消融治疗慢性持续性 AF 的作用在心脏病专家中争论了一段时间,越来越多的证据表明消融可能优于药物治疗。 随着人口老龄化,AF 的发生率可能会增加,因此预计针对这种心律失常进行的消融次数也会增加。

纳入标准:
- 在需要麻醉师提供镇静的 TGH 接受消融手术治疗心房颤动或扑动的患者
- 有效同意
排除标准:
- 基线心率 <40
- 基线收缩压 < 80mmHg
- 基线收缩压 > 180mmHg
- 二度或三度心脏传导阻滞,除非在原位起搏器
- 不受控制的心力衰竭/严重的 LV 功能障碍(射血分数 < 40%)
- 严重肝功能障碍(转氨酶超过正常上限的 2 倍)
- 肾功能不全:估计 GFR < 30ml/min,或需要透析

实验性的:右美托咪定组
研究药物右美托咪定 (PrecedexTM) 以右美托咪定盐酸盐 200mcg/瓶 (100mcg/ml) 的形式提供。
将其添加到 98 ml 0.9% NaCl 中以达到 2mcg/ml 的浓度,并从病例开始时以 0.2-1mcg/kg/小时的速度输注。
小于或等于65岁者开始输注速率为1mcg/kg/小时,大于65岁者为0.7mcg/kg/小时,然后根据术中镇静评分滴定(以达到小于或等于 4 的镇静和激动量表 (SAS) 评分)和心血管参数(在基线的 30% 以内)。
