Evolutie van de cutane microbiota bij IC-patiënten met CVC (ICMc) (ICMc)

Centraal veneuze katheters (CVC's) zijn noodzakelijk bij tot 60% van de IC-patiënten, wat een risico inhoudt op kathetergerelateerde infecties met hoge morbiditeit en mortaliteit. De katheterkolonisatie, voornamelijk afkomstig van de huid, is constant, maar infectie niet. Het is bekend dat dysbiose geassocieerd is met pathologische toestanden en infecties, bijvoorbeeld post-antibioticum C. difficile diarree of atopische dermatitis, waarbij opflakkeringen geassocieerd zijn met dysbiose en overheersing van S. aureus. Cutane dysbiose zou de ontbrekende schakel kunnen zijn tussen katheterkolonisatie en infectie. Onze hypothese is dat onder invloed van meerdere IC-factoren (stress, antibioticatoediening, lokale ontsmettingsprocedures) cutane dybiose optreedt bij IC-patiënten met een centrale veneuze katheter.
Alle volwassen IC-patiënten met een indicatie voor CVK-plaatsing worden gedurende een periode van 6 maanden geïncludeerd. Er wordt een huiduitstrijkje gemaakt op de CVC-inbrengplaats voordat de CVC wordt geplaatst (basislijn), en vervolgens elke 3 dagen (of wanneer het verband wordt verwisseld) terwijl de CVC op zijn plaats zit, en vervolgens bij ontslag op de intensive care. Bacteriële metagenomica met behulp van bacteriële DNA-extractie, 16S PCR-amplificatie en Nanopore-sequencing zullen de beschrijving van de huidmicrobiota en diversiteitsevaluatie mogelijk maken via de Shannon-index. De evolutie van de alfadiversiteit zal worden geëvalueerd door middel van tijdreeksgegevensanalyse: vergelijking van de Shannon-index op verschillende tijdstippen met de Shannon-basisindex (vóór CVC-plaatsing). Er zal een standaard microbiologische kweek van huiduitstrijkjes worden uitgevoerd. Algemene patiëntkenmerken en informatie met betrekking tot CVC-infectie en -behandeling zullen worden verzameld.
Dit onderzoek zal geen impact hebben op het patiëntenbeheer.

Inclusiecriteria:
- Volwassene opgenomen op de intensive care van het Cayenne Hospital Centre bij wie de installatie van een CVC geïndiceerd is
Uitsluitingscriteria:
- Patiënt jonger dan 18 jaar
- Patiënt of vertrouwenspersoon of familie of familielid die bezwaar maakt tegen deelname aan het onderzoek (weigering)
- Patiënt onder gerechtelijke waarborg of onder enig ander beschermend regime (voogdijschap of curatorschap)
- Patiënt met huidlaesies (infectie, brandwond) nabij de cutane inbrengplaats van de CVC
