Milhões de pacientes com dor torácica são atendidos nos departamentos de emergência (DEs) todos os anos. Mais da metade dos pacientes de emergência com dor no peito são admitidos no hospital para serem submetidos a mais testes. Apesar das altas taxas de testes, menos de 1 em cada 10 pacientes com dor no peito são diagnosticados com uma síndrome coronariana aguda (SCA) a um custo anual estimado de US$ 13 bilhões. Os padrões atuais de atendimento para dor torácica aguda não concentram os recursos do sistema de saúde, como hospitalização e testes cardíacos, nos pacientes com maior probabilidade de se beneficiar.
O HEART Pathway, que combina um auxílio à decisão clínica com duas medições seriadas de troponina, foi desenvolvido para identificar pacientes com dor no peito que podem receber alta com segurança sem testes cardíacos objetivos (teste de estresse ou angiografia). Estudos retrospectivos e observacionais anteriores estabeleceram que o uso do HEART Pathway reduz os testes cardíacos em >20%, mantendo uma taxa de eventos adversos aceitavelmente baixa. Agora procuramos integrar o HEART Pathway em configurações clínicas do "mundo real" para determinar a eficácia.
O objetivo desta proposta é construir uma colaboração transformadora preenchendo a lacuna entre pesquisa, educação e operações dos sistemas de saúde para fornecer atendimento ao paciente de maneira mais eficaz e eficiente. A vanguarda dessa colaboração busca melhorar a qualidade do atendimento a pacientes com dor torácica aguda, integrando o HEART Pathway na prestação de cuidados cardiovasculares na Wake Forest Baptist Health (WFBH). Este projeto se baseará em nosso trabalho anterior e fornecerá um modelo para usar a educação e os pontos fortes operacionais dos centros médicos acadêmicos dos EUA para disseminar iniciativas para melhorar a prestação de cuidados.

Critério de inclusão:
- Idade maior ou igual a 21 anos no momento da visita ao Departamento de Emergência (DE)
- Desconforto torácico compatível com possível Síndrome Coronariana Aguda (SCA), conforme indicado pelo médico assistente obtendo um eletrocardiograma (ECG) e biomarcadores cardíacos para avaliação do paciente.
Critério de exclusão:
- Nova elevação do segmento ST em derivações contíguas em qualquer eletrocardiograma (>/= 1 mV)
