Critério de inclusão:
- Gravidez estabelecida >20 semanas de gestação por DUM e/ou avaliação clínica e/ou USG
- Idade: 15 anos (ou limite mínimo de idade elegível*) a 49 anos
- Anemia moderada confirmada (Hb 7,0 a 9,9 g/dL, 6-48 horas após o parto com base em uma amostra de sangue venoso em Hemocue®)
- Entregue no hospital ou centro de saúde participante do estudo
- Capaz de fornecer consentimento informado
- Planos para permanecer na área de estudo durante a duração do estudo
Critério de exclusão:
- Infusão IV de ferro recebida nas últimas 3 semanas
- Contra-indicação à suplementação de ferro (alguns exemplos podem incluir anemia hemolítica, alergia, infecção grave)
- Transfusão de sangue já recebida ou programada durante a internação atual
- Diagnóstico conhecido de depressão pré-existente ou outra doença psiquiátrica
- Natimorto, anomalia congênita maior ou perda neonatal antes da randomização
- Mulheres com teste positivo e sem tratamento prévio para malária
- Apresentar anemia sintomática com dispneia ou fadiga e necessidade de correção imediata
- Mulheres com hemoglobinopatia conhecida (doença falciforme ou talassemia)
- Presença de condições alérgicas graves, como asma grave ou alergias medicamentosas conhecidas
- Mulheres apresentando qualquer doença/condição que requeira cuidados médicos imediatos de acordo com a avaliação do médico

Experimental: IV iron infusion
Participants randomized to the IV iron arm will receive a single-dose infusion of IV FCM, to be initiated between 6-48 hours after delivery.
All randomized participants will receive 400 mcg of folic acid daily for 6 months postpartum.

FCM 50 mg iron/mL will be in a solution of 20 mL vials for infusion, using a dosage of 20 mg elemental iron per kg body weight, up to a maximum of 1 g, in a single IV infusion over 20-30 minutes.
400 mcg of folic acid daily to 6 months.

Comparador Ativo: Oral iron tablets
Participants randomized to the oral iron arm will receive 60 mg elemental iron twice daily.
Treatment will be initiated between 6-48 hours after delivery and prior to discharge from the facility.
After discharge, each participant will take a treatment dose of twice daily for 6 weeks postpartum.
At 6 weeks postpartum, serum Hb will be assessed and participants with Hb < 7.0 g/dL will stop treatment, participants with Hb 7.0-11.9
g/dL will continue taking 60 mg elemental iron twice daily and participants with Hb > 11.9 g/dL will take 60 mg elemental iron once daily.
All randomized participants will receive 400 mcg of folic acid daily for 6 months postpartum.
