Tato studie hodnotí použití lidské acelulární cévy (HAV) u dospělých s vaskulárním traumatem pod krkem, kteří podstupují vaskulární rekonstrukční operaci. Bude to kohorta trupu a kohorta končetin. Všem subjektům bude implantován HAV jako interpoziční céva nebo bypass za použití standardních vaskulárních chirurgických technik. Neexistuje žádné ovládací rameno.

Detailní popis

Toto je prospektivní, multicentrická, multikohortní, nerandomizovaná studie fáze 2 u až 40 dospělých pacientů s vaskulárním traumatem ohrožujícím život nebo končetinu, které vyžaduje chirurgickou opravu. Bude existovat kohorta končetin a kohorta trupu. Kohorta končetiny bude zahrnovat pacienty, kteří vyžadují opravu cévy na horní nebo dolní končetině. Skupina trupu zahrnuje pacienty, kteří vyžadují opravu cév v hrudníku (kromě srdce), břiše a retroperitonea. Subjektům bude implantována Humacyte Human Acellular Vessel (HAV) jako interpoziční céva nebo bypass za použití standardních vaskulárních chirurgických technik. Neexistuje žádné ovládací rameno.

Aktivní doba trvání studie pro každého účastníka studie bude 36 měsíců od implantace HAV nebo do selhání/odstranění/úmrtí HAV, pokud dříve. Sledování po 12. měsíci bude zahrnovat zachycení informací o hodnoceních prováděných při rutinních návštěvách kliniky „standardní péče“ nebo telefonické sledování pacienta nebo jeho lékaře s fyzikálním vyšetřením a ultrazvukem ve 24. a 36. měsíci.

Celková očekávaná délka klinické studie je 61 měsíců (24 měsíců zařazení a 36 měsíců sledování).

Kritéria pro zařazení:

  1. Pacienti se životem nebo končetinou ohrožujícím traumatickým poraněním arteriální cévy v končetině nebo trupu, jiné než srdce, které vyžaduje náhradu nebo rekonstrukci
  2. Předoperační zobrazení nebo klinické vyšetření ukazuje, že poškozená céva má délku defektu ≤ 38 cm a její velikost vhodně odpovídá 6 mm lidské acelulární cévě (HAV) podle posouzení ošetřujícího chirurga s přihlédnutím k vazokonstrikci a situačnímu přítoku a odtoku.
  3. Autologní žilní štěp buď není podle úsudku ošetřujícího chirurga proveditelný (např. kvůli nedostatku vhodného konduitu, přítomnosti těžké žilní nedostatečnosti) nebo není žádoucí kvůli naléhavosti revaskularizace.
  4. Ve věku od 18 do 85 let včetně
  5. Schopnost smysluplně komunikovat s vyšetřujícím personálem a schopna dodržet veškeré studijní postupy. Pokud je pacient v bezvědomí, pak informace od spolehlivého svědka naznačují, že pacient by za normálních okolností byl schopen dodržovat studijní postupy
  6. Pacient nebo příbuzný je schopen, ochotný a kompetentní dát informovaný souhlas
  7. Délka života minimálně 1 rok

Kritéria vyloučení:

  1. Skóre závažnosti poškození končetin (MESS) ≥ 7
  2. Končetina s vysokým rizikem amputace i přes vaskulární rekonstrukci (např. z důvodu rozdrcení)
  3. Katastrofální zranění, která činí přežití nepravděpodobným (např. Zkrácená stupnice zranění (AIS) > 5 nebo skóre závažnosti zranění (ISS) > 60)

Experimentální: Acellular Tissue Engineered Vessel (ATEV)
Patients with life or limb threatening traumatic injury to an arterial vessel in the limb or torso, other than the heart, will be implanted with the Humacyte ATEV as an interposition vessel or bypass using standard vascular surgical techniques.
The investigational medicinal product (IMP) - the Acellular Tissue Engineered Vessel (ATEV) is a sterile acellular tubular graft composed of human collagen types I and III and other extracellular matrix proteins, including fibronectin and vitronectin which can be used for arterial bypass or reconstruction in patients with life or limb threatening vascular trauma. The vessel is 6 mm in diameter and approximately 42 cm in length. The product is supplied on a silicone mandrel immersed in sterile phosphate buffered saline in a sealed and labeled plastic container. The ATEV is implanted using standard vascular surgical techniques similar to placement of predicate peripheral vascular prostheses.
