I dette klynge-randomiserede kontrollerede hybrid type 3-implementeringsforsøg vil efterforskere vurdere to tilgange til opskaleringen af kort alkoholintervention (BAI). BAI er en evidensbaseret intervention til behandling af usund alkoholbrug, der omfatter 2 personlige sessioner og 2 booster telefonsessioner. Ansigt til ansigt 45-minutters sessioner foregår med ~1 måneds mellemrum; 10-minutters telefonsessioner finder sted 2 til 3 uger efter hver ansigt-til-ansigt session. Efterforskere valgte et hybrid type 3-design til at evaluere implementeringsresultater, mens de samtidig sikrede, at effektivitetsresultater, specifikt viral suppression, opnås. Efterforskere vil sammenligne facilitering (FAC) versus EBAI+FAC i 30 ART-klinikker i Vietnam. FAC-armen, sammenligningsarmen, vil bruge interne og eksterne facilitatorer til at hjælpe klinikker og personale med at håndtere fælles barrierer for BAI-implementering. Facilitering fungerer typisk gennem interaktiv problemløsning og support. I EBAI+FAC-armen vil klinikpersonale, defineret som klinikdirektører, læger, sygeplejersker og rådgivere, blive tilbudt den erfaringsbaserede BAI (EBAI), uanset deres eget alkoholforbrug, før BAI-implementering, og klinikpersonale, der er ansvarlige for at levere BAI til patienter vil også blive tilbudt 3 yderligere konsolideringsmuligheder for at integrere deres egne erfaringer med deres levering af BAI til PWH. Randomisering vil være 1:1 med 15 klinikker pr. arm. Klinikker vil blive vurderet før træning for vigtige kontekstuelle faktorer, relateret til klinikkarakteristika og klinikpersonale.
Ved påbegyndelse af forsøg vil personer med HIV (PWH), der starter eller er på ART, blive screenet for usund alkoholbrug med AUDIT-C (tid 0). De PWH, der screener positive for usund alkoholbrug, vil blive tilbudt BAI af uddannede klinikrådgivere. PWH, der ikke screener positive, vil fortsat blive screenet med AUDIT-C ved rutinemæssige ART-klinikbesøg. PWH, der screener positive, men nægter at deltage i forsøget eller er udelukket på grund af alkoholabstinenssymptomer, vil fortsat blive screenet med AUDIT-C ved hvert rutinemæssigt ART-klinikbesøg i henhold til standard klinikprocedurer. PWH kan afslå AUDIT-screening, når det tilbydes. Implementerings- og effektivitetsresultater vil blive evalueret til 12 eller 24 måneder (mål 1). Efter de 12 måneder lange vurderinger vil mekanismer, der ligger til grund for en vellykket implementering i begge dele, blive undersøgt ved brug af kvalitative og kvantitative metoder (mål 2). Efterforskere vil undersøge effekten af at implementere BAI (begge arme) og opleve BAI (EBAI+FAC-armen) på personalemedlemmers alkoholforbrug og holdninger til alkohol og BAI (Mål 3).

Inklusionskriterier:
PWH-kohortedeltagere
- Person, der lever med HIV på et hvilket som helst stadium af HIV-infektion
- Deltager i øjeblikket i undersøgelsens ART-klinik på ethvert ART-stadium (initierer eller modtager ART)
- AUDIT-C score >=4 for mænd eller >=3 for kvinder
- >= 18 år
- Er villig til at give informeret samtykke, hvilket inkluderer samtykke til interview og indsamling af tørrede blodpletter
Deltagere i klinikken:
- Arbejde på ART-klinikken som klinikdirektør, læge, sygeplejerske eller rådgiver

- Uvillig til at give lokaliseringsoplysninger Bemærk: Hvis en deltager screener positivt med AUDIT-C og identificeres at være i væsentlig risiko for alkoholabstinenser baseret på Mini International Neuropsychiatric Interview (MINI) og Clinical Institute Absorbering Assessment of Alcohol Scale (CIWA) vil blive henvist til behandling. De vil ikke være berettiget til tilmelding før efter problemer med alkoholtilbagetrækning er løst.

Fidelity Score

|
Fidelity is a clinic-level measure assessed as a composite of the recipients of the BAI. The timeframe of 15 months reflects individual recruitment over 12 months plus 3 months for completion of the BAI. The score comprises successful completion of the 4 protocol-specified BAI sessions (2 in-person, 2 by phone) within 7 weeks of the initial session weighted by the central fidelity rater's quality rating of the in-person sessions. Fidelity will be assessed using a tailored selection of fidelity measures including the BAI Core Components Checklist. The clinic fidelity score ranges from 0-100. The score will be the percentage of counseling sessions completed, multiplied (weighted) by the combined average quality rating of counseling sessions. Higher scores indicate higher fidelity. |

Acceptability is the perception that the BAI intervention is agreeable, palatable, or satisfactory to clinic staff. Acceptability among clinic staff will be assessed using the Acceptability of Intervention Measure (AIM), which consists of 4 items containing responses on a 5-point Likert scale, ranging from 1-4. The AIM score will be the mean of the 4 item responses (range: 1-5). Higher AIM scores indicate higher acceptability. |

Acceptability among counselor participants will be assessed using the Mental Health Implementation Science Tools (mhIST) Acceptability Scale for providers, which consists of 13 items containing responses on a 4-point Likert scale, ranging from 0-3. The mhIST score is calculated as the mean score of all responses (range: 0-39). Higher mhIST scores indicate higher acceptability. |

Investigators will use an "ingredients" or bottom-up approach, with comparison to "top-down" costing.
The cost estimates will follow the investigators published conceptual framework for assessing implementation costs and cost analysis of health services.
Investigators will include all types of measurable costs (e.g., staff, equipment, consumables, overheads, etc.) associated with key steps and component of the respective implementation strategy and BAI service delivery.
Investigators will follow international conventions for all procedures including economic costing, discounting, and reporting.

Sustainability Score: Provider Support of Sustainment Scale (PRESS)

Sustainability will be measured among clinic staff, including directors, using the provider support of sustainment scale (PRESS), a brief, 3-item measure of sustainment that is pragmatic and useable across different evidence-based intervention (EBIs), provider types, and settings.
Responses are recorded on a 5-point scale ranging from 0 (not at all) to 4 (to a very great extent) and the score is calculated as the mean of the 3 responses with higher scores indicating higher sustainability.
The PRESS captures frontline staff's report of their clinic's continued use of an EBI.
