Multiple Risk Factor Intervention Trial for Prevention of Coronary Heart Disease (MRFIT)

POZADÍ:
Výzkumná zjištění z prospektivních epidemiologických studií u řady populací ve Spojených státech za posledních 40 let přesvědčivě prokázala, že zvýšený cholesterol v krvi, zvýšený krevní tlak a kouření cigaret jsou rizikové faktory spojené s urychleným rozvojem infarktu myokardu, náhlou smrtí a úmrtí na ischemickou chorobu srdeční. Nejsou to jediné rizikové faktory spojené s předčasným rozvojem ICHS, ale tyto faktory jsou modifikovatelné a poskytují silné očekávání, že snížení těchto faktorů by mělo mít příznivý výsledek ve snížení míry rozvoje předčasných srdečních záchvatů.
Multifaktorový přístup ke snížení rizikových faktorů se velmi podobá typu intervence, kterou by provedl lékař při léčbě pacienta. Obecnou koncepci takové studie schválila Pracovní skupina pro arteriosklerózu ve své zprávě z roku 1970 pro Institut.
Dobrovolníci byli vybráni do procesu prostřednictvím série tří prověřovacích návštěv, které začaly v listopadu 1973 a byly dokončeny do 28. února 1976. Z 361 662 mužů pozorovaných při prvním screeningu bylo 12 866 shledáno jako způsobilých a byli randomizováni do studie. Polovina účastníků byla přidělena do speciální intervenční skupiny a polovina do obvyklé pečovatelské skupiny. Speciální intervenční skupině bylo doporučeno dodržovat stravovací vzorec navržený tak, aby měl za následek příjem živin 30 až 35 procent kalorií z tuku, 10 (později 8) procent z nasycených a 10 procent z polynenasycených tuků; přibližně 300 (později 250) mg cholesterolu; a úprava sacharidů dle potřeby pro individuální požadavky. Tato skupina byla také povzbuzena, aby přestala kouřit cigarety kombinací technik, včetně poradenství a audiovizuálních pomůcek. Léčba hypertenze byla založena na programu odstupňované péče spočívajícím v redukci hmotnosti a lécích podobných těm, které se používají v programu pro detekci a sledování hypertenze. Osoby v obvyklé pečovatelské skupině byly odeslány ke svému osobnímu lékaři nebo k jinému zdroji péče pro takové řízení jejich rizikových faktorů, které tito poskytovatelé považovali za vhodné. Primárním cílovým parametrem byla smrt v důsledku ischemické choroby srdeční. Muži ve skupinách se speciální intervencí i s obvyklou péčí se každoročně vraceli k posouzení změn v hladinách rizikových faktorů. Byla odebrána anamnéza a provedeno fyzikální vyšetření. Byl také získán elektrokardiogram pro centralizované čtení k identifikaci nefatálního infarktu myokardu jako dalšího koncového bodu.
Studie se zúčastnilo 22 klinických center po celých Spojených státech, koordinační centrum, centrální laboratoř, dvě EKG odečítací centra a laboratoř pro standardizaci lipidů a také distribuční centrum léků. Studie skončila a hlavní výsledky úmrtnosti byly zveřejněny v září 1982. Aktivní sledování bylo ukončeno podle plánu 28. února 1982, přičemž konečné stanovení vitálního stavu k tomuto datu bylo provedeno u 99,6 procenta studované populace.
DESIGNOVÝ PŘÍBĚH:
Randomizovaný, neslepý design se dvěma skupinami a pevnou velikostí vzorku. Experimentální skupina podstoupila dietu s upraveným obsahem tuků, antihypertenzní terapii a edukaci týkající se životního stylu a kouření. Ti v kontrolní skupině byli posláni k léčbě svým vlastním lékařům.
Post-trial sledování úmrtnosti 12 300 účastníků stále žijících na konci aktivní intervence v únoru 1982 pokračovalo až do června 1998 v rámci grantové podpory (R01HL43232). Použití národního indexu úmrtí pro sledování úmrtnosti 361 662 mužů vyšetřených na MRFIT pokračovalo až do července 1998 s grantovou podporou (R01HL28715). Nedávné analýzy se zaměřují na vztah dietních faktorů, jako je sodík, během studie s úmrtností po ukončení studie. Probíhají práce na dalších prediktorech diabetes mellitus; ukazatele nemocnosti kardiovaskulárních chorob a rizika úmrtí; a prognostický význam změn v ukazatelích hypertrofie a ischemie levé komory.
