Milioni di pazienti con dolore toracico vengono visitati nei Pronto Soccorso (DE) ogni anno. Oltre la metà dei pazienti con DE con dolore toracico viene ricoverata in ospedale per sottoporsi a ulteriori test. Nonostante gli alti tassi di test, a meno di 1 paziente su 10 con dolore toracico viene diagnosticata una sindrome coronarica acuta (ACS) con un costo annuo stimato di 13 miliardi di dollari. Gli attuali modelli di assistenza per il dolore toracico acuto non riescono a concentrare le risorse del sistema sanitario, come l'ospedalizzazione e i test cardiaci, sui pazienti che hanno maggiori probabilità di trarne beneficio.
Il percorso HEART, che combina un aiuto decisionale clinico, con due misurazioni seriali della troponina, è stato sviluppato per identificare i pazienti con dolore toracico che possono essere dimessi in sicurezza senza test cardiaci oggettivi (test da sforzo o angiografia). Precedenti studi retrospettivi e osservazionali hanno stabilito che l'uso del percorso HEART riduce i test cardiaci di > 20%, pur mantenendo un tasso di eventi avversi accettabilmente basso. Cerchiamo ora di integrare il percorso del CUORE nelle impostazioni cliniche del "mondo reale" per determinarne l'efficacia.
L'obiettivo di questa proposta è costruire una collaborazione trasformativa che colmi il divario tra ricerca, istruzione e operazioni dei sistemi sanitari per fornire in modo più efficace ed efficiente l'assistenza ai pazienti. L'avanguardia di questa collaborazione mira a migliorare la qualità dell'assistenza ai pazienti con dolore toracico acuto integrando il percorso HEART nell'erogazione di cure cardiovascolari presso la Wake Forest Baptist Health (WFBH). Questo progetto si baserà sul nostro lavoro precedente e fornirà un modello per l'utilizzo dell'istruzione e dei punti di forza operativi dei centri medici accademici statunitensi per diffondere iniziative volte a migliorare l'erogazione delle cure.

Descrizione
Criterio di inclusione:
- Età maggiore o uguale a 21 anni al momento della visita al Pronto Soccorso (PS).
- Fastidio al torace coerente con una possibile sindrome coronarica acuta (ACS) come indicato dal medico curante che ottiene un elettrocardiogramma (ECG) e biomarcatori cardiaci per la valutazione del paziente.
Criteri di esclusione:
- Nuovo sopraslivellamento del tratto ST nelle derivazioni contigue su qualsiasi elettrocardiogramma (>/= 1 mV)
