Ensaio de Intervenção de Fatores de Risco Múltiplos para a Prevenção da Doença Cardíaca Coronariana (MRFIT)

Resultados de pesquisas de estudos epidemiológicos prospectivos em várias populações nos Estados Unidos ao longo dos últimos 40 anos estabeleceram de forma convincente que níveis elevados de colesterol no sangue, pressão arterial elevada e tabagismo são fatores de risco associados ao desenvolvimento acelerado de infarto do miocárdio, morte súbita e morte por doença coronariana. Esses não são os únicos fatores de risco associados ao desenvolvimento prematuro de DCC, mas esses fatores são modificáveis e fornecem uma forte expectativa de que uma redução desses fatores deve ter um resultado benéfico na redução da taxa de desenvolvimento de ataques cardíacos prematuros.
Uma abordagem multifatorial para a redução dos fatores de risco se assemelha muito ao tipo de intervenção que seria realizada por um médico no tratamento de um paciente. O conceito geral para tal ensaio foi endossado pela Arteriosclerosis Task Force em seu relatório de 1970 para o Instituto.
Os voluntários foram selecionados para o teste por meio de uma série de três visitas de triagem, que começaram em novembro de 1973 e foram concluídas em 28 de fevereiro de 1976. Entre 361.662 homens atendidos na primeira triagem, 12.866 foram considerados elegíveis e foram randomizados para o estudo. Metade dos participantes foi designada para o grupo de intervenção especial e metade para o grupo de cuidados habituais. O grupo de intervenção especial foi aconselhado a seguir um padrão alimentar projetado para resultar em uma ingestão de nutrientes de 30% a 35% das calorias provenientes de gordura, com 10 (mais tarde 8) por cento de gordura saturada e 10% de gordura poliinsaturada; aproximadamente 300 (mais tarde 250) mg de colesterol; e modificação de carboidratos conforme necessário para requisitos individuais. Esse grupo também foi encorajado a parar de fumar por meio de uma combinação de técnicas, incluindo aconselhamento e recursos audiovisuais. O tratamento da hipertensão baseou-se em um programa escalonado de redução de peso e medicamentos semelhantes aos utilizados no Programa de Detecção e Acompanhamento da Hipertensão. Aqueles no grupo de cuidados habituais foram encaminhados ao seu médico pessoal ou outra fonte de cuidados para o gerenciamento de seus fatores de risco, conforme considerado apropriado por esses provedores. O desfecho primário foi a morte por doença coronariana. Homens em ambos os grupos de intervenção especial e cuidados habituais retornaram para avaliação das mudanças nos níveis de fator de risco anualmente. Uma história médica foi tomada e um exame físico realizado. Um eletrocardiograma também foi obtido para leitura centralizada para identificar infarto do miocárdio não fatal como um desfecho adicional.
Participaram do estudo 22 centros clínicos nos Estados Unidos, um centro coordenador, um laboratório central, dois centros de leitura de ECG e um laboratório de padronização de lipídios, bem como um centro de distribuição de medicamentos. O ensaio foi concluído, com os principais resultados de mortalidade publicados em setembro de 1982. O acompanhamento ativo foi encerrado conforme programado em 28 de fevereiro de 1982, com a determinação final do estado vital nessa data realizada para 99,6% da população do estudo.
NARRATIVA DO DESENHO:
Um design randomizado, não cego, com dois grupos e tamanho de amostra fixo. O grupo experimental foi submetido a uma dieta com gordura modificada, terapia anti-hipertensiva e educação relacionada ao estilo de vida e tabagismo. Aqueles no grupo de controle foram encaminhados para seus próprios médicos para tratamento.
A vigilância da mortalidade pós-pesquisa dos 12.300 participantes que ainda viviam no final da intervenção ativa em fevereiro de 1982 continuou até junho de 1998 sob o apoio financeiro (R01HL43232). O uso do Índice Nacional de Óbitos para vigilância da mortalidade dos 361.662 homens rastreados para o MRFIT continuou até julho de 1998 sob o apoio financeiro (R01HL28715). Análises recentes enfocam a relação de fatores dietéticos, como o sódio, durante o ensaio com a mortalidade pós-estudo. Documentos estão em andamento sobre outros preditores de diabetes mellitus; indicadores de morbidade por doença cardiovascular e risco de morte; e importância prognóstica das alterações nos índices de hipertrofia e isquemia ventricular esquerda.
