항고혈압 반응의 약물유전체학적 평가(PEAR)

고혈압(HTN)은 약물이 처방되는 가장 흔한 만성 질환이며 심장마비, 뇌졸중, 신부전 및 심부전의 가장 흔한 위험 요소입니다.
항고혈압 약물 요법에 대한 반응은 환자간 상당한 변동성을 나타내어 고혈압 조절률이 낮고(현재 미국에서 34%) 빈번한 비순응 및 치료 중단에 기여합니다.
우리는 선호되고 약력학적으로 대조되는 두 가지 약물인 베타 차단제(atenolol)와 티아지드 이뇨제(hydrochlorothiazide)에 대한 항고혈압 및 유해 대사 반응의 유전적 예측인자를 식별할 것을 제안합니다. 고혈압.
혈압(BP) 반응의 가정 및 외래 측정과 불리한 대사 반응의 지질 및 인슐린 감수성 측정을 포함한 고품질 표현형 데이터는 두 가지 접근법을 통해 유전적 변이와 관련될 것입니다.
먼저, 70개의 후보 유전자 각각에서 7개의 SNP(single nucleotide polymorphisms)를 테스트하여 베타 차단제 및 이뇨제에 대한 반응에 대한 이러한 유전자의 변이의 영향을 조사합니다(특정 목표 1).

앉은 상태에서 평균 가정 이완기 혈압(DBP) > 85 mmHg 및 가정 수축기 혈압(SBP) < 180 mmHg. 피험자는 또한 90mmHg와 110mmHg 사이의 평균 좌식(> 5분) 클리닉 DBP 및 SBP < 180mmHg를 가져야 합니다.
