Ontwikkeling van een niet-farmacologische pijninterventie voor thuiswonende oudere volwassenen met dementie
Het doel van dit onderzoek is het ontwikkelen van een oefenbehandelprogramma voor knieartrose dat goed werkt bij mensen met geheugenproblemen. Deze studie zal twee delen omvatten. Deelnemers kunnen ervoor kiezen om aan één of beide onderdelen deel te nemen.
In deel 1 nodigen de onderzoekers de volgende mensen uit om deel te nemen aan een interview:
- Personen van 60 jaar of ouder die niet in een verpleeghuis wonen en zowel geheugenproblemen als kniepijn hebben.
- Zorgpartners van mensen met kniepijn en geheugenproblemen. Een zorgpartner kan een familielid of vriend zijn die regelmatig meehelpt.
- Artsen (oefenfysiologen of revalidatietherapeuten zoals fysiotherapeuten en ergotherapeuten) die hebben gewerkt met oudere volwassenen met geheugenproblemen en kniepijn.
Het doel van deze interviews is om te leren van de ervaringen van mensen met geheugenproblemen en kniepijn en degenen die voor hen zorgen. Op basis van deze interviews hopen de onderzoekers een niet-medicamenteel, op maat gemaakt programma voor fysieke activiteit te ontwerpen dat interessant en nuttig zal zijn bij het beheersen van kniepijn bij oudere volwassenen met geheugenproblemen.
Voor mensen met geheugenproblemen en kniepijn en hun zorgpartners: De onderzoekers zullen enkele vragen stellen om te bepalen of ze in aanmerking komen voor deelname aan het onderzoek.
Degenen die in aanmerking komen voor het onderzoek zullen worden uitgenodigd om deel te nemen aan een interview dat online of persoonlijk kan worden afgenomen (Philadelphia, PA). Het interview kan maximaal 90 minuten (1,5 uur) duren en kan in één of meerdere sessies worden afgenomen.
Voor artsen: deelnemers nemen deel aan een focusgroep van 90 minuten met andere artsen, waarbij de lunch persoonlijk wordt geserveerd. Degenen die deelnemen aan een online focusgroep ontvangen een cadeaubon ter waarde van de lunch.
In deel 2 wordt het in deel 1 ontwikkelde programma getest. Het programma zal gebaseerd zijn op fysieke activiteit. Specifieke programmadetails voor deel 2 zullen worden verstrekt zodra deze beschikbaar komen. Deelnemers kunnen ervoor kiezen om zich voor één of beide onderdelen in te schrijven. Het aanmelden voor deel 1 betekent niet dat deelnemers zich ook moeten aanmelden voor deel 2.

Inclusiecriteria voor mensen met dementie:
- Leeftijd 60 jaar of ouder
- Spreekt Engels
- Milde/zeer milde dementie
- Kniepijn die ≥ 3 maanden aanhoudt, mogelijk gerelateerd aan artritis
- Kniepijn is gemiddeld minimaal 2 op 10.
Uitsluitingscriteria voor mensen met dementie:
- Verblijf in een instelling voor langdurige zorg
- Kan of wil geen toestemming geven
- Heeft een aandoening met een levensverwachting van minder dan 1 jaar
- Geen zorgpartner die ook wil meedoen
- Recente veranderingen in pijn of psychotrope medicijnen in de afgelopen 60 dagen
- Recente knieoperatie in het afgelopen jaar
- Heeft een aandoening die lichaamsbeweging gecontra-indiceerd heeft (alleen fase 2)
- Ontvangt momenteel revalidatiediensten (alleen fase 2)
Voor zorgpartners:
- Geen criteria op het gebied van geslacht, geslacht of leeftijd
- Kent een persoon met dementie ≥ 1 jaar
- Spreekt Engels
- In staat en bereid om toestemming te geven, inclusief opname
Voor artsen:

Experimenteel: Op maat gemaakt bewegingsprogramma voor pijn
Specifieke kenmerken van de interventie die in Fase 2 wordt geleverd, zullen gebaseerd zijn op de informatie die in Fase 1 van belanghebbenden is verzameld.

Aanpassing van fysieke activiteit als interventie voor pijnbestrijding met behulp van de driestappenaanpak op maat: in stap 1 de persoon met dementie (PLWD), in stap 2 de omgeving en in stap 3 de verzorger beoordelen.
Met deze aanpak kunnen traditionele oefeningen worden aangepast aan 1) functionele activiteiten (bijvoorbeeld traplopen) die zijn afgestemd op de interesses (bijvoorbeeld dansen, boksen) en behouden vaardigheden van PLWD; 2) geprefereerde en goed getolereerde omgevingen (bijvoorbeeld thuis); en 3) rekening houden met de mate en het type betrokkenheid van de interventionist en zorgverlener.

De haalbaarheidsmaatregel voor interventies is een gestandaardiseerde, zelfgerapporteerde uitkomstmaatstaf met vier vragen om de haalbaarheid te beoordelen.
Elk item wordt gescoord op een vijfpuntsschaal, waarbij 1 = helemaal niet mee eens, 2 = niet mee eens, 3 = niet mee eens of niet mee eens, 4 = mee eens, 5 = helemaal mee eens.
De antwoorden worden opgeteld en variëren van 5 tot 20, waarbij hogere scores wijzen op een grotere haalbaarheid.
Een score van 16 of hoger geeft aan dat het programma haalbaar is, 15 tot 9 geeft aan dat het programma mogelijk haalbaar is, en een score van 8 of minder suggereert dat het programma niet haalbaar is.
