Urine Tenofovir Point-of-care-test om patiënten te identificeren die ART-adherentieondersteuning nodig hebben (UTRA-onderzoek)
ART wordt gegeven aan mensen met hiv om het virus te onderdrukken, wat resulteert in een betere gezondheid voor het individu en een verminderde overdracht van het virus op anderen. Het succes van ART is afhankelijk van therapietrouw. Momenteel wordt de therapietrouw beoordeeld door patiënten rechtstreeks te vragen en vervolgens te bevestigen met een viral load-test, wat duur is en vaak alleen wordt gedaan als de viral load al verhoogd is. Daarom moet er een methode worden gevonden om problemen met therapietrouw vroeg op te sporen voordat de viral load stijgt.
Onlangs is een op urine gebaseerde test ontwikkeld, UTRA (urine tenofovir rapid assay) genaamd. Deze test kan het personeel van de kliniek onmiddellijke resultaten geven over iemands therapietrouw aan de antiretrovirale medicatie Tenofovir (TDF). De studie zal de resultaten van deze urinetest vergelijken met de medicijnniveaus in het bloed, zelfgerapporteerde therapietrouw en apotheekverzamelingsrecords om te zien of deze test kan worden gebruikt als onderdeel van routinematige zorg in ART-klinieken. Als de test effectief is, zou het personeel van de kliniek mensen met therapietrouwproblemen vroegtijdig kunnen identificeren en hen de nodige ondersteuning kunnen geven voordat hun viral load stijgt.

Gedetailleerde beschrijving
Dit is een gerandomiseerde studie, waarbij 200 mensen werden gerekruteerd die op TDF gebaseerde ART gebruikten. Deelnemers worden 1:1 gerandomiseerd naar interventie versus standaard verbeterde therapietrouw. Interventiedeelnemers ontvangen een ondersteuningspakket voor therapietrouw op basis van feedback van de urinetest. De onderzoekers zullen de impact van de therapietrouwtest op de VL-onderdrukkingspercentages in elke arm beoordelen (zonder dat een verandering van regime nodig is) 12 maanden na inschrijving. Het inschrijven van deelnemers met nalevingsproblemen verhoogt de efficiëntie en weerspiegelt de populatie die therapietrouwondersteuning nodig heeft in onze omgeving.
Het UTRA-apparaat dat voor dit onderzoek is gebruikt, heeft een FDA-aanduiding voor niet-significant risico (NSR) gekregen en is vrijgesteld van IDE.
GESCHIKTHEID:
De onderzoekers volgen de retentie in het onderzoek bij deelnemers in elke arm, gemiste bezoeken en het aantal uitgevoerde urinebeoordelingen in de interventiearm.
AANVAARDBAARHEID:
Alle ingeschreven deelnemers zullen ook een gestandaardiseerde vragenlijst invullen over de aanvaardbaarheid van de therapietrouw die ze hebben ontvangen. Deze mate van aanvaardbaarheid zal worden beoordeeld als een secundair resultaat per arm, samen met socio-demografische profielen geassocieerd met lage/hoge aanvaardbaarheid beschreven binnen arm.
KWALITATIEVE PROCEDURES:
- Diepte-interviews met deelnemers: Kwalitatieve gegevens omvatten diepte-interviews van deelnemers (n~20; ~3-4 interviews met elke deelnemer gedurende 6-9 maanden van hun behandeling) met behulp van semi-gestructureerde gidsen uitgevoerd door ervaren sociaal-gedragswetenschappers . De semi-gestructureerde interviewgids zal gevoelens opwekken over de urinetrouw-metriek en adviesboodschappen, zorgen over privacy, voor- en nadelen van het ontvangen van dergelijke resultaten, en de waarschijnlijke impact van deze monitoringtest op aanhoudende therapietrouw of alleen op therapietrouw op korte termijn. . Interviews worden gehouden in de voorkeurstaal van de deelnemers.
- Interviews met zorgverleners: De onderzoekers zullen bovendien de haalbaarheid van de interventie beoordelen door zorgverleners te interviewen, die de UTRA-test in de toekomst op het klinische zorgpunt zullen afnemen. De studie zal de percepties van de provider van de test aan het einde van de studie onderzoeken met behulp van diepte-interviews (n~10), waarbij het waargenomen nut, de complexiteit van het gebruik, stigma/sociale schade en voordelen worden beoordeeld. Deze interviews zullen ook barrières en facilitators opwerpen voor de levering van de op urinetesten gebaseerde adviesberichten.

Inclusiecriteria:
- ≥18 jaar oud
- Bereid en in staat om schriftelijke geïnformeerde toestemming te geven
- HIV-geïnfecteerd, die een op tenofovir gebaseerd ART-regime krijgen of (her)starten
- Het huidige ART-regime omvat ten minste één geneesmiddel met een hoge genetische barrière tegen resistentie, b.v. dolutegravir, atazanavir/ritonavir, darunavir/ritonavir of lopinavir/ritonavir.
- Elke eerdere verhoogde virale belasting >50 kopieën/ml (na ART-initiatie).
- Bereid en in staat om te voldoen aan laboratoriumtests en andere studieprocedures
Uitsluitingscriteria:
- Niet bereid of in staat om geïnformeerde toestemming te geven in een van de aangeboden talen
- Geen op tenofovir gebaseerd ART-regime ontvangen
- Elke andere klinische aandoening waarbij de patiënt naar de mening van een onderzoeker een verhoogd risico loopt bij deelname aan het onderzoek.
