Een prospectief gerandomiseerd onderzoek naar ECP bij subklinische AMR (EUROEXPORT-DSA)
Het gebruik van extracorporale fotoferese als immunomodulerende therapie van subklinische antilichaam-gemedieerde afstoting na longtransplantatie: een prospectieve RCT
Het doel van deze klinische proef is het evalueren van het therapeutische effect van extracorporale fotoferese bij subklinische antilichaamgemedieerde afstoting na longtransplantatie. De belangrijkste vragen die het onderzoek wil beantwoorden zijn:
- Leidt ECP-therapie tot een significante vermindering van de MFI (Mean Fluorescentie Intensiteit) ten opzichte van de uitgangswaarde van de MFI bij klinisch stabiele patiënten met aanhoudende (>6 maanden) dnDSA's (MFI>1000)?
- Wat is de impact van ECP-therapie op de volgende uitkomsten bij deze patiënten: ACR, klinische AMR, CLAD, infecties, uitvalpercentage, overleving, bijwerkingen?
Deelnemers worden willekeurig in twee groepen verdeeld. Elke groep zal bestaan uit 40 patiënten. De controlegroep zal worden geobserveerd en er zal geen actieve behandeling worden toegediend. De behandelgroep krijgt extracorporale fotoferese. Ten eerste zal gedurende de eerste twee maanden eenmaal per twee weken een tweedaagse behandelcyclus worden uitgevoerd. Vervolgens wordt gedurende zes maanden eenmaal per maand een tweedaagse behandelingscyclus uitgevoerd.
Onderzoekers zullen de twee groepen vergelijken met betrekking tot: MFI-waarde, ontwikkeling van ACR, klinische AMR, CLAD, infecties, overleving, bijwerkingen, immunofenotypering, miRNA-expressieprofilering, cytokine-expressie, genexpressiehandtekening van PBMC's en proteomische karakterisering.

Donorspecifieke antilichamen (DSA) en antilichaamgemedieerde afstoting hebben de laatste tijd veel aandacht gekregen vanwege de ernstige gevolgen ervan, maar er zijn beperkte effectieve behandelingen voor aanhoudende DSA's bij patiënten zonder transplantaatdisfunctie vanwege de daarmee gepaard gaande ernstige bijwerkingen. Ons centrum heeft aangetoond dat extracorporale fotoferese (ECP) mogelijk de gemiddelde fluorescentie-intensiteit van dnDSA's bij ontvangers met antilichaam-gemedieerde afstoting (AMR) kan verminderen, wat suggereert dat dit een veilige optie is voor de behandeling van subklinische AMR met dnDSA's en het induceren van tolerogene immunomodulatie in deze complexe populatie.
De hypothese die aan de voorgestelde gerandomiseerde gecontroleerde studie ten grondslag ligt, is dat ECP het potentieel zou kunnen hebben om de last van de novo donorspecifieke antilichamen na longtransplantatie te verminderen, door de humorale allorespons van de gastheer te moduleren en tolerantie te bevorderen, zonder bijwerkingen uit te lokken.
Ons primaire doel is om het therapeutische effect van ECP te evalueren in termen van reductie van de dnDSAs-titer bij klinisch stabiele patiënten met aanhoudende (>6 maanden) dnDSAs (MFI>1000). Als secundaire doelstelling zijn we van plan het therapeutische effect van ECP op de incidentie van klinische AMR, ACR, CLAD, de overleving en het optreden van infectieuze complicaties te beoordelen, evenals het aantal bijwerkingen. Onze experimentele doelstellingen zijn het verschaffen van een diepgaande analyse van de immunologische effecten van ECP.
80 patiënten met aanhoudende dnDSA's (> 3 maanden) met een MFI> 1000 zullen gerandomiseerd worden in twee groepen. Elke groep zal bestaan uit 40 patiënten. Patiënten zullen worden gestratificeerd op basis van de aanwezigheid van HLA-DQ. De controlegroep zal worden geobserveerd en er zal geen actieve behandeling worden toegediend. Dit is onze zorgstandaard in de bestudeerde klinische situatie. De behandelgroep krijgt extracorporale fotoferese. Ten eerste zal gedurende de eerste twee maanden eenmaal per twee weken een tweedaagse behandelcyclus worden uitgevoerd. Vervolgens wordt gedurende zes maanden eenmaal per maand een tweedaagse behandelingscyclus uitgevoerd. Om de specifieke mechanismen van ECP bij het moduleren van de humorale allorespons op te helderen, is een reeks onderzoeken gepland: 1) flowcytometrische immunofenotypering, 2) profilering van miRNA-expressie, 3) cytokine-expressie, 4) genexpressiesignatuur van PBMC's, 5) proteomische karakterisering.
De voorgestelde studie heeft tot doel deze klinische behoefte aan te pakken door de effecten van een veilige therapeutische modaliteit zoals ECP te onderzoeken. Deze studie kan een tweeledig voordeel hebben: ten eerste kan het de last van dnDSA's bij ontvangers van longtransplantaten verminderen, waardoor de incidentie van door antilichamen gemedieerde afstoting wordt verminderd, en ten tweede kan het de tolerantie van de gastheer voor het transplantaat bevorderen. Daarnaast zullen we de immuunmodulerende mechanismen van ECP onderzoeken in de context van humorale allogene respons, die nog niet eerder is onderzocht.

Experimenteel: Behandelingsgroep

Patiënten die gerandomiseerd zijn naar de interventionele groep zullen ECP ontvangen. ECP wordt binnen een week na randomisatie gestart.
In eerste instantie zal gedurende de eerste twee maanden eenmaal per twee weken een tweedaagse behandelingscyclus worden uitgevoerd.
Vervolgens wordt gedurende zes maanden eenmaal per maand een tweedaagse behandelingscyclus uitgevoerd.
