Mirikizumab bij de behandeling van chronische ontstekingsaandoeningen van de zak
Het doel van deze klinische proef is om te leren of Mirikizumab werkt om zakdoorlingen bij volwassenen te behandelen. De belangrijkste vragen die het wil beantwoorden zijn:
Vermindert mirikizumab de symptomen van zakdoorlingen
Deelnemers zullen:
Neem Mirikizumab om de 4 weken gedurende een jaar om de 4 weken eenmaal per maand naar de kliniek gedurende twee maanden en bij het einde van het onderzoek bewaar een dagboek van hun symptomen

Beschrijving
Inclusiecriteria:
- Geïnformeerde toestemming zal worden verkregen vóór eventuele onderzoeksprocedures
- Leeftijd>/= 18 en </ = 80 jaar
Diagnose van chronische pouchitis of Crohn's-achtige ziekte van het zakje op basis van de volgende criteria:
- Chronische antibiotica-afhankelijke pouchitis: terugkerende symptomen van pouchitis (frequentie, urgentie, bloedingen, buikpijn of kramp, of bekkenkampioenschap) die reageren op antibioticatherapie maar terugvallen kort na het stoppen van antibiotica, waardoor continue antibiotische therapie nodig is om symptomcontrole te bereiken.
- Chronische antibiotica refractaire pouchitis: gebrek aan respons op standaard antibioticatherapie, vereiste van een langere duur van antibioticatherapie dan verwacht met minimale verbetering van de symptomen, in samenhang met typische symptomen van pouchitis (frequentie, urgentie, bloeding, buikpijn of kramp, of pelvisch stoming).
- Crohn's-achtige ziekte van de zak: aanwezigheid van een perianal of andere fistel die zich minstens 12 maanden ontwikkelde na de laatste fase van de ileale zak Anal anastomosis (IPAA) chirurgie, strictuur van het zakje of pre-pouch ileum, of de aanwezigheid van pre-pouch ileitis. Zaklichaamsontsteking (pouchitis) kan vaak naast elkaar bestaan in de setting van Crohn's-achtige ziekte van het zakje.
- Deelnemers met een bewezen geschiedenis van colitis ulcerosa en geschiedenis van 1,2, gemodificeerde -2 of 3 -fase IPAA en ileostomie takedown
- Mogelijkheid om toegang te krijgen tot internet voor elektronische database -invoer
- Alleen die personen voor wie een provider overweegt om mirikizumab te starten voor de behandeling van chronische pouchitis of Crohn's-achtige ziekte van het zakje komt in aanmerking voor deelname.
Uitsluitingscriteria:
- Voorafgaande blootstelling aan mirikizumab
- Bekende overgevoeligheid voor Mirikizumab of zijn metabolieten
- Huidige infectie met Clostridioides difficile
- Bekende hiv of actieve hepatitis b/c
- Klinisch significante leverziekte (primaire scleroserende cholangitis met LFT's <1,5 bovengrens van normaal kan worden opgenomen)
- Ernstige leverstoornissen, gedefinieerd als Child-Pugh-klasse C
- Bekende verminderde nierfunctie met een glomerulaire filtratiesnelheid <45 ml/min/1.732
- Geschiedenis van maligniteit, behalve voor basaalcelcarcinoom, niet-gemetastatische plaveiselcelcarcinoom van de huid, of eerdere maligniteit met curatieve therapie ten minste 5 jaar voorafgaand aan screening en geen herhaling.
- Klinisch significante laboratoriumresultaten bij screening of basislijn, zoals beoordeeld door de onderzoeker van lokale testen.
- Vrouw die zwanger is, borstvoeding geeft of van plan is zwanger te worden of van een kinderdrevend potentieel is en geen zeer effectieve anticonceptiemethode gebruikt.
- Deelname aan een klinische proef van een goedgekeurd of niet-goedgekeurd onderzoek van onderzoek binnen 30 dagen vóór screening.
- Elke aandoening, die naar de mening van de onderzoeker de veiligheid of naleving van het protocol van de deelnemer in gevaar kan brengen.

Remissie zoals gemeten door gemodificeerde Pouchitis Disease Activity Index (MPDAI)

De Pouchitis Disease Activity Index (PDAI) werd ontwikkeld voor het diagnosticeren van actieve pouchitis en het kwantificeren van de ernst van pouchitis.
De PDAI omvat de klinische criteria van 1) ontlastingsfrequentie 2) rectale bloeding 3) fecale urgentie/buikkrampen en 4) aanwezigheid of afwezigheid van koorts, endoscopische criteria en histologische criteria door biopsieën.
De PDAI werd aangepast om de kosten van biopsie en histologie weg te laten.
De MPDAI omvat het klinische gedeelte en de endoscopische subscore van de PDAI.
Scorebereik is 0-12.
Hoe hoger de score, hoe slechter de ziekteactiviteit.
MPDAI -remissie wordt gedefinieerd als een MPDAI -score van 4 of lager en een reductie ten opzichte van de basislijn van 2 of meer punten in de totale score van MPDAI.

De endoscopische zakscore (EPS) is een gestandaardiseerde beoordeling van ontstekingen in het zakje van patiënten met een ileale zakanalanastomose (IPAA), inclusief die met ontsteking buiten het zaklichaam.
De EPS is een nieuwe methode voor endoscopische scoren die is ontwikkeld om meer gedetailleerde longitudinale beoordelingen van respons te bieden op geavanceerde therapieën bij patiënten met inflammatoire aandoeningen van het zakje.
Scorebereik is 0-45.

De Global Quality of Life Scale van Cleveland vraagt de patiënt om hun huidige QOL, de huidige kwaliteit van de gezondheid en het huidige energieniveau te beoordelen met behulp van een 1-10 rating (waar 10 het beste is).
De score op elk van deze 3 componenten wordt toegevoegd en de laatste Global Quality of Life Utility Score van Cleveland Clinic kan worden verkregen door dit resultaat met 30 te delen.
Een hogere score betekent verbeterde kwaliteit van leven.
