In dit clustergerandomiseerde, gecontroleerde, hybride type 3-implementatieonderzoek zullen onderzoekers twee benaderingen van de opschaling van korte alcoholinterventies (BAI) beoordelen. De BAI is een evidence-based interventie om ongezond alcoholgebruik aan te pakken en bestaat uit twee persoonlijke sessies en twee telefonische boostersessies. Face-to-face sessies van 45 minuten vinden plaats met een tussenpoos van ongeveer één maand; Telefonische sessies van 10 minuten vinden 2 tot 3 weken na elke face-to-face sessie plaats. Onderzoekers kozen voor een hybride type 3-ontwerp om de implementatieresultaten te evalueren en er tegelijkertijd voor te zorgen dat de effectiviteitsresultaten, met name virale onderdrukking, worden bereikt. Onderzoekers zullen facilitatie (FAC) vergelijken met EBAI+FAC in 30 ART-klinieken in Vietnam. De FAC-tak, de vergelijkingstak, zal interne en externe facilitators gebruiken om klinieken en personeel te helpen gemeenschappelijke belemmeringen voor de implementatie van BAI aan te pakken. Faciliteren werkt doorgaans via interactieve probleemoplossing en ondersteuning. In de EBAI+FAC-tak zal het kliniekpersoneel, gedefinieerd als kliniekdirecteuren, artsen, verpleegkundigen en adviseurs, de ervaringsgerichte BAI (EBAI) aangeboden krijgen, ongeacht hun eigen alcoholgebruik, voorafgaand aan de implementatie van de BAI, en het kliniekpersoneel dat verantwoordelijk is voor het leveren van de BAI aan patiënten krijgen ook 3 extra consolidatiemogelijkheden aangeboden om hun eigen ervaringen met hun levering van de BAI aan PWH te integreren. De randomisatie zal 1:1 zijn met 15 klinieken per arm. Klinieken zullen voorafgaand aan de training worden beoordeeld op belangrijke contextuele factoren, gerelateerd aan de kenmerken van de kliniek en het personeel van de kliniek.
Bij aanvang van de proef worden personen met HIV (PWH) die een ART starten of ondergaan, gescreend op ongezond alcoholgebruik met de AUDIT-C (tijdstip 0). Die PWH die positief screenen op ongezond alcoholgebruik zullen de BAI aangeboden krijgen door getrainde kliniekadviseurs. PWH die niet positief screenen, zullen tijdens routinematige ART-kliniekbezoeken worden gescreend met de AUDIT-C. PWH die positief screenen maar weigeren deel te nemen aan het onderzoek of worden uitgesloten vanwege alcoholontwenningsverschijnselen, zullen bij elk routinebezoek aan de ART-kliniek worden gescreend met de AUDIT-C, volgens de standaardprocedures van de kliniek. PWH kan AUDIT-screening weigeren wanneer deze wordt aangeboden. De implementatie- en effectiviteitsresultaten zullen worden geëvalueerd tot 12 of 24 maanden (Doelstelling 1). Na de twaalf maanden durende evaluaties zullen de mechanismen die ten grondslag liggen aan een succesvolle implementatie in beide armen worden onderzocht met behulp van kwalitatieve en kwantitatieve methoden (Doelstelling 2). Onderzoekers zullen het effect onderzoeken van de implementatie van de BAI (beide armen) en het ervaren van de BAI (EBAI+FAC-arm) op het alcoholgebruik van medewerkers en de houding ten opzichte van alcohol en BAI (Doelstelling 3).

Inclusiecriteria:
PWH-cohortdeelnemers
- Persoon die met HIV leeft in welk stadium van de HIV-infectie dan ook
- Bezoekt momenteel de ART-studiekliniek in elk ART-stadium (initiëren of ontvangen van ART)
- AUDIT-C-score >=4 voor mannen of >=3 voor vrouwen
- >= 18 jaar oud
- Bereid om geïnformeerde toestemming te geven, waaronder toestemming voor een interview en het verzamelen van opgedroogde bloedvlekken
Deelnemers kliniekpersoneel:
- Werk bij de ART-kliniek als kliniekdirecteur, arts, verpleegkundige of adviseur
- Bereid om geïnformeerde toestemming te geven
Uitsluitingscriteria:

- Psychische stoornissen die deelname verhinderen
- Cognitieve beperking
- Bedreigend gedrag
- Niet bereid locatiegegevens te verstrekken Opmerking: Als een deelnemer positief screent met de AUDIT-C en op basis van het Mini International Neuropsychiatric Interview (MINI) en de Clinical Institute Withdrawal Assessment of Alcohol Scale (CIWA) wordt vastgesteld dat hij een substantieel risico loopt op alcoholontwenning, zal worden verwezen voor behandeling. Ze komen pas in aanmerking voor inschrijving nadat de problemen met alcoholontwenning zijn opgelost.
- Deze deelnemers kunnen na de behandeling opnieuw worden gescreend, toestemming krijgen en worden ingeschreven.

Fidelity Score

Fidelity is a clinic-level measure assessed as a composite of the recipients of the BAI. The timeframe of 15 months reflects individual recruitment over 12 months plus 3 months for completion of the BAI. The score comprises successful completion of the 4 protocol-specified BAI sessions (2 in-person, 2 by phone) within 7 weeks of the initial session weighted by the central fidelity rater's quality rating of the in-person sessions. Fidelity will be assessed using a tailored selection of fidelity measures including the BAI Core Components Checklist. The clinic fidelity score ranges from 0-100. The score will be the percentage of counseling sessions completed, multiplied (weighted) by the combined average quality rating of counseling sessions. Higher scores indicate higher fidelity. |

Investigators will use an "ingredients" or bottom-up approach, with comparison to "top-down" costing.
The cost estimates will follow the investigators published conceptual framework for assessing implementation costs and cost analysis of health services.
Investigators will include all types of measurable costs (e.g., staff, equipment, consumables, overheads, etc.) associated with key steps and component of the respective implementation strategy and BAI service delivery.
Investigators will follow international conventions for all procedures including economic costing, discounting, and reporting.

Sustainability Score: Provider Support of Sustainment Scale (PRESS)

Sustainability will be measured among clinic staff, including directors, using the provider support of sustainment scale (PRESS), a brief, 3-item measure of sustainment that is pragmatic and useable across different evidence-based intervention (EBIs), provider types, and settings.
Responses are recorded on a 5-point scale ranging from 0 (not at all) to 4 (to a very great extent) and the score is calculated as the mean of the 3 responses with higher scores indicating higher sustainability.
The PRESS captures frontline staff's report of their clinic's continued use of an EBI.
