Descrição detalhada
Este estudo é um estudo de fase IIIb, multicêntrico, aberto e randomizado de asciminibe oral 80 mg uma vez ao dia (QD) versus nilotinibe 300 mg duas vezes ao dia (BID) em pacientes adultos com Ph+ CML-CP recentemente diagnosticado.
Os participantes serão randomizados no estudo em uma proporção de 1:1 para asciminib ou nilotinib. Nenhum cruzamento do tratamento do estudo entre os braços será permitido.
Os participantes serão tratados até toxicidade inaceitável, progressão da doença e/ou a critério do investigador ou dos participantes. Uma visita/chamada de acompanhamento de segurança será realizada aproximadamente 30 dias após o término da visita de tratamento. Os participantes que descontinuarem o tratamento do estudo prematuramente devido a qualquer motivo, serão acompanhados para sobrevivência e progressão (para Fase Acelerada (AP)/Crise Blástica (BC)) até o final do estudo.

Critério de inclusão:
- Pacientes com CML-CP dentro de 3 meses após o diagnóstico.
Diagnóstico de LMC-CP (critérios ELN 2020) com confirmação citogenética do cromossomo Filadélfia
A LMC em fase crônica documentada atenderá a todos os critérios abaixo Baccarani et al 2013:
- < 15% de blastos no sangue periférico e na medula óssea,
- < 30% de blastos mais promielócitos no sangue periférico e na medula óssea,
- < 20% de basófilos no sangue periférico,
- Contagem de PLT ≥ 100 x 10^9/L (≥ 100.000/mm3), exceto trombocitopenia induzida por tratamento
- Sem evidência de envolvimento leucêmico extramedular, com exceção de hepatoesplenomegalia.
- Evidência de transcrição típica de BCR::ABL1 [e14a2 e/ou e13a2] passível de quantificação padronizada de RQ-PCR pela avaliação laboratorial central.
- Status de desempenho ECOG de 0 ou 1.
Função adequada do órgão final, conforme definido por:
- Bilirrubina total (TBL) < 3 x LSN; pacientes com síndrome de Gilbert só podem ser incluídos se TBL ≤ 3,0 x LSN ou bilirrubina direta ≤ 1,5 x LSN,
- CrCl ≥ 30 mL/min calculado usando a fórmula de Cockcroft-Gault, Lipase sérica ≤ 1,5 x LSN. Para lipase sérica > LSN - ≤ 1,5 x LSN, o valor deve ser considerado não significativo clinicamente e não associado a fatores de risco para pancreatite aguda.
Os pacientes devem ter os seguintes valores laboratoriais dentro dos limites normais ou corrigidos para dentro dos limites normais com suplementos antes da randomização:
- Potássio (aumento de potássio de até 6,0 mmol/L é aceitável se associado a CrCl* ≥ 90 mL/min),
- Cálcio total (corrigido para albumina sérica); (aumento de cálcio de até 12,5 mg/dl ou 3,1 mmol/L é aceitável se associado a CrCl* ≥ 90 mL/min),
- Magnésio (aumento de magnésio de até 3,0 mg/dL ou 1,23 mmol/L se associado a CrCl* ≥ 90 mL/min),
Para pacientes com insuficiência renal leve a moderada (CrCl* ≥ 30 mL/min e <90 mL/min) - potássio, cálcio total (corrigido para albumina sérica) e magnésio devem estar dentro dos limites normais ou corrigidos para dentro dos limites normais com suplementos antes à randomização.
- CrCl calculado usando a fórmula de Cockcroft-Gault.
Critério de exclusão:
- Tratamento prévio de LMC com quaisquer outros agentes anticancerígenos, incluindo quimioterapia e/ou agentes biológicos ou transplante prévio de células-tronco, com exceção de hidroxiureia e/ou anagrelida.
- Infiltração do SNC confirmada citopatologicamente conhecida (na ausência de suspeita de envolvimento do SNC, punção lombar não é necessária).
Função cardíaca prejudicada ou anormalidade da repolarização cardíaca, incluindo, entre outros, qualquer um dos seguintes:
- História de infarto do miocárdio (IM), angina pectoris, enxerto de revascularização do miocárdio (CABG) dentro de 6 meses antes de iniciar o tratamento do estudo.
- Arritmias cardíacas clinicamente significativas (por exemplo, taquicardia ventricular), bloqueio completo do ramo esquerdo, bloqueio AV de alto grau (por exemplo, bloqueio bifascicular, Mobitz tipo II e bloqueio AV de terceiro grau).
