急性呼吸窘迫综合征 (ARDS) 是一种高度致命的疾病,治疗选择有限。
近年来研究表明,俯卧位通气可通过促进肺复张、提高通气/灌注(V/Q)比值、增强呼吸系统顺应性、促进排痰、有效避免过度充气等方式降低ARDS患者的死亡率和肺损伤。背肺。
电阻抗断层扫描(Electrical Impedance Tomography,EIT)技术已被用于评估俯卧位通气对肺V/Q匹配的影响,一些研究证实俯卧位通气可以改善肺V/Q匹配和氧合指数。
然而,以往的研究多为病例报告或小样本生理学研究,缺乏重复俯卧位通气过程中肺V/Q匹配的动态变化。
因此,本研究假设俯卧位通气可以增加ARDS患者的肺V/Q匹配,其改善与氧合指数、有创通气时间和患者预后的变化相关。
反复俯卧位通气可使肺V/Q匹配保持在较高水平,不再受体位变化影响,可加速肺功能恢复,改善ARDS患者预后。

PaO2/FiO2小于150mmHg的ARDS患者。
描述
纳入标准:
- 年龄≥18岁;
- 符合柏林对 ARDS 定义的诊断标准;
- 有创通气时间<48h;
- PaO2/FiO2 < 150mmHg。
排除标准:
- 俯卧位禁忌症;
- EIT 的禁忌症;
- 血液动力学不稳定;
- 高钠血症患者;
- 患者已接受体外膜肺氧合治疗。
