Vi står overfor et dyptgående og fremvoksende folkehelseproblem i form av akutt og kronisk hjernedysfunksjon blant overlevende på intensivavdelinger for unge og eldre som endrer samfunnets landskap.
Hele to tredjedeler av ICU-pasienter utvikler delirium, som er assosiert med lengre opphold, milliarder av dollar i kostnader globalt og 3 ganger overdødelighet etter 6 måneder.
Over halvparten av ICU-overlevende lider av en funksjonssvekkende demenslignende sykdom, som ser ut til å være relatert til deliriums varighet.
Påvirkningen på en persons liv er ofte ødeleggende.
Hovedmålet med det foreslåtte INQRI-prosjektet er å implementere, analysere og spre et evidensbasert, sykepleierledet, tverrprofesjonelt, flerkomponentprogram fokusert på å forbedre omsorgen og resultatene til kritisk syke voksne.
Studien vil fokusere på å bruke Awakening-Breathing Coordination, Delirium Monitoring/Management & Early Mobility (ABCDE), et program for deliriumscreening, forebygging og behandling utviklet ved Vanderbilt University.
ABCDE-tilnærmingen analyserer og slår sammen de beste tilgjengelige bevisene knyttet til delirium, analgesi og sedasjon (DAS) behandling på intensivavdelingen og skreddersyr de farmakologiske og ikke-farmakologiske intervensjonene brukt i tidligere DAS kliniske studier til et program som kan tas i bruk i praksis på mindre " forskningsfokuserte" intensivavdelinger.
Spesifikt er studiens mål å (1) implementere ABCDE-programmet i et medisinsk senter som for tiden ikke utfører rutinemessige deliriumscreeninger på intensivavdelingen og identifisere tilretteleggere og barrierer for programadopsjon; (2) teste virkningen av ABCDE-programmet på pasientresultater, sykepleiekvalitetsresultater og systemresultater; og (3) vurdere i hvilken grad ABCDE-implementering er effektiv, bærekraftig og bidrar til spredning til andre miljøer.

Det primære resultatet vil være en sammenligning mellom 3-måneders baseline-perioden med respiratorfrie dager og 9-månedersperioden etter intervensjonen.
