Questo è uno studio prospettico, multicentrico, multicoorte, non randomizzato di fase 2 su un massimo di 40 pazienti adulti con trauma vascolare pericoloso per la vita o per gli arti che richiede riparazione chirurgica. Ci sarà una coorte degli arti e una coorte del busto. La coorte dell'arto includerà pazienti che richiedono la riparazione di una nave contenuta all'estremità superiore o inferiore. La coorte del torso include pazienti che richiedono la riparazione dei vasi all'interno del torace (escluso il cuore), dell'addome e del retroperitoneo. Ai soggetti verrà impiantato un Humacyte Human Acellular Vessel (HAV) come vaso di interposizione o bypass utilizzando tecniche chirurgiche vascolari standard. Non c'è braccio di controllo.
La durata attiva dello studio per ciascun partecipante allo studio sarà di 36 mesi dall'impianto dell'HAV o fino al fallimento/rimozione/morte dell'HAV, se precedente. Il follow-up dopo il mese 12 comporterà l'acquisizione di informazioni sulle valutazioni eseguite durante le visite cliniche di routine "standard di cura" o tramite telefono di follow-up con il paziente o il suo medico con esame fisico ed ecografia al mese 24 e al mese 36
La durata totale prevista dello studio clinico è di 61 mesi (24 mesi di arruolamento e 36 mesi di follow-up).

Criterio di inclusione:
- Pazienti con lesione traumatica che minaccia la vita o l'arto di un vaso arterioso dell'arto o del tronco, diverso dal cuore, che richiede la sostituzione o la ricostruzione
- L'imaging preoperatorio o l'esame clinico indicano che il vaso danneggiato ha una lunghezza del difetto di ≤ 38 cm ed è adeguatamente dimensionato per il vaso acellulare umano (HAV) da 6 mm secondo il giudizio del chirurgo curante, tenendo conto della vasocostrizione e delle considerazioni situazionali di afflusso e deflusso.
- L'innesto venoso autologo non è fattibile a giudizio del chirurgo curante (ad es. a causa della mancanza di disponibilità di un condotto adatto, presenza di grave insufficienza venosa) o non è desiderabile a causa dell'urgenza della rivascolarizzazione
- Età compresa tra i 18 e gli 85 anni
- In grado di comunicare in modo significativo con il personale investigativo e in grado di rispettare intere procedure di studio. Se il paziente è incosciente, le informazioni fornite da un testimone attendibile indicano che il paziente sarebbe normalmente in grado di rispettare le procedure dello studio
- Il paziente o il parente è in grado, disposto e competente a dare il consenso informato
- Aspettativa di vita di almeno 1 anno
Criteri di esclusione:
- Mangled Extremity Severity Score (MESS) di ≥ 7
- Arto ad alto rischio di amputazione nonostante la ricostruzione vascolare (ad esempio, a causa di una lesione da schiacciamento)
- Lesioni catastrofiche che rendono improbabile la sopravvivenza (ad es. Scala abbreviata delle lesioni (AIS) > 5 o Injury Severity Score (ISS) >60)

Sperimentale: Acellular Tissue Engineered Vessel (ATEV)
Patients with life or limb threatening traumatic injury to an arterial vessel in the limb or torso, other than the heart, will be implanted with the Humacyte ATEV as an interposition vessel or bypass using standard vascular surgical techniques.
|
The investigational medicinal product (IMP) - the Acellular Tissue Engineered Vessel (ATEV) is a sterile acellular tubular graft composed of human collagen types I and III and other extracellular matrix proteins, including fibronectin and vitronectin which can be used for arterial bypass or reconstruction in patients with life or limb threatening vascular trauma.
The vessel is 6 mm in diameter and approximately 42 cm in length.
The product is supplied on a silicone mandrel immersed in sterile phosphate buffered saline in a sealed and labeled plastic container.
The ATEV is implanted using standard vascular surgical techniques similar to placement of predicate peripheral vascular prostheses.
