Jedes Jahr werden Millionen von Patienten mit Brustschmerzen in Notaufnahmen behandelt. Über die Hälfte der ED-Patienten mit Brustschmerzen werden zur weiteren Untersuchung ins Krankenhaus eingeliefert. Trotz hoher Testraten wird bei weniger als einem von zehn Patienten mit Brustschmerzen letztendlich ein akutes Koronarsyndrom (ACS) diagnostiziert, was geschätzte jährliche Kosten von 13 Milliarden US-Dollar verursacht. Aktuelle Behandlungsmuster bei akuten Brustschmerzen konzentrieren die Ressourcen des Gesundheitssystems, wie etwa Krankenhausaufenthalte und Herztests, nicht auf die Patienten, die am wahrscheinlichsten davon profitieren.
Der HEART Pathway, der eine klinische Entscheidungshilfe mit zwei seriellen Troponinmessungen kombiniert, wurde entwickelt, um Patienten mit Brustschmerzen zu identifizieren, die ohne objektive Herztests (Stresstests oder Angiographie) sicher entlassen werden können. Frühere retrospektive Studien und Beobachtungsstudien haben gezeigt, dass die Verwendung des HEART Pathway die Herztests um mehr als 20 % reduziert und gleichzeitig die Rate unerwünschter Ereignisse akzeptabel niedrig hält. Wir versuchen nun, den HEART Pathway in „reale“ klinische Umgebungen zu integrieren, um die Wirksamkeit zu bestimmen.
Ziel dieses Vorschlags ist der Aufbau einer transformativen Zusammenarbeit, die die Lücke zwischen Forschung, Bildung und Gesundheitssystembetrieben schließt, um die Patientenversorgung effektiver und effizienter zu gestalten. Der Vorreiter dieser Zusammenarbeit zielt darauf ab, die Qualität der Versorgung von Patienten mit akuten Brustschmerzen zu verbessern, indem der HEART Pathway in die kardiovaskuläre Versorgung bei Wake Forest Baptist Health (WFBH) integriert wird. Dieses Projekt wird auf unserer bisherigen Arbeit aufbauen und ein Modell für die Nutzung der Ausbildung und der operativen Stärken akademischer medizinischer Zentren in den USA zur Verbreitung von Initiativen zur Verbesserung der Gesundheitsversorgung liefern.

Beschreibung
Einschlusskriterien:
- Alter mindestens 21 Jahre zum Zeitpunkt des Besuchs in der Notaufnahme
- Brustbeschwerden im Zusammenhang mit einem möglichen akuten Koronarsyndrom (ACS), wie vom behandelnden Arzt angezeigt, der ein Elektrokardiogramm (EKG) und kardiale Biomarker zur Beurteilung des Patienten erstellt.
Ausschlusskriterien:
- Neue ST-Strecken-Hebung in zusammenhängenden Ableitungen in jedem Elektrokardiogramm (>/= 1 mV)
