Predittori vantaggiosi dello studio APACE (Acute Coronary Syndromes Evaluation). (APACE)
Il triage dei pazienti con sospetta sindrome coronarica acuta al pronto soccorso è una sfida diagnostica che richiede molto tempo. Pertanto sono necessari marcatori precoci altamente sensibili del danno miocardico per escludere più rapidamente un infarto miocardico acuto (IMA), soprattutto per le prime 3-4 ore dopo l'insorgenza del dolore toracico nell'AMI (periodo "cieco per la troponina").
Pertanto testiamo l'ipotesi che l'uso di un'anamnesi meticolosa del paziente e di nuovi marcatori cardiaci possa fornire un rilevamento o un'esclusione più rapida dell'IMA nei pazienti che si presentano con dolore toracico acuto al pronto soccorso.
Lo studio prospettico di coorte è progettato per arruolare pazienti che presentano dolore toracico acuto a riposo nelle ultime 12 ore al pronto soccorso. Verranno prelevati diversi campioni di sangue per il rilevamento dei nuovi marcatori e confrontati con il gold standard per la diagnosi di AMI (troponina cardiaca T ad alta sensibilità). Tutti i pazienti verranno contattati telefonicamente a 3, 12, 24 e 60 mesi per determinare lo stato funzionale, gli eventi cardiaci avversi maggiori (morte, infarto miocardico, bypass aortocoronarico, intervento coronarico percutaneo) e i risultati dell'esame cardiaco (test da sforzo , angiografia coronarica) se eseguita.

Background: Il triage dei pazienti con sospetta sindrome coronarica acuta nel pronto soccorso è una sfida diagnostica che richiede molto tempo. Il triage e la gestione dei pazienti con bassa probabilità di malattia coronarica spesso causano costi ospedalieri eccessivi. Pertanto sono necessari marcatori precoci altamente sensibili del danno miocardico per escludere più rapidamente un infarto miocardico acuto (IMA).
Le troponine cardiache (T e I) rappresentano attualmente il gold standard per la diagnosi definitiva di IMA grazie alla loro elevata sensibilità e specificità per il rilevamento del danno cellulare miocardico. Sfortunatamente, la troponina non è rilevabile dagli attuali test nel sangue periferico entro 3-4 ore dall'insorgenza del dolore toracico nell'IMA (periodo "cieco per la troponina").
Nuovi marcatori cardiaci come la nuova troponina I/T ad alta sensibilità, l'albumina modificata per ischemia e il fattore di crescita placentare hanno dimostrato alcuni vantaggi rispetto alla troponina come un elevato valore predittivo negativo per l'IMA, una verificabilità più rapida nel sangue periferico e un possibile valore come marcatore di rischio indipendente . Tuttavia, manca ancora la valutazione clinica in un’ampia coorte di pazienti non selezionati che si presentano al pronto soccorso.
Scopo: Testare l'ipotesi che l'uso di un'anamnesi meticolosa del paziente e di nuovi marcatori cardiaci (tra cui troponina I/T ad alta sensibilità, mieloperossidasi, albumina modificata per ischemia, fattore di crescita placentare) possa fornire un rilevamento o un'esclusione più rapida dell'IMA nei pazienti che presentano IMA acuto dolore al petto al pronto soccorso.
Pazienti e metodi: Lo studio prospettico di coorte è progettato per arruolare pazienti non selezionati che si presentano con dolore toracico acuto a riposo nelle ultime 12 ore al pronto soccorso. Verranno prelevati diversi campioni di sangue per il rilevamento dei nuovi marcatori (basale, 1, 2, 3 e 6 ore) e confrontati con il gold standard per la diagnosi di IMA (troponina cardiaca T ad alta sensibilità). La tempistica e il trattamento dei pazienti sono lasciati alla discrezione del medico curante e verranno eseguiti secondo la routine domiciliare standard dell'ospedale. Tutti i pazienti verranno contattati telefonicamente a 6, 12, 24 e 60 mesi per determinare lo stato funzionale, gli eventi cardiaci avversi maggiori (morte, infarto miocardico, bypass aortocoronarico, intervento coronarico percutaneo) e i risultati dell'esame cardiaco (test da sforzo , angiografia coronarica) se eseguita.
Risultati attesi: La nostra ipotesi è che l'uso meticoloso dell'anamnesi del paziente e di nuovi marcatori cardiaci possa migliorare l'individuazione dell'IMA fornendo una diagnosi precoce dell'IMA con un elevato valore predittivo negativo entro il periodo "cieco per la troponina".
Significato: il rilevamento precoce di necrosi miocardica nel sangue periferico potrebbe aiutare a escludere più rapidamente l'IMA. Inoltre consentirà una diagnosi più rapida e una terapia adeguata dell'IMA. Ciò può portare ad un miglioramento significativo nella gestione dei pazienti e ad una riduzione dei costi ospedalieri.

- Tipica angina pectoris o altre sensazioni toraciche che si sospetta siano causate da ischemia miocardica
- Sintomi a riposo o da sforzo minore
- Insorgenza dei sintomi nelle ultime 12 ore prima della presentazione
- Consenso informato scritto
Criteri di esclusione:
- Età < 18 anni
- Shock cardiogeno
- Malattia renale terminale che richiede dialisi regolare
