ACHTERGROND:
De behandeling van patiënten met symptomatische coronaire hartziekte is de afgelopen twintig jaar aanzienlijk geëvolueerd met de toepassing van invasieve technieken bij een steeds groter deel van de patiënten.
Met verfijningen in CABG-chirurgie in de afgelopen twintig jaar, is operatieve myocardiale revascularisatie mogelijk bij de meeste patiënten met myocardischemie. Er zijn verschillende multicenter, gerandomiseerde onderzoeken gerapporteerd waarin medische behandeling met chirurgische behandeling bij patiënten met coronaire hartziekte werd vergeleken. Op basis van de drie grootste trials en een uitgebreide observationele literatuur lijkt er een consensus te ontstaan over de omstandigheden waaronder medische of chirurgische behandeling de voorkeursmethode lijkt te zijn.
Het is duidelijk dat CABG angina pectoris verlicht bij de overgrote meerderheid van patiënten met ernstige symptomen. Het is ook duidelijk dat deze procedure met een zeer laag risico kan worden uitgevoerd door ervaren operatieteams. CABG is dus geïndiceerd voor patiënten met een geschikte coronaire anatomie die ernstige angina pectoris hebben die ongevoelig is voor medische therapie en patiënten met significante obstructie van de linker hoofdkransslagader.
De timing van operatieve interventie bij patiënten met minder ernstige angina pectoris is momenteel een punt van discussie. Het is echter duidelijk dat bepaalde categorieën patiënten de overleving hebben verbeterd na een electieve bypassoperatie. Deze categorieën omvatten patiënten met stenose van de linkerhoofdkransslagader, drievoudige vaataandoening met een matig verminderde ventrikelfunctie en mogelijk andere klinisch gedefinieerde patiënten met een hoog risico. De overige patiënten met milde, symptomatische coronaire aandoeningen, zo lijkt het, kunnen het zich veroorloven om operatieve interventie uit te stellen totdat de symptomen verergeren en palliatie nodig hebben.
Er is langetermijninformatie beschikbaar over enkele patiënten die begin jaren zeventig CABG hadden. Tegen tien jaar is er aanzienlijke progressie van de ziekte aanwezig in zowel de eigen coronaire circulatie als in de bypass-transplantaten. Onderzoekers hebben gemeld dat maar liefst tweederde van de bypasstransplantaten gesloten of vernauwd is en dat de intrinsieke coronaire ziekte 10 jaar na de operatie is gevorderd in maar liefst de helft van de niet-bypass-vaten. De timing van chirurgie bij minder ernstig symptomatische patiënten, strategieën om ziekteprogressie in getransplanteerde vaten en natuurlijke bloedsomloop te verminderen, en hernieuwde belangstelling voor het gebruik van de interne borstslagader om het hart te revasculariseren, zijn momenteel zeer actieve onderzoekskwesties.
PTCA werd voor het eerst toegepast op coronaire aandoeningen bij de mens in 1977. Er zijn grote vorderingen gemaakt op het gebied van katheterontwerp, ballonconstructie en identificatie van zowel hoog- als laagrisicopatiënten voor deze procedure. Ervaren angioplastiekteams kunnen nu met succes ernstige laesies verwijden bij meer dan 90 procent van de geprobeerde patiënten. Dit slagingspercentage kan worden bereikt met minder dan één procent operatieve mortaliteit en minder dan vijf procent myocardinfarct en/of opkomende coronaire bypassoperatie. Maar liefst een derde van de met succes verwijde patiënten zal binnen de volgende zes maanden tot een jaar de terugkeer van angina ervaren, wat een weerspiegeling is van restenose van de verwijde laesie. De meeste van deze personen kunnen met succes een tweede dilatatie ondergaan.
