Perifere aderziekte (PAD) is een vorm van hart- en vaatziekten die de slagaders aantast die de bloedtoevoer naar de benen verzorgen. De prevalentie van PAD neemt snel toe in de Verenigde Staten. Wanneer PAD ernstig is, kan het pijn veroorzaken bij beweging en het vermogen van een persoon beperken om te lopen of zijn normale dagelijkse activiteiten uit te voeren. Bij ernstige PAD wordt vaak een procedure uitgevoerd waarbij een slagader open wordt gehouden met een buisje dat een stent wordt genoemd.
Het is gebleken dat programma's voor op oefeningen gebaseerde revalidatie deze patiënten enorm helpen in termen van de pijn die zij ervaren bij het lopen, hun vermogen om dagelijkse activiteiten uit te voeren of de activiteiten die vereist zijn voor het werk, en een verbeterde kwaliteit van leven. Er is ook een nieuwere vorm van therapie, prehabilitatie genaamd, waarvan is aangetoond dat deze belangrijke voordelen heeft voor patiënten met PAD. Prehabilitatie omvat vier tot zes weken oefentherapie en leefstijladviezen voordat een stentprocedure wordt ondergaan. Prehabilitatie helpt patiënten de inspanningstolerantie te verbeteren en levensstijlveranderingen aan te brengen die de risicofactoren vóór de procedure verbeteren. Degenen die deelnemen aan prehabilitatieprogramma's hebben minder complicaties bij de stentprocedure, brengen minder dagen door in het ziekenhuis en hebben een beter vermogen om te oefenen en sneller weer aan het werk te gaan.
Hoewel de voordelen van prehabilitatie en revalidatie algemeen bekend zijn, verzuimen de meeste artsen deze behandelingen aan te bevelen aan hun patiënten die een stentprocedure voor PAD ondergaan. Het onderzoeksteam gaat de impact van prehabilitatie, revalidatie of beide vormen van therapie bestuderen. De resultaten zullen waarschijnlijk meer artsen aanmoedigen om de beste optie voor hun patiënten aan te bevelen, en mogelijk zowel prehabilitatie als revalidatie aan te bevelen bij patiënten die een stentprocedure ondergaan.

Beschrijving
Inclusiecriteria:
- Mannelijke of vrouwelijke volwassene met de diagnose perifeer vaatlijden (PAD)
- Pijn in de benen ervaren tijdens het lopen
- Gepland voor een stent voor PAD
- Veilig kunnen sporten
- Postmenopauzaal en niet in de vruchtbare leeftijd
Uitsluitingscriteria:
- Medisch instabiele patiënten
- Recent MI binnen 3 maanden
- Maligniteit
- Ongecontroleerde diabetes mellitus (HBA1C ≥8 mmol/l)
- Alcoholisme of ander recreatief drugsgebruik

Experimenteel: Prehabilitatie
Proefpersonen die gerandomiseerd zijn naar de prehabilitatiegroep zullen 6 weken geïndividualiseerde oefentherapie ondergaan, inclusief een aanloopperiode van 1 week (3 sessies) uitgevoerd in de VAPAHCS-rehabilitatiefaciliteit.
De proefpersonen zullen dan voornamelijk thuis oefenen, waarbij een oefensessie in de kliniek wekelijks wordt herhaald gedurende de duur van hun deelname.
Deze sessies zullen geïndividualiseerd worden op basis van tolerantie en in overeenstemming met gestandaardiseerde richtlijnen, maar zullen over het algemeen bestaan uit 45-60 minuten begeleide oefeningen die aërobe en weerstandstraining combineren, inclusief warming-up en cool-down.
Tijdens het onderzoek wordt aan de proefpersonen gevraagd om dagelijks 30 minuten zonder toezicht aerobe oefeningen naar keuze uit te voeren, met de nadruk op het verbeteren van de loopprestaties thuis.
De richtlijnen voor het monitoren, de veiligheid en het voorschrijven van patiënten, opgesteld door de American Heart Association, American College Sports Medicine en de American Association of Cardiovascular and Pulmonary Rehabilitation, zullen worden gevolgd.

Actieve vergelijker: Rehabilitatie
Proefpersonen die gerandomiseerd zijn naar de revalidatiegroep zullen ongeveer twee weken na de endovasculaire interventie met oefensessies beginnen, zoals beschreven in de richtlijnen voor PAD.
De stabiliteit van de patiënt en de doorgankelijkheid van de wond zullen van het grootste belang zijn voordat een patiënt aan het revalidatieprogramma begint.
Net als bij prehabilitatie zullen de proefpersonen een kennismakingsperiode van 1 week en 3 sessies ondergaan, uitgevoerd in de VAPAHCS-rehabilitatiefaciliteit, en daarna één keer per week terugkeren voor sessies onder toezicht in de kliniek.
De wekelijkse persoonlijke bezoeken zullen worden uitgevoerd in het revalidatiecentrum om naleving te monitoren en aan te moedigen, vragenlijsten over activiteiten te beoordelen, accelerometriegegevens te downloaden en eventuele klinische problemen aan te pakken.
Net als bij deelnemers aan de prehabilitatiegroep zal de revalidatieperiode van zes weken geïndividualiseerd zijn en gestandaardiseerde richtlijnen volgen die zijn ontworpen voor revalidatie bij patiënten met PAD.

De waargenomen barrières en voordelen voor fysieke activiteit zullen worden beoordeeld met behulp van twee methoden: een semi-gestructureerd interview en toediening van de Exercise Benefits and Barriers Scale (EBBS), om zowel open kwalitatieve als kwantitatieve gegevens over deze constructen te verkrijgen.
De EBBS kan in zijn geheel of als twee afzonderlijke weegschalen worden gebruikt.
Over het geheel genomen varieert de score voor EBBS van 43 tot 172, waarbij een hogere score is omdat het individu inspanning positiever ervaart.
Wanneer de voordelenschaal alleen wordt gebruikt, ligt het scorebereik tussen 29 en 116, waarbij hoe hoger de score is, hoe het individu de oefening positiever ervaart.
Wanneer de Barriers Scale alleen wordt gebruikt, ligt het scorebereik tussen 14 en 56.
