Beskrivelse
Inklusionskriterier:
- Være ≥ 18 år på dagen for underskrivelse af informeret samtykke.
- Hav en forventet levetid på mindst 3 måneder.
- Har en histologisk eller cytologisk bekræftet diagnose af stadium IV NSCLC.
- Har en præstationsstatus på 0 eller 1 på ECOG.
- Har en målbar sygdom baseret på RECIST 1.1.
- Har leveret væv fra en arkivvævsprøve eller nyligt opnået kerne- eller excisionsbiopsi af tumorlæsion.
- Hos patienter med ikke-pladecellet ikke-småcellet lungecancer skal efterforskere være i stand til at producere kildedokumentation af EGFR-mutationsstatus eller ALK-translokationsstatus.
- Demonstrere tilstrækkelig organfunktion.
- Kvindelige patienter i den fødedygtige alder bør have en negativ urin- eller serumgraviditetstest inden for 72 timer.
- Kvindelige forældre i den fødedygtige alder skal være villige til at bruge 2 præventionsmetoder eller være kirurgisk sterile.
- Mandlige patienter skal acceptere at bruge en passende præventionsmetode.
- Patienter med sensibiliserende EGFR-mutation eller ALK-omlejring skal have udviklet sig på en passende tyrosinkinasehæmmer (TKI)
Ekskluderingskriterier:
- Har tidligere modtaget behandling med kemoterapi eller biologisk behandling for stadium IV NSCLC.
- Deltager i øjeblikket i eller har deltaget i en undersøgelse af et forsøgsmiddel eller bruger et forsøgsudstyr.
- Har en diagnose af immundefekt eller modtager systemisk steroidbehandling eller enhver anden form for immunsuppressiv terapi.
- Har tidligere haft en mAb inden for 4 uger før undersøgelsesdag 1, eller som ikke er kommet sig over bivirkninger på grund af midler administreret mere end 4 uger tidligere.
- Har tidligere haft kemoterapi eller stråling.
- Har en kendt yderligere malignitet, der skrider frem eller kræver aktiv behandling.
- Har kendte aktive CNS-metastaser og/eller karcinomatøs meningitis.
- Har en aktiv autoimmun sygdom, der kræver systemisk behandling inden for de seneste 3 måneder eller en dokumenteret anamnese med klinisk alvorlig autoimmun sygdom, eller et syndrom, der kræver systemiske steroider eller immunsuppressive midler.
- Har tegn på interstitiel lungesygdom eller aktiv, ikke-infektiøs pneumonitis.
- Har en aktiv infektion, der kræver systemisk terapi.
- Har en historie eller aktuelle beviser for enhver tilstand, terapi eller laboratorieabnormitet, der kan forvirre resultaterne af forsøget.
- Har kendte psykiatriske eller stofmisbrugslidelser.
- Er gravid eller ammer, eller forventer at blive gravid eller få børn inden for den forventede varighed af forsøget.
- Har modtaget tidligere behandling med et anti-PD-1, anti-PD-L1, anti-PD-L2, anti-CD137 eller CTLA-4 antistof.
- Har en kendt historie med HIV.
- Har kendt aktiv Hepatitis B eller Hepatitis C.
- Har modtaget en levende vaccine inden for 30 dage før den planlagte første dosis af undersøgelsesterapi.
- Har en kendt historie med aktiv TB.
- Overfølsomhed over for pembrolizumab eller et eller flere af dets hjælpestoffer.
