Voorspellen van de overleving na chirurgische resectie voor het gehele spectrum van anatomisch reseceerbare HCC: een Metroticket-aanpak
Klinische uitkomsten na chirurgische resectie bij HCC zijn een continuüm en houden duidelijk verband met de tumorlast, maar behoeven een betere definitie. De onderzoekers beschrijven het gebruik van de 'metroticket'-benadering om chirurgische resultaten over het hele spectrum van anatomisch reseceerbare HCC te analyseren om de algehele overleving te definiëren, inclusief tumoren in het tussenstadium (BCLC B). De analyse die de onderzoekers in dit onderzoek geven, stelt de arts in staat de optimale chirurgische resectiekandidaat te selecteren op basis van robuuste overlevingsgegevens op lange termijn.
Bovendien vergelijkt de studie de resultaten voor open chirurgie versus laparoscopische chirurgie, en de overlevingsresultaten voor virale en niet-virale HCC met behulp van albumine-bilirubine (ALBI) voor een uitgebreider studieresultaat.

Patiënten die van 1 januari 2000 tot 30 juni 2018 een chirurgische resectie (zowel open chirurgie als laparoscopische chirurgie) voor HCC hebben ondergaan door de gezamenlijke hepato-pancreato-galchirurgiedienst van de Singapore Healthcare Group of Hospitals (Singapore General Hospital en het National Cancer Center Singapore ) werden met terugwerkende kracht geïdentificeerd uit een prospectief bijgehouden institutionele database.
De beoordeling van de grootte, het aantal knobbeltjes en de vasculaire invasie van HCC was gebaseerd op preoperatieve CT/MRI-beeldvorming en pathologische beoordeling van de gereseceerde monsters. Alle resecties werden histologisch bevestigd als HCC. Patiënten met macrovasculaire invasie en extrahepatische invasie werden uitgesloten van de analyse, net als patiënten die werden gereseceerd voor andere palliatieve doeleinden (bijv. gescheurde HCC).

Inclusiecriteria:
- Ondubbelzinnige diagnose van hepatocellulair carcinoom (HCC) door histologie
- Patiënten die een chirurgische resectie ondergingen vanwege HCC
Uitsluitingscriteria:
- Patiënten die een leverresectie ondergingen vanwege andere maligniteiten, zoals cholangiocarcinoom
- Patiënten met HCC die geen leverresectie hebben ondergaan.
- Patiënten met HCC die een levertransplantatie hebben ondergaan.

Bivariate contourgrafieken van de overlevingskans na 5 jaar als functie van X = AFP-concentratie (ng/ml) en Y = tumorlastindex (grootte van de grootste knobbel + 2*aantal knobbeltjes) (TBI) werden verkregen als afgevlakte contouren afkomstig van een empirisch afgeleide bivariate 5-jaarsoverlevingsverdeling S(X,Y)=1- F(x ≤X,y ≤Y).
De functie F(x ≤X,y ≤Y) is de empirische, bivariate cumulatieve verdeling van de 5-jaarssterfte als functie van X en Y gedefinieerd over het domein (0<X≤5000,1≤Y≤15) in onze patiëntencohort.
Alle patiënten in ons studiecohort werden gedurende ten minste 5 jaar gevolgd na de respectieve operatie, waardoor een schatting van de werkelijke sterfte na 5 jaar mogelijk werd gemaakt.

De modellering van de metrokaartjes was gebaseerd op de tumorgrootte en het aantal knobbeltjes dat werd geëxtraheerd bij preoperatieve radiologische beeldvorming en postchirurgische pathologische beoordeling.
Continue variabelen worden samengevat als gemiddelde, standaarddeviatie, 25e en 75e percentiel, mediaan en minimum en maximum.

Er wordt gebruik gemaakt van een Bland-Altman-analyse, die is gebaseerd op een eenvoudige maar effectieve grafische benadering voor het evalueren van bias en voor het berekenen van grenzen aan de verwachte omvang van de individuele verschillen die tussen de twee meetmethoden optreden.
Er werd een identiteitslijn van 45° met spreidingsdiagram gemaakt om de correlatie tussen de twee metingen weer te geven.
De bias werd geëvalueerd door (d) ̅ het gemiddelde verschil van de metingen te berekenen.
Grenzen van overeenstemming, berekend als (d) ̅ ±2sd, waarbij sd de standaardafwijking van de individuele verschillen is, gedefinieerde grenzen waarbinnen naar verwachting 95% van de meetverschillen zullen vallen.
De B-A-plot definieert de grenzen van overeenkomsten.
B-A-analyse wordt uitgevoerd met behulp van TBI over een breed bereik, 0 ≤ TBI ≤ 30, en een beperkt bereik gericht op kleine tumoren, 0 < TBI ≤ 6.
