Un essai prospectif randomisé sur la PEC dans la RAM subclinique (EXPORT-DSA)
L'utilisation de la photophérèse extracorporelle comme thérapie immunomodulatrice du rejet subclinique médié par les anticorps après une transplantation pulmonaire : un ECR prospectif
Le but de cet essai clinique est d'évaluer l'effet thérapeutique de la photophérèse extracorporelle dans le rejet subclinique médié par les anticorps après une transplantation pulmonaire. Les principales questions auxquelles il vise à répondre sont :
- Le traitement par ECP entraîne-t-il une réduction significative du MFI (intensité moyenne de fluorescence) par rapport au MFI de base chez les patients cliniquement stables présentant des dnDSA persistants (> 6 mois) (MFI > 1 000) ?
- Quel est l'impact du traitement ECP sur les résultats suivants chez ces patients : ACR, AMR clinique, CLAD, infections, taux d'abandon, survie, événements indésirables ?
Les participants seront randomisés en deux groupes. Chaque groupe comprendra 40 patients. Le groupe témoin sera observé et aucun traitement actif ne sera administré. Le groupe de traitement recevra une photophérèse extracorporelle. Premièrement, un cycle de traitement de deux jours sera effectué une fois toutes les deux semaines pendant les deux premiers mois. Ensuite, un cycle de traitement de deux jours sera réalisé une fois par mois pendant 6 mois.
Les chercheurs compareront les deux groupes concernant : la valeur MFI, le développement de l'ACR, l'AMR clinique, le CLAD, les infections, la survie, les événements indésirables, l'immunophénotypage, le profilage de l'expression des miARN, l'expression des cytokines, la signature de l'expression génique des PBMC et la caractérisation protéomique.

Les anticorps spécifiques du donneur (DSA) et le rejet médié par les anticorps ont récemment attiré une attention particulière en raison de leurs conséquences graves, mais il existe des traitements efficaces limités pour les DSA persistants chez les patients sans dysfonctionnement du greffon en raison de leurs effets secondaires graves associés. Notre centre a démontré que la photophérèse extracorporelle (ECP) peut potentiellement réduire l'intensité moyenne de fluorescence des dnDSA chez les receveurs présentant un rejet médié par les anticorps (RAM), ce qui la suggère comme une option sûre pour traiter la RAM subclinique avec les dnDSA et induire une immunomodulation tolérogène dans cette population complexe.
L'hypothèse sous-jacente à l'essai contrôlé randomisé proposé est que la PEC pourrait avoir le potentiel de réduire le fardeau des anticorps spécifiques du donneur de novo après une transplantation pulmonaire, en modulant l'alloréponse humorale de l'hôte et en favorisant la tolérance, sans provoquer d'effets secondaires.
Notre objectif principal est d'évaluer l'effet thérapeutique de la PEC en termes de réduction du titre de dnDSA chez les patients cliniquement stables avec des dnDSA persistants (> 6 mois) (MFI> 1000). Comme objectif secondaire, nous avons l'intention d'évaluer l'effet thérapeutique de la PEC sur l'incidence de la RAM clinique, de l'ACR, du CLAD, de la survie et de la survenue de complications infectieuses ainsi que le taux d'effets indésirables. Nos objectifs expérimentaux sont de fournir une analyse approfondie des effets immunologiques de l'ECP.
80 patients présentant des dnDSA persistants (> 3 mois) avec un MFI > 1000 seront randomisés en deux groupes. Chaque groupe comprendra 40 patients. Les patients seront stratifiés selon la présence de HLA-DQ. Le groupe témoin sera observé et aucun traitement actif ne sera administré. C’est notre norme de soins dans la situation clinique étudiée. Le groupe de traitement recevra une photophérèse extracorporelle. Premièrement, un cycle de traitement de deux jours sera effectué une fois toutes les deux semaines pendant les deux premiers mois. Ensuite, un cycle de traitement de deux jours sera réalisé une fois par mois pendant 6 mois. Pour élucider les mécanismes spécifiques de l'ECP dans la modulation de l'alloréponse humorale, une série d'études sont prévues : 1) immunophénotypage cytométrique en flux, 2) profilage de l'expression des miARN, 3) expression des cytokines, 4) signature de l'expression génique des PBMC, 5) caractérisation protéomique.
L'étude proposée vise à répondre à ce besoin clinique en étudiant les effets d'une modalité thérapeutique sûre telle que la PEC. Cette étude pourrait avoir un double avantage : premièrement, elle pourrait réduire le fardeau des dnDSA chez les receveurs de transplantation pulmonaire, réduisant ainsi l'incidence du rejet médié par les anticorps, et deuxièmement, elle pourrait favoriser la tolérance de l'hôte au greffon. De plus, nous étudierons les mécanismes immunomodulateurs de la PEC dans le contexte de la réponse allogénique humorale, qui n'a jamais été étudiée auparavant.
