De klinische proef in de PREVENT-studie is een patiëntgerandomiseerde controlestudie (RCT) van twee herinneringsinterventiearmen voor vaccinatie met het humaan papillomavirus (HPV), die zal plaatsvinden in landelijke klinieken die worden beheerd door Sea Mar Community Health Centers. De resultaten van de BootCamp Translation (BCT)-activiteiten zullen in het Engels en Spaans worden uitgevoerd en zullen de PREVENT RCT-ontwikkelings- en berichtstrategieën informeren; BCT-activiteiten maken geen deel uit van de RCT.
De PREVENT RCT zal een gerandomiseerd, gecontroleerd onderzoek met drie armen uitvoeren, waarin de voltooiing van de volgende benodigde dosis HPV-vaccinatie en de tijdige voltooiing van de reeks HPV-vaccins met meerdere doses worden beoordeeld. De onderzoeksarmen voor elke proef zullen bestaan uit geautomatiseerde herinneringen, geautomatiseerde en live herinneringen en de gebruikelijke zorg. Ouders/verzorgers van kinderen en adolescenten die voor het onderzoek worden geselecteerd, zullen worden gekozen op basis van vastgestelde onderzoekscriteria die worden toegepast op de elektronische medische dossiers die zijn gekoppeld aan staatsimmunisatieregisters. Omdat het om een onderzoek met een minimaal risico gaat, zullen de onderzoekers alleen voor de interventie een ontheffing van geïnformeerde toestemming aanvragen. De RCT zal worden uitgevoerd als onderdeel van de standaardzorg, en patiënten zullen zich er niet van bewust zijn dat ze aan het onderzoek deelnemen. De afgeleverde vaccinatieberichten omvatten opt-out-keuzes zoals voorgeschreven door BCT-activiteiten en lokaal beleid.
Afhankelijk van de specifieke interventies die tijdens de BCT zijn gedefinieerd, zullen ouders/verzorgers (P/C) van kinderen en adolescenten (C/A) een aantal herinneringen worden gestuurd om ouders aan te moedigen een HPV-vaccin te verkrijgen voor hun leeftijdsgeschikte C/A. Herinneringen kunnen bestaan uit sms-berichten, geautomatiseerde telefoongesprekken, brieven per post, live gesprekken of patiëntnavigatie. Herinneringen worden afgeleverd door een leverancier (geautomatiseerde herinneringen) of kliniekpersoneel (live herinneringen). Herinneringen worden afgeleverd in het Engels en Spaans, en er kunnen tolkdiensten worden gebruikt voor live herinneringen aan het kleine deel van de patiënten dat andere talen spreekt dan Engels of Spaans. De onderzoekers zullen elektronische medische dossiergegevens gebruiken om HPV-vaccinatiegebeurtenissen te documenteren als onze primaire uitkomst van interesse. De onderzoekers zullen ook het bereik voor elke interventiecomponent beoordelen, gedefinieerd als het aandeel patiënten dat een bepaalde interventiecomponent krijgt. De onderzoekers zullen tijdens het onderzoek ook de gemiste kansen beoordelen, gedefinieerd als het percentage patiënten dat andere aanbevolen vaccins krijgt (bijvoorbeeld Tdap of meningokokken), maar niet HPV, voor elke onderzoeksarm. Doel 3 zal kwalitatieve gegevens op patiënt- en zorgverlenerniveau verzamelen om de reactie op het programma, factoren die verband houden met de implementatie en duurzaamheid op lange termijn, en mogelijkheden voor aanvullende klinische interventies te beoordelen. Tijdens actieve onderzoeksrekrutering zullen de onderzoekers elke zes maanden een vergadering van onze monitorraad voor gegevensveiligheid bijeenroepen. De onderzoekers zullen onderzoeksresultaten en onderzoeksproducten verspreiden in overeenstemming met ons verspreidingsplan. De bevindingen van het onderzoek zullen dienen als basis voor een groter onderzoek op meerdere niveaus naar HPV-vaccinatie in plattelandsgemeenschappen.

Inclusiecriteria:
- Ouders/verzorgers (P/C) van kinderen/adolescenten (C/A) in de leeftijd van 9 tot 17 jaar (d.w.z. die in aanmerking komen voor HPV-vaccinatie);
- P/C met actieve kliniekpatiënten (d.w.z. die in de afgelopen 12 maanden in de kliniek zijn gezien); En
- P/C die Engels of Spaans spreekt.
Uitsluitingscriteria:
- P/C of C/A met eerdere, uitgesloten HPV-vaccinatiegeschiedenis (bijvoorbeeld voltooide vaccinatie of niet gepland);
- P/C of C/A met klinische aandoeningen die de CDC HPV-vaccinatieaanbevelingen beïnvloeden (bijv. zwangerschap);
- P/C of C/A met andere factoren die de CDC HPV-aanbevelingen zouden beïnvloeden; En
- P/C die geen Spaans of Engels spreekt.

Experimenteel: Engelse taalsessie en berichtenuitwisseling
De persoonlijke of live Zoom-presentatie in het Engels zal bestaan uit een expertpresentatie over het onderzoek naar de veiligheid en effectiviteit van het HPV-vaccin en sociale normen over vaccinatie. Tijdens deze sessie ontvangen de deelnemers actueel bewijsmateriaal over effectieve berichtgeving door artsen (bijvoorbeeld door het gebruik van vermoedelijke uitspraken, het bundelen van HPV-vaccinaanbevelingen met aanbevelingen voor andere vaccins) naast nieuwe formaten voor educatieve berichten (bijvoorbeeld digitale video’s, verhalen ). Deelnemers aan de Engelstalige sms-groep zullen gedurende een periode van drie maanden iteratieve feedback geven aan het onderzoeksteam over HPV-vaccinatieberichten. |

Het maakt gebruik van een iteratief, flexibel schema van persoonlijke ontmoetingen, gecombineerd met korte, gerichte teleconferenties. BCT benadrukt geen foute antwoorden en geen hiërarchie van expertise. Het proces vereist maximaal negen (9) uur deelnemerstijd gedurende 3-6 maanden. Deelnemers worden voor en na de persoonlijke sessie ondervraagd om de leerresultaten te beoordelen en fundamentele sociodemografische informatie vast te leggen. Het BCT-proces zal het formaat (modus en frequentie) en de inhoud van berichten definiëren om de voltooiing van vaccinseries te bevorderen. Een lage score (minimum: 1 extreem onwaarschijnlijk) is een ongunstige uitkomst en een hoge score (maximaal: 5 uiterst waarschijnlijk) is een gunstig resultaat. Deze uitkomstmaat zal het aantal patiënten per reactie melden voor de BCT -Engelse sessies en de BCT -Spaanse sessies. |

Een lage score (minimum: 1 helemaal niet tevreden) is een ongunstige uitkomst en een hoge score (maximaal: 10 extreem tevreden) is een gunstig resultaat. Deze uitkomstmaat zal de gemiddelde score van deelnemers rapporteren voor de BCT -Engelse sessies en de BCT Spaanse sessies. |
