Transcraniële echografiebeoordeling van cerebrale pulsatiliteit bij de besluitvorming over shunts voor vermoedelijke idiopathische normotensieve hydrocefalie: een prospectieve cohortstudie (HYDROPULSE)
Bijdrage van de cerebrale pulsatiliteitsevaluatie via transcraniële echografie aan de besluitvorming voor cerebrospinale vloeistofdrainage bij patiënten met vermoedelijke idiopathische normotensieve hydrocefalie: een prospectieve cohortstudie
Dit zal een enkelcentrum, prospectieve, analytische cohortstudie zijn. Alle geïncludeerde patiënten zullen transcraniële echografie (TPI) ondergaan met meting van cerebrale pulsatiliteitsparameters vóór subtractieve lumbaalpunctie tijdens opname op de neurochirurgieafdeling, gevolgd door een tweede echo-evaluatie ofwel één jaar na de operatie voor geopereerde patiënten, ofwel één jaar na de initiële echo-evaluatie voor niet-geopereerde patiënten. Deze echo-evaluatie zal worden geïntegreerd in de routinematige werkwijze, die al systematisch wordt uitgevoerd bij verdenking op idiopathische normale druk hydrocefalus (iNPH).
De beslissing van het neurochirurgische team om cerebrospinale vloeistof (CSV) drainering uit te voeren zal blind worden genomen voor de echometingen en uitsluitend gebaseerd op het standaard klinische protocol.
Het primaire doel van de studie is om aan te tonen dat de gemiddelde amplitude van cerebrale pulsatiliteit (MeanBTP) vóór subtractieve lumbaalpunctie significant hoger is bij patiënten die klinische verbetering vertonen 12 maanden na CSV-drainering vergeleken met diegenen die dat niet doen, met een type I-foutrisico (alfa) van 0,05.

Gedetailleerde beschrijving
Dit zal een prospectieve, single-center cohortstudie zijn waarbij patiënten met vermoedelijke idiopathische normale druk hydrocephalus (iNPH) worden geïncludeerd. In aanmerking komen vereist het voldoen aan de inclusiecriteria en bevestiging van niet-tegenwerking na schriftelijke geïnformeerde toestemmingsprocedures.
Alle patiënten zullen transcraniële pulsbeeldvorming (TPI) ondergaan met meting van cerebrale pulsatiliteitsparameters vóór subtractieve lumbaalpunctie tijdens de ziekenhuisopname. Geopereerde patiënten zullen een tweede TPI-beoordeling krijgen 12 maanden na de operatie, terwijl niet-geopereerde patiënten herhaalde TPI zullen ondergaan 12 maanden na de baseline. Echobeoordelingen zullen worden geïntegreerd in het standaard hydrocephalus werkprotocol.
De beslissing om cerebrospinale vloeistof (CSF) drainering uit te voeren zal worden genomen door het neurochirurgische team dat geblindeerd is voor de echobevindingen en uitsluitend gebaseerd op standaard klinische en radiologische criteria. Baseline klinische (demografie, vasculaire risicofactoren, cognitieve/loopsymptomen/urinesymptomen, neurologische voorgeschiedenis) en radiologische variabelen (ventrikelvolume, Evans index, callosale hoek, DESH, Radscale) zullen worden verzameld.
Tijdens de ziekenhuisopname zullen loop- en cognitieve functie worden geëvalueerd voor en 48 uur na lumbaalpunctie. TPI zal twee indices van cerebrale pulsatiliteit leveren: gemiddelde hersenweefsel pulsatiliteit (MeanBTP) en maximale hersenweefsel pulsatiliteit (MaxBTP), uitgedrukt als piekamplitude, gemiddelde piekamplitude en wortelgemiddeld kwadraat.
Follow-up zal gestandaardiseerde klinische en radiologische beoordelingen omvatten. Niet-geopereerde patiënten zullen worden geëvalueerd op 12 maanden. Geopereerde patiënten zullen worden gezien op 2 maanden en 12 maanden postoperatief, met documentatie van complicaties, shuntklepinstellingen, veranderingen in ventrikelvolume en subduraal vocht.
Statistische analyses zullen worden uitgevoerd met STATA v11 (StataCorp, USA). Continue variabelen zullen worden uitgedrukt als gemiddelden ± SD of medianen; categorische variabelen als percentages. Normaliteit zal worden beoordeeld met de Kolmogorov-Smirnov test, en variantiegelijkheid met F-test. Vergelijkingen zullen gebruikmaken van Student's t-test, chi-kwadraat, of Fisher's exact test indien van toepassing. Cox proportionele hazards regressie zal worden toegepast voor multivariabele analyse. P < 0,05 zal als significant worden beschouwd.
Het primaire doel zal zijn om aan te tonen dat de gemiddelde amplitude van cerebrale pulsatiliteit (MeanBTP) gemeten vóór subtractieve lumbaalpunctie significant hoger zal zijn bij patiënten die klinische verbetering vertonen 12 maanden na cerebrospinale vloeistof (CSF) drainering in vergelijking met degenen die dat niet doen, met een type I foutrisico (alfa) van 0,05.
De secundaire doelstellingen zullen zijn om cerebrale pulsatiliteit (gemeten door transcraniële pulsbeeldvorming, TPI) te correleren met functionele indices van iNPH en CSF draineringsuitkomsten, waaronder: logopedische evaluatie, fysiotherapie-gebaseerde loopbeoordeling, globale neurologische beperking, ventrikelvolume en shuntklep openingsdruk.

- Opgenomen in het Universitair Ziekenhuis Tours voor 'hydrocephalusbeoordeling'
- Patiënt die schriftelijke en ondertekende toestemming heeft verleend
Niet-insluitingscriteria
- Personen die bezwaar hebben gemaakt tegen gegevensverwerking
- Loopbeoordeling door het fysiotherapieteam onmogelijk EN cognitieve beoordeling door het logopedieteam onmogelijk
Uitsluitingscriteria
- Patiënten die al zijn voorzien van een CSF-drainagesysteem (VPS)
- Vermoeden van obstructieve hydrocephalus (aqueductstenose, tumor, etc.)
- Patiënten onder wettelijke bescherming (voogdij, curatele, gerechtelijke bescherming).
