O objetivo desta proposta é determinar a viabilidade de um novo tratamento de biofeedback baseado em realidade virtual (VR-B) entre jovens altamente vulneráveis a dificuldades na regulação emocional. O biofeedback, que envolve o fornecimento de informações em tempo real sobre fisiologia, é uma abordagem bem estudada para aumentar a capacidade de autorregulação, com potencial para aliviar problemas de saúde mental. Em crianças (assim como em adultos), o biofeedback pode ser idealmente facilitado por meio de ambientes de videogame, que oferecem ambientes envolventes, imersivos e dinâmicos que podem ser alterados em tempo real. Os videogames são extremamente populares entre os jovens e podem representar um importante caminho para o tratamento em populações adolescentes. A presente proposta visa avaliar um novo tratamento de videogame baseado em realidade virtual para desregulação emocional para jovens atualmente sob a supervisão do sistema de justiça juvenil. A exposição a fatores de risco conhecidos por promover a autorregulação prejudicada, como maus-tratos infantis, é quase endêmica nessa população. De fato, jovens 'delinquentes' (ou seja, jovens em contato com o sistema de justiça) são consideravelmente mais propensos a atender aos critérios diagnósticos para uma série de distúrbios que envolvem centralmente a desregulação do afeto ou excitação, como transtorno de estresse pós-traumático, ansiedade, depressão, transtorno desafiador opositivo e muitos outros.
Objetivos Específicos:
OBJETIVO 1: Avaliar a viabilidade de um tratamento de realidade virtual para jovens
- Hipótese 1: Os jovens do estudo que recebem tratamento com o paradigma baseado em realidade virtual relatarão qualitativamente a experiência como tolerável e positiva com desconforto mínimo.
OBJETIVO 2: Identificar possíveis mudanças na sinalização fisiológica (por exemplo, frequência cardíaca, condutância da pele) durante as sessões de tratamento
- Hipótese 2: Os jovens que recebem tratamento de biofeedback de realidade virtual (VRB) exibirão alterações fisiológicas ao longo das sessões (por exemplo, aumento da variabilidade da frequência cardíaca ou alterações nas respostas de condutância da pele).
OBJETIVO 3: Identificar possíveis mudanças na gravidade dos sintomas nas sessões de tratamento
- Hipótese 3: Os jovens que recebem tratamento de biofeedback de realidade virtual (VRB) apresentarão maiores melhorias na regulação emocional dos jovens e reduzirão a gravidade dos sintomas de saúde mental do TEPT.

Critério de inclusão:
- 13 a 17 anos de idade na matrícula
- Disponibilidade de um cuidador para fornecer consentimento verbal
- Capaz de fornecer parecer favorável/consentimento em inglês
- Acuidade visual adequada para ler texto em um monitor de computador
Critério de exclusão:
- Relato de desconforto anterior com experiências imersivas, incluindo realidade virtual, filmes 3D ou mídia semelhante
- Psicose ativa ou condição psiquiátrica comparativamente prejudicial

Experimental: Grupo Experimental DEEP VR
Os participantes identificados no Programa do Tribunal Juvenil do Condado de Dane serão solicitados a experimentar até 6 sessões de VR-B.
Os participantes usarão um head-mounted display leve, de ultra-alta resolução e sem fio (Oculus Quest 2 Enterprise VR Headset).
Cada sessão passará por uma série de quatro etapas.
Primeiro, os participantes começarão com um período de aclimatação de 5 minutos dentro de um ambiente de demonstração VR.
Em segundo lugar, os níveis basais de excitação fisiológica serão capturados durante um período de descanso de 5 minutos, no qual os participantes serão solicitados a sentar-se silenciosamente em um ambiente virtual sereno.
Em terceiro lugar, os participantes progredirão na experiência DEEP VR por 15 minutos.
Por fim, os participantes preencherão uma pequena série de questionários online.

DEEP VR é uma experiência de realidade virtual (ou seja, videogame usando um head-mounted display) em que os participantes ('jogadores') atravessam uma serena aventura subaquática por aproximadamente 15 minutos.

Mean Change in Difficulty in Emotion Regulation Scale (DERS)

The DERS is a 36-item child-report survey measuring difficulties with emotion regulation.
Respondents report the degree to which the given statement applied to them on a five point Likert scale: 1 = Almost Never to 5 = Always, with a possible total score range of 36 - 180.
Higher scores indicate greater difficulties with emotion regulation.

Physiological Arousal Measured by Change in Heart Rate

Participants will have electrodes affixed to their abdomen to measure heart rate.
Before every session, baseline levels of physiological arousal will be captured over a 5 minute resting period.
Participants will be asked to sit quietly within a serene virtual outdoor scene followed by a 15 minute DEEP VR experience containing several task conditions.

Assesses lifetime trauma experiences and severity of PTSD symptom severity in a 20-item survey scored on a 5-point likert scale from 0 'not at all' to 4 'almost always' for a total range of scores between 0 - 80, where higher scores indicate increased PTSD symptoms.
