in dit onderzoek worden gebruikt, maar ook voor antibiotica, die de "gouden standaard" zijn bij de behandeling van odontogene infecties, evenals voor andere veelgebruikte antibiotica in de behandeling. Analyses werden uitgevoerd met standaard bacteriologische technieken (het materiaal op geschikte voedingsbodems zaaien en gedurende 18-24 uur bij 35-37 °C incuberen) om de bacteriën te isoleren. Identificatie van bacteriesoorten werd uitgevoerd door klassieke bacteriologische methoden (morfologische, culturele en fysiologisch-biochemische), evenals door het geautomatiseerde systeem Matrix Assisted Laser Desorption Ionization - Time of Flight (MALDI-TOF), Duitsland. Na identificatie werden gevoeligheidstests van geïsoleerde bacteriesoorten voor antimicrobiële geneesmiddelen uitgevoerd met behulp van de standaard schijfdiffusiemethode volgens Kirby-Bauer, Mueller-Hinton-agar en bloed Mueller-Hinton-agar (HiMedia, India), afhankelijk van welke bacteriesoort in kwestie was in overeenstemming met de richtlijnen van het European Committee on Antimicrobial Susceptibility Testing (EUCAST) of Clinical & Laboratory Standards Institute (CLSI). Schijven van Bio-Rad, Frankrijk, werden gebruikt om de microbiologische analyses uit te voeren.
Alle chirurgische ingrepen werden uitgevoerd onder plaatselijke verdoving, met 2% lidocaïne met adrenaline 1:80.000 in een totaal volume van 4 ml (2 ml oplossing voor injectie bevatte 40 mg lidocaïne hydrochloride in de vorm van lidocaïne hydrochloride monohydraat en 0,025 mg adrenaline in de vorm van adrenalinetartraat: Lidocaïne 2% - adrenaline, 40 mg +0,025. Bij alle patiënten omvatte de operatie een driehoekige mucoperiostale flap, gehecht met synthetisch polyfilament niet-resorptief hechtmateriaal (3-0). Patiënten kregen het advies een analgeticum te nemen dat 200 mg ibuprofen en 325 mg paracetamol bevatte.
In overeenstemming met bekende ethische principes en in overeenstemming met de principes van Good Clinical Practice (GCP), werd de zorg voor het welzijn van de patiënten maximaal gerespecteerd, in overeenstemming met de laatste herziening van de Verklaring van Helsinki en aanvaarde principes die van toepassing zijn op klinische proeven op mensen. De houder van de bescherming van de patiënten in deze klinische proef was de hoofdonderzoeker, die samenwerkte met de ethische commissie van de tandheelkundige kliniek van Vojvodina, Universiteit van Novi Sad. De schriftelijke geïnformeerde toestemming van de patiënt impliceerde dat de patiënt volledige informatie over het onderzoek had ontvangen en werd benadrukt dat hij het recht had om onafhankelijk te beslissen om deel te nemen, zonder dwang en externe invloeden, of enige schadelijke gevolgen als ze weigerden deel te nemen.

Inclusiecriteria:
- patiënten ≥18 jaar, geïndiceerd voor chirurgische verwijdering van de geïmpacteerde derde molaren in de onderkaak, met een goede systemische gezondheid (patiënten geclassificeerd als ASA I en ASA II) werden in de studie opgenomen.
Uitsluitingscriteria:
- overgevoeligheid voor studiegeneesmiddelen; geschiedenis van systemische antibacteriële therapie binnen 6 maanden voorafgaand aan randomisatie; zwangerschap of borstvoeding; fluorochinolon-gerelateerde peesaandoening; klinisch relevante hartaandoeningen of geneesmiddelen die het QT-interval verlengen; ernstige leverinsufficiëntie (Child-Pugh C). Criteria voor het niet opnemen van de patiënten in het onderzoek waren ook gevallen waarin, naast het verwijderen van de geïmpacteerde derde kies van de onderkaak, een andere orale chirurgische ingreep werd uitgevoerd, evenals patiënten met momenteel aanwezige pericoronitis.

Patiënten kregen oraal filmomhulde tabletten met ofwel 400 mg moxifloxacine.

Alle patiënten werden geëvalueerd bij de postoperatieve follow-ups op de eerste, tweede en zevende postoperatieve dag.

Een derde van de patiënten kreeg placebo-tabletten die verschillende stoffen bevatten zonder antimicrobiële werking (99% microkristallijne cellulose, 0,5% siliciumdioxide en 0,5% magnesiumstearaat, die dezelfde kleur en hetzelfde uiterlijk hadden als de gebruikte antibiotica.
Filmomhulde tabletten werden gedurende de eerste vijf dagen postoperatief eenmaal daags dubbelblind toegediend.
Alle patiënten werden geëvalueerd bij de postoperatieve follow-ups op de eerste, tweede en zevende postoperatieve dag.

Postoperatieve pijn beoordeeld met visuele analoge schaal (VAS)

De postoperatieve pijn werd beoordeeld met behulp van een visueel analoge schaal (VAS).
VAS vertegenwoordigt een horizontaal getekende lengte van 100 mm, waarvan het begin een pijnvrije toestand aan de linkerkant aangeeft en het einde aan de rechterkant de toestand van de sterkst denkbare pijn.
Alle patiënten gaven door een streepje op de schaal te plaatsen de intensiteit van de pijn aan die ze op de juiste dag in de postoperatieve periode in de hoogste intensiteit voelden.
De intensiteit van pijn werd bepaald door de afstand van de gemarkeerde plaats op de schaal vanaf het begin van de schaal aan de linkerkant te meten en uitgedrukt in millimeters.
