バックグラウンド:
症候性冠状動脈性心疾患患者の管理は、過去 20 年間で大幅に進化し、患者の割合が増え続けています。
過去 20 年間の CABG 手術の改良により、手術による心筋血行再建術は、心筋虚血のほとんどの患者で実行可能です。 冠動脈疾患患者の内科的管理と外科的管理を比較した、複数の多施設ランダム化試験が報告されています。 3つの最大の試験と広範な観察文献に基づいて、内科的または外科的管理が選択される方法であると思われる状況に関してコンセンサスが生まれているようです.
重度の症状を有する大多数の患者において、CABG が狭心症を緩和することは明らかです。 この手順は、経験豊富な手術チームによって非常に低いリスクで実行できることも明らかです。 したがって、CABG は、冠状動脈の構造が適切で、内科治療に抵抗性の重度の狭心症を有する患者、および左主冠動脈の重大な閉塞を有する患者に適応となります。
重症度の低い狭心症の患者に対する手術介入のタイミングは、現在いくつかの論争のポイントです。 しかし、特定のカテゴリーの患者では、待機的バイパス手術後の生存率が改善したことは明らかです。 これらのカテゴリーには、左主冠動脈狭窄症、心室機能がやや損なわれた三重血管疾患の患者、およびおそらく他の臨床的に定義された高リスク患者が含まれます。 軽度の症候性冠動脈疾患を有する残りの患者は、症状が悪化して緩和が必要になるまで、手術介入を延期する余裕があるように思われる.

PTCA は 1977 年にヒトの冠動脈疾患に初めて適用されました。 カテーテルの設計、バルーンの構造、およびこの処置のリスクの高い患者と低い患者の両方の識別に大きな進歩がありました。 経験豊富な血管形成術チームは、試行された患者の 90% 以上で重度の病変を正常に拡張できるようになりました。 この成功率は、手術による死亡率が 1% 未満、心筋梗塞および/または緊急の冠動脈バイパス移植手術が 5% 未満で達成できます。 しかし、拡張に成功した患者の 3 分の 1 ほどが、拡張病変の再狭窄を反映して、次の 6 か月から 1 年以内に狭心症の再発を経験します。 これらの個人のほとんどは、2 回目の拡張を成功させることができます。
したがって、いくつかの機械的手段による緩和を必要とするほどの症候性冠動脈疾患の患者には、2 つの非常に効果的な介入が利用できます。 それぞれに長所と短所があります。 PTCA は比較的非侵襲的であり、最初は入院日数が少なく、集中治療も少なくて済み、後で必要になった場合に CABG のために静脈と縦隔を保存します。 しかし、多血管疾患の患者に存在するすべての病変が拡張できるわけではありません。 再狭窄および症状の再発の実質的なリスクがあり、PTCA の失敗に続いて直ちに CABG 手術を受けなければならないわずかではあるが有限のリスクがあり、手術のリスクが大幅に増加します。 CABG 手術は、より完全な血行再建術と比較的低い短期間の移植片閉鎖をもたらし、PTCA とほぼ同等の急性リスクで行うことができます。 しかし、外科的介入が患者のアテローム性動脈硬化症の傾向を変えないことは明らかです。 10 年間で、移植片の 3 分の 2 がアテローム性動脈硬化症または血栓症に侵されており、患者の約半分で本来の非バイパス冠循環でアテローム性動脈硬化症が進行しています。 2 回目の手術は技術的にかなり難しく、手術のリスクが高く、症状の劇的な緩和はあまりありません。 PTCA と CABG のどちらを先に適用すべきかが不明な、重篤で症候性の多血管性冠動脈疾患患者が相当数存在します。
1984 年 9 月、冠動脈バイパス移植手術に関するワークショップは、手術と血管形成術を含む臨床試験を検討することを推奨しました。 1985 年 1 月に設立された NHLBI タスク フォースは、研究所が多血管冠動脈疾患の臨床試験を検討することを推奨しました。 心臓病諮問委員会は満場一致で試験の計画と実施を推奨しました。 1985 年 9 月、National Heart, Lung, and Blood Advisory Council は、この臨床試験のイニシアチブについて議論し、熱心に推奨しました。
デザインの物語:
先行する心筋梗塞の有無にかかわらず、重症狭心症または不安定狭心症のために臨床ユニットで冠動脈造影を受けた患者は、研究への参加を求められました。 無作為化を拒否した合計 2,013 人の適格な患者と、血管造影所見に基づいて不適格であった 422 人が、フォローアップ登録に参加するよう求められました。 残りの患者は、1988 年 8 月から 1991 年 8 月までの間に 914 人が CABG に、915 人が PTCA に無作為に割り付けられました。 ベースライン データには、臨床プロファイル、12 誘導心電図、および冠動脈造影の特徴、狭心症と機能状態、投薬、危険因子、生活の質に関する情報が含まれていました。 最初の血行再建術は、無作為化後 2 週間以内に実施されました。 血管造影図 (ベースラインおよび 1 年および 5 年を対象としたサブスタディ)、および ECG は、それぞれのコアラボによって解釈されます。 PTCA の予定された複数の段階は、1 つの手順としてカウントされました。 ステントなどの新しい介入デバイスは、最初の血行再建術では使用されませんでした。 フォローアップの訪問は、研究開始後 4 週間から 14 週間、1 年、3 年、5 年後に診療所で実施され、電話による連絡は 6 か月、2 年、4 年に行われました。 危険因子の修正の重要性は、患者とその主治医に対して研究全体を通して強調されました。 主要エンドポイントは、すべての原因による死亡率でした。
