Plasmodium vivax 말라리아는 이환율의 주요 원인이며 열대 지역의 사망률에 상당한 기여를 합니다(Battle et al. 2019). 재발은 질병의 주요 원인이며 감염률이 높은 환경에서는 심각한 빈혈과 어린 아이의 사망 및 유산의 원인이 됩니다.
동남아시아는 2017년에 P. vivax 말라리아 부담으로 추정되는 전 세계 부담 중 가장 높은 비율을 차지합니다(1,430만 중 720만).
동남아시아에서 급성 P. vivax 혈액 단계 감염 치료를 받은 환자의 절반 이상이 적어도 한 번은 재발할 것입니다(Betuela et al. 2012; Chu et al. 2018; Commons et al. 2019; Luxemburger et al. 1999; Poespoprodjo et al. 2009; Sutanto et al. 2013) 항재발 약물을 투여하지 않은 경우. 현재 재발을 예방하기 위해 널리 사용되는 유일한 약물(근치적 치료)은 8-아미노퀴놀린 프리마퀸입니다. 동남아시아와 서태평양 지역에서 재발을 예방하는 데 필요한 프리마퀸 용량은 다른 지역보다 높습니다. P. vivax는 전 세계 P. vivax 부하의 80% 이상이 많은 인구가 발생하는 이 지역에서 더 많이 재발하므로 세계 재발의 대부분이 동아시아에서 발생할 가능성이 있으므로 효과적인 급진적 치료의 이점은 이 지역에서 가장 큽니다. .
현재 권장되는 14일 프리마퀸 요법에 대한 준수 여부는 가변적이며 이는 광범위한 프리마퀸 사용 및 효능을 손상시킵니다(Cheoymang et al. 2015; Grietens et al. 2010; Leslie et al. 2004; Maneeboonyang et al. 2011). 최근 서서히 제거되는 8-아미노퀴놀린인 타페노퀸이 출시되어 여러 국가에 등록되었습니다. 타페노퀸은 감독된 투여가 가능한 단일 용량으로 제공될 수 있습니다(Lacerda et al. 2019; Llanos-Cuentas et al. 2014a; Llanos-Cuentas et al. 2019). 그러나 tafenoquine은 G6PD 결핍에서 심각한 용혈을 유발할 수 있으므로 G6PD 활성이 70% 미만인 사람에게는 금기입니다(현재 정성 검사에서 정상으로 검사할 수 있는 여성 이형 접합체 포함). 따라서 정량적 G6PD 결핍 검사는 여성 G6PD 결핍 이형 접합체가 등록되지 않았음을 보장하기 위해 중간 G6PD 활동을 가진 개인을 식별하는 데 필요합니다. 정량적 결과를 제공하는 근거리 환자 정량적 G6PD 검사(G6PD Biosensor)(Bancone et al. 2018; Pal et al. 2019; Zobrist et al. 2021)는 타페노퀸이 처방되기 전에 사용될 것입니다.
현재 권장되는 타페노퀸 용량은 차선책입니다. 300mg 용량은 동남아시아 지역으로 제한했을 때 3상 연구의 메타 분석에서 저용량 프리마퀸보다 현저히 열등한 것으로 입증되었습니다(Llanos-Cuentas et al. 2019; Watson et al. 2022).
이러한 데이터와 사전 등록 연구(Watson et al. 2022)의 모든(>1000) 환자에 대한 개별 환자 데이터 메타 분석은 동남아시아 및 서태평양 지역에서 450mg 성인 용량의 타페노퀸이 필요함을 시사합니다(그림 2-3). 1200mg까지의 용량은 G6PD 정상인에게 안전하고 내약성이 우수한 것으로 입증되었습니다.
그러나 최근 연구에 따르면 ACT는 타페노퀸의 치료 효능을 길항할 수 있습니다. 이것이 사실이라면 단일 혈액 단계 약물을 사용하여 열대열 말라리아와 vivax 말라리아 모두의 현재 치료를 복잡하게 만들 것입니다. 비 풍토병 지역으로 돌아온 인도네시아 군인에 대한 INSPECTOR 임상시험 결과 타페노퀸을 DHA-PPQ와 병용했을 때(21%) 매우 낮은 급진적 치료 효능이 나타났습니다. 외 2020).
이로 인해 Krintafel®(미국의 브랜드 타페노퀸 형태)에 대한 제품 라벨 업데이트가 촉발되었으며, 타페노퀸은 아르테미시닌 기반 병용 요법(ACT)과 같은 다른 항말라리아제와 함께가 아니라 클로로퀸과만 병용해야 합니다(CDC 2020). 이것은 이제 국립 말라리아 프로그램이 vivax 말라리아에 대한 ACT를 권장하는 클로로퀸 내성 P. vivax 기생충이 있는 지역에서 타페노퀸의 사용을 방지합니다. 과학적 근거
단회 투여 요법인 타페노퀸은 근본적인 치료에 필요한 더 긴 과정의 프리마퀸에 비해 엄청난 이점이 있습니다. ACT와 함께 사용할 수 없는 경우 사용이 점진적으로 제한됩니다. 이 시험은 DHA-PPQ가 클로로퀸과 비교하여 타페노퀸 라디칼 치료 효능을 유의하게 감소시키는지 여부를 평가할 것입니다.
