主要目的是确定除短效 β 受体激动剂和口服皮质类固醇(如果有指征)外,从儿科急诊科 (PED) 开具吸入性皮质类固醇处方对 28 天内复发率的影响。 次要目标包括该干预措施对研究期间住院率和哮喘生活质量的影响。
选择标准包括年龄≤18 岁且主诉与哮喘恶化一致的患者,这些主诉与先前由医生诊断为哮喘或哮喘预测指数 (API) 中的一个主要患者在过去一年中有两次喘息发作。 在这次就诊期间接受口服皮质类固醇作为急性哮喘发作治疗的一部分并且在干预后被主治医师认为足够好出院的儿童将被招募入组。 排除标准包括在就诊前 4 周内接受哮喘控制药物治疗或对干预哮喘控制药物过敏的患者。
患者将使用随机数生成器以 1:1 的比例随机分配到干预组或标准护理(对照组)组。 分配到干预组的患者将在出院时接受哮喘控制药物治疗。 干预组将从 PED 获得为期一个月的低剂量吸入皮质类固醇。 5 岁以下的患者或喜欢雾化药物的患者将接受一个月的低剂量 Pulmicort(布地奈德)溶液 0.25mg/respule,每天两次通过雾化器给药。 ≥5 岁的患者将接受一种低剂量 QVAR(二丙酸倍氯米松)计量吸入器 (MDI) 40 微克/喷,并附有说明,每天两次喷两次,使用间隔器。 分配到对照组的患者将不会接受 PED 提供的哮喘控制药物。 两组都将按照标准治疗和吸入沙丁胺醇接受口服皮质类固醇的处方。 迷你小儿哮喘控制工具 (MPACT) 是一种经过验证的问卷,用于快速识别 PED 中的持续性哮喘症状,也将在出院前进行管理,以评估持续性哮喘症状。

纳入标准:
- 与哮喘发作一致的主诉

我们用于本研究的吸入性皮质类固醇是布地奈德雾化溶液和倍氯米松定量吸入器。
这些药物的低剂量布地奈德为 0.5 毫克/天,倍氯米松为 160 微克/天。
5 岁以下患者将接受低剂量布地奈德溶液 0.25mg/respule,每天两次通过雾化器给药。
布地奈德已获 FDA 批准用于 5 岁以下儿童。