- QTcF ≥ 450 ms (pacientes do sexo masculino), ≥ 460 ms (pacientes do sexo feminino) na média de três ECG basais seriados (usando a fórmula QTcF). Se QTcF ≥ 450 ms e os eletrólitos não estiverem dentro dos limites normais, os eletrólitos devem ser corrigidos e, em seguida, o paciente deve ser rastreado novamente para QTcF.
- Síndrome do QT longo, história familiar de morte súbita idiopática ou síndrome do QT longo congênito ou qualquer um dos seguintes:
- Fatores de risco para Torsades de Pointes (TdP), incluindo hipocalemia ou hipomagnesemia não corrigida, história de insuficiência cardíaca ou história de bradicardia clinicamente significativa/sintomática.
- Medicação(ões) concomitante(s) com um "risco conhecido de Torsades de Pointes" de acordo com www.crediblemeds.org que não pode ser descontinuado ou substituído 7 dias antes do início do medicamento do estudo por medicação alternativa segura.
- Incapacidade de determinar o intervalo QTcF.
- Doença médica concomitante grave e/ou não controlada que, na opinião do Investigador, pode causar riscos de segurança inaceitáveis ou comprometer a conformidade com o protocolo (por exemplo, diabetes descontrolada, infecção ativa ou descontrolada; hipertensão arterial ou pulmonar não controlada, hiperlipidemia clinicamente significativa não controlada).
- História de distúrbio hemorrágico congênito ou adquirido significativo não relacionado ao câncer.
- Grande cirurgia dentro de 4 semanas antes da entrada no estudo ou pacientes que não se recuperaram de uma cirurgia anterior.
- História de outra malignidade ativa dentro de 3 anos antes da entrada no estudo, com exceção de câncer de pele basocelular anterior ou concomitante e carcinoma in situ anterior tratado curativamente.
- História de pancreatite aguda dentro de 1 ano antes da randomização ou história médica de pancreatite crônica.
- História de doença hepática crônica levando a insuficiência hepática grave ou doença hepática aguda em curso.
- História conhecida de infecção crônica por Hepatite B (HBV) ou Hepatite C (HCV). O teste para antígeno de superfície da hepatite B (HBs Ag) e anticorpo central da hepatite B (HBc Ab/anti HBc) será realizado na triagem. Se o anti-HBc for positivo, a avaliação do HBV-DNA será realizada na triagem. Um paciente com HBV-DNA positivo não será incluído no estudo. Além disso, um paciente com HBsAg positivo não será incluído no estudo. O teste HCV Ab também será realizado na triagem. Para obter detalhes sobre os critérios, consulte o Apêndice 4.
- Histórico de Vírus da Imunodeficiência Humana (HIV), a menos que esteja bem controlado com uma dose estável de terapia antirretroviral no momento da triagem.

TTDAE is defined as the interval from the date of first study treatment administration to the date of discontinuation of study treatment due to an adverse event (AE).
The comparison between the asciminib and nilotinib treatment arms is performed using a cause-specific hazard model, in which discontinuations due to AE are considered the event of interest and discontinuations for reasons other than AE are treated as competing risks.
The number of participants who discontinued study treatment due to AE within the specified timeframe is presented in the results table.
The formal comparison between treatment arms is performed using a cause-specific hazard model, and the corresponding hazard ratio, confidence interval, and p-value are provided in the Statistical Analysis section.

The EORTC QLQ-C30 contains 30 items and is composed of both multi-item scales and single-item measures based on the participant's experience over the past week.
These include five functional scales (physical, role, emotional, cognitive and social functioning), three symptom scales (fatigue, nausea/vomiting, and pain), six single items (dyspnea, insomnia, appetite loss, constipation, diarrhea and financial impact) and a global health status/QoL scale.

The EORTC QLQ-CML24 assesses specific concepts relevant to the experience of patients with CML.
The QLQ-CML24 has 24 items which assess symptom burden, impact on daily life and on worry/mood, body image problems, and satisfaction with care and with social life based on the participant's experience over the past week.
