Het doel van deze studie is om de werkzaamheid en veiligheid van chidamide, regorafenib te beoordelen in combinatie met iParomlimab en tuvonralimab voor de behandeling van geavanceerde colorectale kanker in derde lijntherapie en daaropvolgende lijnen.
Een verkennend onderzoek naar de combinatie van chidamide, regorafenib, plus iParomlimab en tuvonralimab voor de behandeling van geneesmiddel-esistent en efractory dvanced colorectale kanker
Patiënten met gevorderde colorectale kanker, met name die van het PMMR/MSS-type, vertonen suboptimale therapeutische reacties in de derde lijn en daaropvolgende behandelingen. De werkzaamheid van de bestaande standaard behandelingsregimes is sterk beperkt. In de juiste studie was de mediane algehele overleving (OS) van regorafenib 6,4 maanden, de mediane progressievrije overleving (PFS) was 1,9 maanden en het objectieve responspercentage (ORR) was 1%. In de Concur -studie gericht op de Aziatische bevolking was het mediane besturingssysteem van de Regorafenib -groep 8,8 maanden, de mediane PFS was 3,2 maanden en de ORR was 4%. In de Fresco -studie was het mediane besturingssysteem van Fruquintinib 9,3 maanden, de mediane PFS was 3,7 maanden en de ORR was 4,7%. In de Terra-studie was het mediane besturingssysteem van trifluridine/tipiracil (TAS-102) 7,8 maanden, de mediane PFS was 2,0 maanden en de ORR was 1,1%. De zonlichtstudie onderzocht de gecombineerde behandelingsmodaliteit van TAS-102 plus bevacizumab, waar het mediane besturingssysteem van het gecombineerde regime 10,8 maanden was, de mediane PFS 5,6 maanden was en de ORR 6,1%was. Het is duidelijk dat de therapeutische werkzaamheid van geavanceerde colorectale kanker in de derde lijn en daarbuiten is gepleiteerd. In de afgelopen jaren heeft fundamenteel onderzoek ontdekt dat epigenetische regulatie aanzienlijk synergeteert met anti-angiogenese en immuuncontrolepuntremmingstherapie. Kleinschalige klinische verkenningen hebben ook gunstige werkzaamheid en klinische vooruitzichten aangegeven, die verder onderzoek rechtvaardigen. Daarom overwegen we de combinatie van chidamide, regorafenib en iParomlimab/tuvonralimab te gebruiken om de werkzaamheid en veiligheid ervan te onderzoeken bij de behandeling van geavanceerde colorectale kanker in de derde lijn en daarna, en om nieuwe doorbraak te verkennen voor refractaire colorectale kanker na meerdere lijnen van de behandeling.
Het doel van deze studie is om de werkzaamheid en veiligheid van het combinatieregime van chidamide, regorafenib en iParomlimab/tuvonralimab te beoordelen bij patiënten met gevorderde colorectale kanker in de derde lijn en daarna. De studie zal worden uitgevoerd in het Shanghai Changzheng Hospital. De studie -medicijnen, waaronder chidamide, regorafenib en iParomlimab/tuvonralimab, zijn allemaal commercieel verkrijgbaar in China.

Beschrijving
Inclusiecriteria:
- Patiënten met geavanceerd colorectaal adenocarcinoom bevestigd door histologie of cytologie;
- Degenen die ziekteprogressie hebben meegemaakt of intolerant zijn voor de behandeling na het ontvangen van ≥ 2 regels systemische standaardtherapie;
- Patiënten moeten ten minste één meetbare laesie hebben (volgens RECIST 1.1 -criteria);
- Patiënten die ten minste 3 weken vóór de eerste medicijnbehandeling lokale radiotherapie hebben ontvangen, komen in aanmerking voor inclusie; De door RECIST geëvalueerde laesies mogen echter niet binnen het radiotherapieveld zijn;
- Patiënten moeten minstens 18 jaar oud zijn;
- Prestatiestatus ECOG-score van 0-1;
- Verwachte overleving ≥ 12 weken;
- Patiënten moeten het in staat zijn om het schriftelijke geïnformeerde toestemmingsformulier te begrijpen en vrijwillig te ondertekenen;
- Vrouwen in de vruchtbare leeftijd moeten binnen 7 dagen vóór het begin van de behandeling een negatieve zwangerschapstest hebben. Tijdens de studieperiode moeten zowel de patiënt als de echtgenoot van de patiënt anticonceptiemaatregelen nemen.
Uitsluitingscriteria:
- Patiënten die een grote operatie hebben ondergaan of binnen 4 weken vóór de eerste dosis van het studiemedicijn ernstig trauma hebben ondergaan;
- Patiënten met overgevoeligheid voor elk onderdeel van het studieprotocol;
- Patiënten die van plan zijn zwanger te worden of die zwanger zijn;
- Patiënten met hersenmetastasen en een onvermogen om hun toestand nauwkeurig te beschrijven;
- Patiënten die binnen 6 maanden voorafgaand aan het begin van de studiebehandeling een van de volgende ziekten hebben meegemaakt: een hartinfarct, ernstige/onstabiele angina pectoris, NYHA Klasse 2 of hoger congestief hartfalen, ongecontroleerde aritmie, enz.;
- Abnormale laboratoriumtests:
Absolute neutrofielentelling (ANC) <1.500/mm3; Ploedplaatjestelling <75.000/mm3; Totaal bilirubine> 1,5 keer de bovengrens van normaal; Alanine aminotransferase (ALT) en aspartaat aminotransferase (AST)> 2,5 keer de bovengrens van normaal (voor patiënten met levermetastasen> 5 keer de bovengrens van normaal); Creatinine> 1,5 keer de bovengrens van normaal;
- Patiënten die andere kanker (behalve gevorderde colorectale kanker) hebben gehad binnen 5 jaar voorafgaand aan het begin van deze onderzoeksbehandeling. Cervicaal carcinoom in situ, genezen basale celcarcinoom en blaas epitheliale tumoren zijn uitgesloten;
- Patiënten met een geschiedenis van drugsmisbruik, drugsverslaving of alcoholafhankelijkheid;
- Patiënten zonder wettelijke capaciteit of met beperkte civiele capaciteit;
- Andere voorwaarden die de onderzoeker ongepast achten voor opname in het onderzoek.

Experimenteel: Het combinatieregime van chidamide, regorafenib en iParomlimab/tuvonralimab
|
Alle patiënten werden behandeld met het volgende regime: Chidamide: 30 mg per dosis, twee keer per week; Regorafenib: 80 mg per dosis, eenmaal per dag, oraal gedurende 3 weken, gevolgd door een rust van 1 weken en om de 4 weken herhaald; IPAROMLIMAB/TUVONRALIMAB: 5 mg/kg, eens in de 3 weken, intraveneuze infusie.
