進行性胆道悪性腫瘍患者の治療における個別化ネオアンチゲンに特異的な精密 T 細胞を使用した養子細胞免疫療法の臨床研究
目的:
この研究の目的は、進行性胆道悪性腫瘍の治療におけるネオアンチゲンに対する樹状細胞プレシジョン T 細胞の安全性と予後を評価することです。
方法:
この研究では、樹状細胞精度の複数抗原 T 細胞を使用した新しい治療法を設計します。 40人の患者が登録される予定です。 それらは、ゲムシタビン群と、ゲムシタビンと組み合わせたネオアンチゲン用の樹状細胞精密 T 細胞群にランダムに分けられます。 ゲムシタビン治療は週に1回、合計6回行われます。 ゲムシタビン治療と組み合わせたネオアンチゲン用樹状細胞プレシジョン T 細胞: ゲムシタビン: 採血開始の 60 日前までに、週に 1 回、合計 6 回。 ネオアンチゲン用樹状細胞プレシジョンT細胞:3週間に1回、合計3期間。 メール臨床指標は、無増悪生存期間と全生存期間です。

- 1. 年齢 18 歳~65 歳の男性または女性、2. 平均余命>6ヶ月;3. 東部協力腫瘍学グループ (ECOG) スコア: 0-2;4。 臨床検査: ① 白血球 ≥ 3 x 109/L。 血小板数 ≥ 60 x 109/L;ヘモグロビン≧85g/L; ②総ビリルビン≤100mol/L;アミノフェラーゼは通常の5倍未満。 ③血清クレアチニンが正常の1.5倍未満;5. 署名されたインフォームドコンセント;6. 不妊症患者は積極的に避妊法を使用します。
除外基準:
- 1. 予想される全生存期間 < 6 か月;2. その他の重篤な疾患: 心臓、肺、腎臓、消化器疾患、神経疾患、精神疾患、免疫調節疾患、代謝疾患、感染症など。3. 血清 クレアチニン > 2.5mg/dL;血清グルタミン酸オキサロ酢酸トランスアミナーゼ (SGOT) > 正常の 5 倍、総ビリルビン > 100μmol/L、 4.他の薬剤、生物学的療法、化学療法または放射線療法が1か月以内に使用された。5. 署名されたインフォームドコンセントがない場合。

DC-PNAT: 3 週間に 1 回、合計 3 つの期間。

ゲムシタビン:ゲムシタビン 1000mg/m2、生理食塩水 100ml:週 1 回、採血開始 60 日前までに計 6 回点滴(静脈内)。 DC-PNAT: DC 細胞懸濁液 (1×107 DC+生理食塩水 + 0.25% ヒト血清アルブミン) 各注入ごとに 1ml、各注入ごとに皮下注射、3 サイクル、各サイクルは 19 日目、20 日目に 2 回注入を受けました。 40、41; 61、62。 PNAT 細胞懸濁液 (1-6×109 PNAT + 生理食塩水 + 0.25% ヒト血清アルブミン) 各注入 300 ml、各注入ごとに IV (静脈内)、3 サイクル、各サイクルは 21、42、63 日目に 1 回注入。 |
