I denne klynge-randomiserte kontrollerte, hybrid type 3 implementeringsforsøk, vil etterforskere vurdere to tilnærminger til oppskaleringen av kort alkoholintervensjon (BAI). BAI er en evidensbasert intervensjon for å adressere usunn alkoholbruk som omfatter 2 personlige økter og 2 boostertelefonøkter. Ansikt til ansikt 45-minutters økter skjer med ca. 1 måneds mellomrom; 10-minutters telefonøkter skjer 2 til 3 uker etter hver ansikt-til-ansikt-økt. Etterforskere valgte en hybrid type 3-design for å evaluere implementeringsresultater, samtidig som de sikret at effektivitetsresultater, spesielt viral undertrykkelse, oppnås. Etterforskere vil sammenligne tilrettelegging (FAC) versus EBAI+FAC i 30 ART-klinikker i Vietnam. FAC-armen, sammenligningsarmen, vil bruke interne og eksterne tilretteleggere for å hjelpe klinikker og ansatte med å takle vanlige barrierer for implementering av BAI. Tilrettelegging fungerer vanligvis gjennom interaktiv problemløsning og støtte. I EBAI+FAC-armen vil klinikkpersonale, definert som klinikkdirektører, leger, sykepleiere og rådgivere, bli tilbudt den erfaringsbaserte BAI (EBAI), uavhengig av eget alkoholbruk, før BAI-implementering, og klinikkpersonell som er ansvarlig for å levere BAI til pasienter vil også bli tilbudt 3 ekstra konsolideringsmuligheter for å integrere sine egne erfaringer med deres levering av BAI til PWH. Randomisering vil være 1:1 med 15 klinikker per arm. Klinikker vil bli vurdert føropplæring for sentrale kontekstuelle faktorer, knyttet til klinikkkarakteristikker og klinikkpersonale.
Ved oppstart av forsøk vil personer med HIV (PWH) som starter eller på ART screenes for usunn alkoholbruk med AUDIT-C (tid 0). De PWH som screener positive for usunn alkoholbruk vil bli tilbudt BAI av trente klinikkrådgivere. PWH som ikke screener positive vil fortsette å bli screenet med AUDIT-C ved rutinemessige ART-klinikkbesøk. PWH som screener positive, men nekter å delta i studien eller er ekskludert på grunn av alkoholabstinenssymptomer, vil fortsette å bli screenet med AUDIT-C ved hvert rutinemessig ART-klinikkbesøk i henhold til standard klinikkprosedyrer. PWH kan avslå AUDIT-screening når det tilbys. Implementerings- og effektivitetsresultater vil bli evaluert til 12 eller 24 måneder (mål 1). Etter de 12 måneder lange vurderingene vil mekanismer som ligger til grunn for vellykket implementering i begge armer bli utforsket ved bruk av kvalitative og kvantitative metoder (Mål 2). Etterforskere vil utforske effekten av å implementere BAI (begge armer) og oppleve BAI (EBAI+FAC-armen) på ansattes alkoholbruk og holdninger til alkohol og BAI (Mål 3).

PWH-kohortdeltakere
- Person som lever med HIV på alle stadier av HIV-infeksjon
- Går for tiden på studiet ART-klinikken på ethvert ART-stadium (initierer eller mottar ART)
- AUDIT-C score >=4 for menn eller >=3 for kvinner
- >= 18 år
- Villig til å gi informert samtykke, som inkluderer samtykke til intervju og innsamling av tørkede blodflekker
Deltakere i klinikken:
- Arbeid på ART-klinikken som klinikkdirektør, lege, sykepleier eller rådgiver
- Villig til å gi informert samtykke

- Psykologisk forstyrrelse som hindrer deltakelse
- Kognitiv svikt
- Truende oppførsel
- Ikke villig til å gi lokaliseringsinformasjon Merk: Hvis en deltaker screener positivt med AUDIT-C og er identifisert å ha en betydelig risiko for alkoholabstinens basert på Mini International Neuropsychiatric Interview (MINI) og Clinical Institute Absorbering Assessment of Alcohol Scale (CIWA) vil bli henvist til behandling. De vil ikke være kvalifisert for påmelding før etter at problemer med alkoholabstinens er tatt opp.

Fidelity Score
Tidsramme: 15 months from clinic activation
|
Fidelity is a clinic-level measure assessed as a composite of the recipients of the BAI. The timeframe of 15 months reflects individual recruitment over 12 months plus 3 months for completion of the BAI. The score comprises successful completion of the 4 protocol-specified BAI sessions (2 in-person, 2 by phone) within 7 weeks of the initial session weighted by the central fidelity rater's quality rating of the in-person sessions. Fidelity will be assessed using a tailored selection of fidelity measures including the BAI Core Components Checklist. The clinic fidelity score ranges from 0-100. The score will be the percentage of counseling sessions completed, multiplied (weighted) by the combined average quality rating of counseling sessions. Higher scores indicate higher fidelity. |

Acceptability is the perception that the BAI intervention is agreeable, palatable, or satisfactory to clinic staff. Acceptability among clinic staff will be assessed using the Acceptability of Intervention Measure (AIM), which consists of 4 items containing responses on a 5-point Likert scale, ranging from 1-4. The AIM score will be the mean of the 4 item responses (range: 1-5). Higher AIM scores indicate higher acceptability. |

The mhIST score is calculated as the mean score of all responses (range: 0-39). Higher mhIST scores indicate higher acceptability. |

Investigators will use an "ingredients" or bottom-up approach, with comparison to "top-down" costing.
The cost estimates will follow the investigators published conceptual framework for assessing implementation costs and cost analysis of health services.
Investigators will include all types of measurable costs (e.g., staff, equipment, consumables, overheads, etc.) associated with key steps and component of the respective implementation strategy and BAI service delivery.
Investigators will follow international conventions for all procedures including economic costing, discounting, and reporting.

Sustainability Score: Provider Support of Sustainment Scale (PRESS)

Sustainability will be measured among clinic staff, including directors, using the provider support of sustainment scale (PRESS), a brief, 3-item measure of sustainment that is pragmatic and useable across different evidence-based intervention (EBIs), provider types, and settings.
Responses are recorded on a 5-point scale ranging from 0 (not at all) to 4 (to a very great extent) and the score is calculated as the mean of the 3 responses with higher scores indicating higher sustainability.
The PRESS captures frontline staff's report of their clinic's continued use of an EBI.
