Ticagrelor met aspirine of alleen bij hoogrisicopatiënten na coronaire interventie (TWILIGHT)
Het doel van deze studie is om het gebruik van ticagrelor alleen te vergelijken met ticagrelor en aspirine samen. Zowel ticagrelor als aspirine voorkomen dat bloedplaatjes aan elkaar kleven en een bloedstolsel vormen dat de bloedtoevoer naar het hart kan blokkeren. In deze studie wordt gekeken naar de effectiviteit en veiligheid van alleen ticagrelor, vergeleken met ticagrelor plus aspirine, bij het verminderen van klinisch relevante bloedingen en het verminderen van ischemische bijwerkingen bij hoogrisicopatiënten die een percutane interventie hebben ondergaan met ten minste één geneesmiddelafgevende stent. Een patiënt wordt als een hoog risico beschouwd als hij aan bepaalde klinische en/of anatomische criteria voldoet.
Maximaal 9000 proefpersonen zullen worden ingeschreven op het moment van hun index-PCI. Proefpersonen die 3 maanden na inschrijving aan de criteria voor randomisatie voldoen, worden gedurende nog eens 12 maanden gerandomiseerd naar ticagrelor plus aspirine of ticagrelor plus placebo. Follow-upkliniekbezoeken zullen worden uitgevoerd op 3 maanden, 9 maanden en 15 maanden na inschrijving.

Dit is een multicenter, prospectieve, geblindeerde studie met twee armen. Tot 9000 hoogrisicopatiënten die een succesvolle PCI hebben ondergaan met ten minste één lokaal goedgekeurde medicijnafgevende stent die is ontladen op DAPT met aspirine en ticagrelor voor een beoogde duur van ten minste 3 maanden uit nog nader te bepalen centra in de VS, Canada, Europa en Azië . Het primaire doel van deze studie is het bepalen van de impact van plaatjesaggregatieremmende monotherapie met alleen ticagrelor versus DAPT met ticagrelor plus aspirine gedurende 12 maanden bij het verminderen van klinisch relevante bloedingen (werkzaamheid) bij hoog-risicopatiënten die een PCI ondergaan en die een kuur van 3 maanden hebben voltooid. aspirine plus ticagrelor. Beschrijving
Inclusiecriteria:
- Patiënten met een hoog risico die een succesvolle PCI hebben ondergaan met ten minste één lokaal goedgekeurde medicijnafgevende stent die is ontladen op DAPT met aspirine en ticagrelor voor een beoogde duur van ten minste 3 maanden, komen in aanmerking voor de TWILIGHT-studie.
- Voor deelname aan het onderzoek moet aan ten minste één klinische inclusie, één angiografische inclusie en geen van de uitsluitingscriteria worden voldaan.
Criteria voor klinische opname:
- Volwassen patiënten ≥ 65 jaar
- Vrouwelijk geslacht
- Troponine Positief acuut coronair syndroom
- Gevestigde vasculaire ziekte gedefinieerd als eerder MI, gedocumenteerde PAD of CAD/PAD revascularisatie
- Diabetes mellitus behandeld met medicijnen (orale hypoglykemie, subcutane injectie van insuline)
- Chronische nierziekte gedefinieerd als een geschatte glomerulaire filtratiesnelheid (eGFR) < 60 ml/min/1,73 m2 of creatinineklaring (CrCl) < 60 ml/min
Angiografische opnamecriteria:
- Multivat coronaire hartziekte
- Doellaesie die totale stentlengte >30 mm vereist
- Trombotische doellaesie(s)
- Bifurcatie laesies met Medina X,1,1 classificatie waarvoor minimaal 2 stents nodig zijn
- Linker hoofd (≥50%) of proximale LAD (≥70%) laesie
- Verkalkte doellaesie(s) die atherectomie vereisen
Uitsluitingscriteria:
- Onder de 18 jaar
- Contra-indicatie voor aspirine
- Contra-indicatie voor ticagrelor
- Geplande operatie binnen 90 dagen
- Geplande coronaire revascularisatie (chirurgisch of percutaan) binnen 90 dagen
- Behoefte aan chronische orale antistolling
- Eerdere beroerte
- Dialyse-afhankelijk nierfalen
- Actieve bloeding of extreem risico op ernstige bloeding (bijv. actieve maagzweer, gastro-intestinale pathologie met een verhoogd risico op bloedingen, maligniteiten met een verhoogd risico op bloedingen)
- Salvage PCI- of STEMI-presentatie.
- Levercirrose
- Levensverwachting < 1 jaar
- Geïnformeerde toestemming niet kunnen of willen geven
- Vrouwen in de vruchtbare leeftijd. Gedefinieerd: een vrouw wordt beschouwd als potentieel (WOBCP) na de menarche en totdat ze postmenopauzaal wordt, tenzij permanent onvruchtbaar. Permanente sterilisatiemethoden omvatten hysterectomie, bilaterale salpingectomie en bilaterale ovariëctomie. een postmenopauzale toestand wordt gedefinieerd als geen menstruatie gedurende 12 maanden zonder alternatieve medische oorzaak.
- Fibrinolytische therapie binnen 24 uur na index-PCI
