Descripción
Criterios de inclusión:
- Embarazo establecido > 20 semanas de gestación por LMP y/o evaluación clínica y/o USG
- Edad: 15 años (o límite inferior de edad elegible*) a 49 años
- Anemia moderada confirmada (Hb 7,0 a 9,9 g/dL, 6-48 horas después del parto según una muestra de sangre venosa en Hemocue®)
- Dar a luz en un hospital o centro de salud participante en el estudio
- Capaz de proporcionar consentimiento informado
- Planes para permanecer en el área de estudio durante la duración del estudio
Criterio de exclusión:
- Infusión de hierro IV recibida en las últimas 3 semanas
- Contraindicación para la suplementación con hierro (algunos ejemplos pueden incluir anemia hemolítica, alergia, infección grave)
- Transfusión de sangre ya recibida o programada durante el ingreso hospitalario actual
- Diagnóstico conocido de depresión preexistente u otra enfermedad psiquiátrica
- Muerte fetal, anomalía congénita importante o pérdida neonatal antes de la aleatorización
- Mujeres con resultado positivo y sin tratamiento previo para la malaria
- Presentar anemia sintomática con disnea o fatiga y necesidad de corrección inmediata
- Mujeres con hemoglobinopatía conocida (enfermedad de células falciformes o talasemia)
- Presencia de afecciones alérgicas graves, como asma grave o alergias conocidas a medicamentos
- Mujeres que presenten cualquier enfermedad/afección que requiera atención médica inmediata según la evaluación del médico

Experimental: IV iron infusion
Participants randomized to the IV iron arm will receive a single-dose infusion of IV FCM, to be initiated between 6-48 hours after delivery.
All randomized participants will receive 400 mcg of folic acid daily for 6 months postpartum.

FCM 50 mg iron/mL will be in a solution of 20 mL vials for infusion, using a dosage of 20 mg elemental iron per kg body weight, up to a maximum of 1 g, in a single IV infusion over 20-30 minutes.
400 mcg of folic acid daily to 6 months.

Comparador activo: Oral iron tablets
Participants randomized to the oral iron arm will receive 60 mg elemental iron twice daily.
Treatment will be initiated between 6-48 hours after delivery and prior to discharge from the facility.
After discharge, each participant will take a treatment dose of twice daily for 6 weeks postpartum.
At 6 weeks postpartum, serum Hb will be assessed and participants with Hb < 7.0 g/dL will stop treatment, participants with Hb 7.0-11.9
g/dL will continue taking 60 mg elemental iron twice daily and participants with Hb > 11.9 g/dL will take 60 mg elemental iron once daily.
All randomized participants will receive 400 mcg of folic acid daily for 6 months postpartum.
