Geautomatiseerde cognitieve training: karakterisering van factoren die cognitieve verbetering bij verworven hersenletsel voorspellen

Om factoren te onderzoeken die cognitieve verbetering voorspellen na deelname aan een intensieve training van 6 maanden, 5 dagen per week, gebruikt u het ABI Wellness BEARS-platform en de Brainex Software Symbol Relations Module.
De studie zal de impact onderzoeken van intensieve werkgeheugentraining op neurocognitieve markers van hersenplasticiteit (interventiegerelateerde veranderingen) in 1) prestaties op neuropsychologische tests, 2) BDNF-niveaus in bloed en speeksel, 3) ERP-metingen van werkgeheugen, en 4 ) fMRI in rusttoestand en structurele MRI.

Beschrijving
Inclusiecriteria:
- Patiënt van het Mass General Brigham Health System met primaire aandachts- en executief functionerende problemen en/of een diagnose van milde cognitieve stoornis of milde neurocognitieve stoornis (niet-amnestisch profiel), als gevolg van verworven hersenletsel (ABI) dat minstens 12 maanden eerder is opgelopen contact te bestuderen.
- Leeftijden 25-65 jaar oud
- Vaardigheid in het Engels
- Bereid en in staat om alle studiegerelateerde activiteiten gedurende twaalf maanden te voltooien, inclusief reizen naar Brigham and Women's Hospital (Boston) voor vier persoonlijke beoordelingsbezoeken en twee serum- en speekselmonsterafnames.
- Toegang tot een computer met webcam en stabiel internet.
- Een betrouwbare onderzoeksinformant die op vier tijdstippen één vragenlijst kan invullen over de cognitie/dagelijks functioneren van de deelnemer.
Uitsluitingscriteria:
- Geschiedenis van alcohol- of middelenmisbruik of afhankelijkheid in de afgelopen twee jaar, volgens de DSM-5-criteria.
- Grote kans op een onderliggende progressieve neurodegeneratieve aandoening.
- Bewijs van een matige tot ernstige cognitieve stoornis, gebaseerd op een score van 21 of minder op het Mini-Mental Status Examination (MMSE) (Tombaugh & McIntyre, 1992).
- Patient Health Questionnaire (PHQ)-9 (Kroenke et al., 2010) Score ≥ 19, tenzij de behandelende arts oordeelt dat er geen sprake is van een actieve depressie (bijv. aanpassingsstoornis, rouwreactie).
- Actieve psychotische symptomen.
- Ernstige zintuiglijke verliezen waardoor het onwaarschijnlijk is dat deelnemers aan de studietraining zouden kunnen deelnemen, zelfs met substantiële aanpassingen (zelfrapportage van extreme moeite met het lezen van gewone krantenpapier of een op prestaties geteste gecorrigeerde visietestscore van slechter dan 20/30).
- Communicatieproblemen die de deelnemer ervan weerhouden effectief deel te nemen aan dit zeer interactieve onderzoeksprotocol (gebaseerd op de beoordeling door de interviewer van iemands vermogen om begrepen te worden en anderen te begrijpen).
- Huidige deelname aan een farmacologisch of ander interventioneel onderzoek.
- Levensverwachting van < 2 jaar.

Experimenteel: Geautomatiseerde cognitieve training

Deelnemers loggen vijf dagen per week (M-T-W-Th-F) in op het ABI Wellness Platform en trainen 45 tot 60 minuten met de module Symbol Relations.
Om de week ontmoeten de deelnemers hun trainingsbegeleider via zoom om de voortgang van de training te bekijken en eventuele trainingsgerelateerde vragen op te lossen.

Correlaties zullen worden onderzocht tussen fMRI-parameters, cognitieve en trainingsgegevens (bijv. NIH-toolbox Total Cognitive Composite, Fluid Intelligence Composite, Crystallized Composite, CCT-betrokkenheid en vooruitgang).
