Evaluatie van de effectiviteit van hormonale behandeling bij adolescenten die lijden aan genderdysforie (TRANSADO)
Geslachtsdysforie (GD) is een aanzienlijk lijden dat meer dan zes maanden aanhoudt bij een persoon, met betrekking tot de kloof die wordt gevoeld tussen de genderidentiteit van iemand en het geboortegeslacht. Vanaf het begin van de puberteit zullen de meeste van deze adolescenten die zich als transgender identificeren in deze zin volhouden en op een gegeven moment een hormonale of chirurgische herschikking ondergaan. Voor deze adolescenten bevelen internationale aanbevelingen voor goede praktijken een vroege behandeling aan aan het begin van de puberale ontwikkeling (Tanner 2) om de progressie van de puberteit te blokkeren, met de mogelijkheid van hormonale transitie door toediening van geslachtshormonen van het gewenste geslacht op 16-jarige leeftijd. Om de psychosociale gevolgen van GD te verminderen, oefenen steeds meer referentieteams deze hormonale transitie echter uit vanaf de leeftijd van 14 jaar, zonder dat enig gerandomiseerd onderzoek het voordeel ervan aantoont in vergelijking met een transitie op de leeftijd van 16 jaar. Zonder behandeling is de comorbiditeit onder adolescenten die lijden aan genderdysforie zeer hoog, met angst en depressie, suïcidaal risico en schooluitval.
Onze hypothese is dat wanneer de hormonale transitie wordt gestart op een leeftijd die dichter bij de fysiologische puberteit ligt, dit de comorbiditeiten zal verminderen en de kwaliteit van leven van deze adolescenten zal verbeteren. Dit is het eerste therapeutische gerandomiseerde onderzoek in Frankrijk naar deze transgender adolescentenpopulatie, een veld waar internationale aanbevelingen gebaseerd op de principes van ‘Evidence Based Medicine’ angstaanjagend zijn. We hopen dat de resultaten van dit onderzoek een leidraad zullen zijn voor de transgender jeugdgezondheidszorg, waardoor ze, als het onderzoek positief is, eerder toegang krijgen tot hormonale behandelingen en hun algehele functioneren, hun angst- en depressieve symptomen en hun kwaliteit van leven kunnen verbeteren.

Beschrijving
Inclusiecriteria:
- Adolescenten van 14 +/- 6 maanden,
- In de puberteit zijn gekomen: Tanner-score ≥2 voor FtM; leerlooierscore ≥2 en/of testosteron >0,3 ng/ml voor MtF
- het presenteren van de criteria voor genderdysforie volgens de DSM5, beoordeeld door ten minste twee kinderpsychiatrische interviews met een tussenpoos van ten minste zes maanden, waarbij de diagnose van genderdysforie klinisch was vastgesteld en die van een geassocieerde autismespectrumstoornis werd weerlegd en/of een geassocieerde cognitieve stoornis, bevestigd door specifieke schalen (Gender Identity / Gender Dysphoria Questionnaire for Adults and Adolescents (GIDYQ-AA), en Recalled Childhood Gender Identity Scale),
- wiens indicatie voor hormonale transitie is gevalideerd in een multidisciplinair overleg na minimaal één consult bij de kinderendocrinoloog met klinisch onderzoek, bloeddrukmeting en informatie over hormonale behandelingen in de context van genderdysforie.
Uitsluitingscriteria:
- contra-indicatie voor experimentele hormonale behandeling (zie paragraaf 1.5)
- Noodzaak van aanpassing van de hormonale behandeling (FtM-patiënt behandeld met anticoagulantia of met trombofilie).
- Patiënt met een risico op verergering van bepaalde ziekten onder behandeling met oestrogeen (MtF-patiënten met ongecontroleerde diabetes met een HBA1C > 8%, patiënten met cholelithiasis, cholelithiasis, systemische lupus erythematosus, ernstig astma, ernstige arteriële hypertensie, ernstige migraine, otosclerose, epilepsie die niet onder controle is behandeling.
- Patiënten met kanker met een risico op hypercalciëmie (en bijbehorende hypercalciurie), gekoppeld aan botmetastasen.
- Ernstig hart-, lever- of nierfalen of ischemische hartpathologieën, vanwege het risico op ernstige complicaties gekenmerkt door oedeem, met of zonder congestief hartfalen.
- Ongecontroleerde hoge bloeddruk.
- Patiënten die lijden aan epilepsie en migraine.
- Patiënten met een bestaan of een voorgeschiedenis van trombo-embolische ongevallen.
- Onbehandelde ernstige chronische depressie Voortdurende behandeling met anticoagulantia
- Voortdurende behandeling met vitamine K-antagonist
- autismespectrumstoornissen (klinische screening, bij twijfel bevestigd door de Social Responsiveness Scale (SRS) > 65e percentiel, uitgevoerd als onderdeel van de gebruikelijke zorg in geval van een klinisch argument,
- Cognitief tekort (klinische screening, bevestigd door een IQ < 80 op de schaal van Weschler (WISC IV), uitgevoerd als onderdeel van de gebruikelijke zorg in geval van een klinisch argument.
- weigering om deel te nemen aan het onderzoek vanwege de adolescent of een van de dragers van het ouderlijk gezag (de twee dragers en de adolescent moeten schriftelijke toestemming ondertekenen nadat zij de juiste informatie hebben ontvangen).

The Children's Global Assessment Scale (CGAS), adapted from the Global Assessment Scale for adults, is a rating of general functioning for children and young people until 16 years old.
The clinician assesses a range of aspects of psychological and social functioning and gives the child or young person a single score between 1 and 100, based on their lowest level of functioning.
The score puts them in one of ten categories that range from "needs constant supervision" (1-10) to "superior functioning" (91-100).
The measure can be used by clinicians as well as researchers to complement other scales measuring more specific symptoms.
