Supercharged TRAM-evaluatie bij cervicale slokdarmastroplastiek na slokdarmresectie (Supercharged)
Slokdarmreconstructie heeft een hoge morbiditeit en mortaliteit, in veel gevallen als gevolg van slokdarmreconstructie. Naadlekkage en fistel zijn de belangrijkste complicaties bij slokdarmresectie. Er zijn veel onderzoeken uitgevoerd om de oorzaak van naadlekkage na slokdarmresectie te onderzoeken, maar geen van hen concludeerde dat het verband houdt met een operatie of hechtingstechniek. Het lijkt echter te worden veroorzaakt door de ischemie die wordt veroorzaakt na maagmobilisatie voor reconstructie van de slokdarm, of zelfs spanning in de anastomose.
Gezien de hoge morbiditeit en mortaliteit na slokdarmresectie na gastroplastiek, zijn strategieën om een nieuwe bron van vascularisatie te creëren en het aantal naadlekkages te verminderen belangrijk. In deze studie zullen onderzoekers evalueren of een supercharged TRAM-flaptransfer effectief is bij het verminderen van de morbiditeit gerelateerd aan anastomose-ischemie bij patiënten die een slokdarmresectie ondergaan.

Het overbrengen van spierdelen is een van de belangrijkste reconstructietechnieken die bij plastische chirurgie worden gebruikt. Transverse rectus abdominis myocutaneous (TRAM) flaptransfers worden zeer overwogen vanwege de resultaten van hoge kwaliteit, de brede toepassing in veel gevallen en het kleine aantal beoordelingen op de lange termijn.
Beegle publiceerde in 1991 een nieuwe techniek voor het gebruik van TRAM-supercharged, waarbij microchirurgische anastomose wordt gebruikt tussen TRAM's eenstevige gastro-epiploïsche diepe slagader en aders en thoracale takken en bloedvaten, zoals axillaire en thoracodorsale vaten.
Op zoek naar weefselbloedperfusie herstellen en de morbiditeit geassocieerd met anastomose-ischemie verminderen, toonden sommige onderzoeken aan dat interpositie van de dikke darm of jejunum plus een extra bloedtoevoer via veneuze en arteriële anastomose - supercharged colon of jejunum effectief is. De isoperistaltische supercharged coloninterpositie was een goede optie om grote slokdarmdelen te reconstrueren waarin geen maag beschikbaar was.
Gezien de hoge morbiditeit en mortaliteit na slokdarmresectie na gastroplastiek, zijn strategieën om een nieuwe bron van vascularisatie te creëren en het aantal naadlekkages te verminderen belangrijk. Dit is een gerandomiseerde klinische studie in één instelling met deelnemers die werden gerekruteerd in de kliniek voor chirurgie van het spijsverteringsstelsel, aan het Instituto do Câncer do Estado de São Paulo (ICESP). Patiënten zullen worden gerandomiseerd naar conventionele slokdarmresectie of TRAM supercharged slokdarmresectie, en onderzoekers zullen postoperatieve complicaties in beide groepen evalueren.

- Diagnose van slokdarmkanker;
- Vermogen om te begrijpen en samen te werken tijdens de behandeling;
Uitsluitingscriteria:
- Vorige gastrectomie;
- Eerdere buikoperatie met risico op verandering van de vascularisatie van de maag;
- Eerdere hoofd-halsoperaties met risico op verandering van cervicale vaten.
