Inclusiecriteria:
- Cryptogene beroerte
- Leeftijd >60 jaar
- Rechts-links-shunt zoals geëvalueerd door echocardiografie (TEE).
Uitsluitingscriteria:
-≤60 jaar oud
- Lacunaire (kleine vat) beroerte.
- Permanente of paroxismale atriale fibrillatie/fladderen (klinisch zichtbaar of gedetecteerd door continue ECG-bewaking).
- Behoefte aan chronische antistollingstherapie.
- Elke contra-indicatie voor plaatjesaggregatieremmers (aspirine, clopidogrel, ticagrelor).
- Aanwezigheid van extracraniale of intracraniale atherosclerose die ≥50% luminale stenose veroorzaakt in slagaders die het gebied van ischemie voeden.
- Aanwezigheid van complexe atheroomplaques bij de stijgende aorta-aortaboog (≥ 4 mm dik, verzweerd of met mobiele trombi) zoals beoordeeld door TEE.
- Aanwezigheid van intracardiale trombus zoals beoordeeld door TEE.
- Ongecontroleerde hypertensie (systemische drukwaarden >160/90 mmHg ondanks optimale medische behandeling).
- Geschiedenis van een hartinfarct of coronaire interventie. (percutane coronaire interventie, coronaire bypass-transplantaat).
- Geschiedenis van eerdere klepchirurgie of transkatheterklepreparatie.
- Aanwezigheid van diepe veneuze trombose op het moment van indexslag zoals geëvalueerd door Doppler-echografie.
- Linkerventrikelejectiefractie <50% zoals geëvalueerd door TTE.
- Significante (matige of ernstige) klepaandoening zoals geëvalueerd door echocardiografie.
- Geschiedenis van congestief hartfalen.
- Ernstige chronische nierfunctiestoornis definieerde een geschatte glomerulaire filtratiesnelheid <30 ml/min/m2 of de noodzaak van dialyse.
- Geïsoleerde ASS of ASS geassocieerd met PFO maar met een hemodynamisch significante links-naar-rechts-shunt die gesloten moet worden.
- Andere specifieke oorzaak van beroerte vastgesteld (bijv. arteritis, dissectie, migraine/vasospasme en drugsmisbruik).
- Voorafgaande chirurgische of endovasculaire behandelingen van PFO of ASS.
- Reumatische hartziekte.
- Vergroting van het linker atrium gedefinieerd als een diameter van het linker atrium >41 mm bij mannen en ≥39 mm bij vrouwen.
- Aanwezigheid van hoge belasting van premature atriale contracties (> 500 per 24 uur) zoals beoordeeld door continue ECG-bewaking.
- Opvolging onmogelijk of verwachte slechte naleving.
- Actieve kanker.
- Aanwezigheid van een inferieur vena cava-filter.
- Ernstige hypertensie van de longslagader (systolische pulmonale druk >60 mmHg).
- Functionele afhankelijkheid zoals gemeten door een gemodificeerde Rankin-schaalscore >3 (niet in staat om zonder hulp in de eigen lichamelijke behoeften te voorzien en niet in staat om zonder hulp te lopen).
- Elke medische aandoening die een levensverwachting <2 jaar bepaalt.
- Deelname aan een ander gerandomiseerd onderzoek.
- Het niet verstrekken van ondertekende geïnformeerde toestemming.

Experimenteel: transcatheter PFO sluiting + optimale medische behandeling
Patiënten ondergaan een transkatheter PFO-sluiting (+ optimale medische behandeling).
Patiënten zullen antitrombotische middelen krijgen (enkele plaatjesaggregatieremmende behandeling) en aanpasbare vasculaire risicofactoren (dyslipidemie, hypertensie, diabetes) volgens richtlijnen voor beroertepreventie.
Het type antitrombotische therapie wordt overgelaten aan het oordeel van de arts die verantwoordelijk is voor de patiënt.

Transcatheter PFO-sluitingsprocedure zal worden uitgevoerd volgens de normen en ervaring van elk deelnemend centrum. Elk goedgekeurd PFO-afsluitapparaat zal in het onderzoek worden toegelaten. Patiënten zullen ook antitrombotische middelen krijgen (enkele plaatjesaggregatieremmende behandeling) en aanpasbare vasculaire risicofactoren (dyslipidemie, hypertensie, diabetes) volgens richtlijnen voor beroertepreventie. Het type antitrombotische therapie wordt overgelaten aan het oordeel van de arts die verantwoordelijk is voor de patiënt.
