Detaillierte Beschreibung
Dies ist eine prospektive, multizentrische, nicht randomisierte Phase-2-Studie mit mehreren Kohorten an bis zu 40 erwachsenen Patienten mit lebens- oder gliedmaßenbedrohlichen Gefäßtraumata, die einer chirurgischen Reparatur bedürfen. Es wird eine Extremitäten-Kohorte und eine Torso-Kohorte geben. Die Gliedmaßenkohorte umfasst Patienten, die eine Reparatur eines in der oberen oder unteren Extremität eingeschlossenen Gefäßes benötigen. Die Torso-Kohorte umfasst Patienten, die eine Reparatur von Gefäßen im Thorax (ohne Herz), Abdomen und Retroperitoneum benötigen. Den Probanden wird ein humanes azelluläres Gefäß (HAV) von Humacyte als Zwischengefäß oder Bypass unter Verwendung von standardmäßigen vaskulären chirurgischen Techniken implantiert. Es gibt keinen Kontrollarm.
Die aktive Studiendauer für jeden Studienteilnehmer beträgt 36 Monate ab HAV-Implantation oder bis HAV-Versagen/Entfernung/Tod, falls früher. Die Nachsorge nach dem 12. Monat beinhaltet die Erfassung von Informationen zu Bewertungen, die bei routinemäßigen Klinikbesuchen nach dem „Standard der Versorgung“ oder per telefonischer Nachsorge mit dem Patienten oder seinem Arzt mit körperlicher Untersuchung und Ultraschall in Monat 24 und Monat 36 durchgeführt werden
Die erwartete Gesamtdauer der klinischen Studie beträgt 61 Monate (24 Monate nach Aufnahme und 36 Monate Nachbeobachtung).

Beschreibung
Einschlusskriterien:
- Patienten mit lebens- oder gliedmaßenbedrohlicher traumatischer Verletzung eines arteriellen Gefäßes in der Gliedmaße oder im Rumpf, außer dem Herzen, das ersetzt oder rekonstruiert werden muss
- Die präoperative Bildgebung oder klinische Untersuchung zeigt, dass das beschädigte Gefäß eine Defektlänge von ≤ 38 cm hat und nach Einschätzung des behandelnden Chirurgen unter Berücksichtigung der Vasokonstriktion und situationsbedingter Zu- und Abflüsse in angemessener Größe an das 6 mm große humane azelluläre Gefäß (HAV) angepasst ist.
- Eine autologe Venentransplantation ist nach Einschätzung des behandelnden Chirurgen entweder nicht durchführbar (z. B. wegen fehlender Verfügbarkeit eines geeigneten Conduits, Vorliegen einer schweren venösen Insuffizienz) oder wegen der Dringlichkeit einer Revaskularisation nicht wünschenswert
- Im Alter von 18 bis einschließlich 85 Jahren
- In der Lage, sinnvoll mit dem Untersuchungspersonal zu kommunizieren und die gesamten Studienverfahren einzuhalten. Wenn der Patient bewusstlos ist, weisen die Informationen eines zuverlässigen Zeugen darauf hin, dass der Patient normalerweise in der Lage wäre, die Studienverfahren einzuhalten

Ausschlusskriterien:
- Mangled Extremity Severity Score (MESS) von ≥ 7
- Extremität mit hohem Amputationsrisiko trotz Gefäßrekonstruktion (z. B. wegen Quetschung)
- Katastrophale Verletzungen, die ein Überleben unwahrscheinlich machen (z. Abbreviated Injury Scale (AIS) > 5 oder Injury Severity Score (ISS) > 60)
- HAV darf nicht zur Reparatur von Koronararterien verwendet werden
- Bekannte Schwangere

Experimental: Acellular Tissue Engineered Vessel (ATEV)
Patients with life or limb threatening traumatic injury to an arterial vessel in the limb or torso, other than the heart, will be implanted with the Humacyte ATEV as an interposition vessel or bypass using standard vascular surgical techniques.
|
The investigational medicinal product (IMP) - the Acellular Tissue Engineered Vessel (ATEV) is a sterile acellular tubular graft composed of human collagen types I and III and other extracellular matrix proteins, including fibronectin and vitronectin which can be used for arterial bypass or reconstruction in patients with life or limb threatening vascular trauma.
The vessel is 6 mm in diameter and approximately 42 cm in length.
The product is supplied on a silicone mandrel immersed in sterile phosphate buffered saline in a sealed and labeled plastic container.
The ATEV is implanted using standard vascular surgical techniques similar to placement of predicate peripheral vascular prostheses.