Er zijn dus twee behoorlijk effectieve interventies beschikbaar voor patiënten met symptomatische coronaire hartziekte die voldoende zijn om palliatie op een of andere mechanische manier te vereisen. Elk heeft sterke en zwakke punten. PTCA is relatief niet-invasief, vereist aanvankelijk minder ziekenhuisdagen en minder intensieve zorg, en behoudt aders en mediastinum voor CABG mocht dit later nodig zijn. Niet alle laesies die aanwezig zijn bij een patiënt met een meervatsziekte kunnen echter worden verwijd. Er is een aanzienlijk risico op restenose en herhaling van symptomen, en een klein maar eindig risico om onmiddellijk een CABG-operatie te moeten ondergaan na een mislukte PTCA, waardoor het risico van de operatie aanzienlijk toeneemt. CABG-chirurgie resulteert in een meer volledige revascularisatie en een relatief lage kortdurende sluiting van het transplantaat, en kan worden uitgevoerd met een acuut risico dat ongeveer gelijk is aan PTCA. Het is echter duidelijk dat chirurgische interventie de neiging van een patiënt tot atherosclerose niet verandert; in tien jaar is tweederde van de transplantaten gecompromitteerd door atherosclerose of trombose en is er bij ongeveer de helft van de patiënten progressie van atherosclerose in de oorspronkelijke, niet-gebypasseerde coronaire circulatie. Tweede operaties zijn technisch aanzienlijk moeilijker, brengen een hoger operatierisico met zich mee en resulteren in een minder dramatische verlichting van de symptomen. Er is een substantiële populatie van patiënten met ernstige, symptomatische meervatskransslagaderaandoening bij wie het onduidelijk is of eerst PTCA of CABG moet worden toegepast.
In september 1984 adviseerde de Workshop on Coronary Artery Bypass Graft Surgery de overweging van een klinische proef met chirurgie en angioplastiek. Een NHLBI-taskforce, opgericht in januari 1985, adviseerde dat het instituut een klinische studie naar multivatische coronaire hartziekte zou overwegen. De Adviescommissie Cardiologie adviseerde unaniem het ontwerp en de uitvoering van een proef. In september 1985 besprak de National Heart, Lung and Blood Advisory Council dit initiatief voor klinische proeven en beval het enthousiast aan.
ONTWERP VERHAAL:
Patiënten die coronaire arteriografie ondergingen op de klinische afdelingen vanwege ernstige angina pectoris of onstabiele angina pectoris, met of zonder voorafgaand myocardinfarct, werden gevraagd om deel te nemen aan het onderzoek. In totaal werden 2.013 in aanmerking komende patiënten die randomisatie weigerden en 422 die niet in aanmerking kwamen op basis van angiografische bevindingen, gevraagd om deel te nemen aan de follow-upregistratie. De overige patiënten werden vervolgens gerandomiseerd, 914 naar CABG en 915 naar PTCA, tussen augustus 1988 en augustus 1991. Basisgegevens omvatten het klinische profiel, 12-afleidingen elektrocardiogram en informatie over coronaire angiografische kenmerken, angina pectoris en functionele status, medicatie, risicofactoren en kwaliteit van leven. Initiële revascularisatie werd uitgevoerd binnen twee weken na randomisatie. Angiogrammen (baseline en substudie gericht op 1 jaar en 5 jaar) en ECG's worden geïnterpreteerd door de respectievelijke kernlaboratoria. Geplande meerdere stadia van PTCA werden geteld als een enkele procedure. Nieuwe interventionele apparaten, zoals stents, werden niet gebruikt tijdens de initiële revascularisatie. Vervolgbezoeken werden gehouden in de klinieken in de weken vier tot en met veertien na aanvang van de studie en na één, drie en vijf jaar, met telefonische contacten na zes maanden en twee en vier jaar. Het belang van de wijziging van risicofactoren werd gedurende het hele onderzoek benadrukt aan de patiënten en hun primaire artsen. Het primaire eindpunt was sterfte door alle oorzaken.
De proef is verlengd tot en met november 2002 om de minimale tienjarige follow-up van alle BARI-patiënten te voltooien, de relatieve werkzaamheid van PTCA versus CABG te bepalen in subgroepen van vrouwen, zwarten, diabetici en ouderen, en de impact van BARI op de volksgezondheid te beoordelen. In de follow-up zullen alle momenteel ingeschreven patiënten jaarlijkse telefonische interviews hebben. Na tien jaar zal het elektrocardiogram bij alle patiënten worden verkregen en zullen de bloedlipideniveaus alleen bij gerandomiseerde patiënten worden uitgevoerd. Alle ziekenhuisopnames sinds het laatste contact worden vermeld op het jaarlijkse telefonische contact. Angina pectoris wordt de afgelopen zes weken beoordeeld. Bij elk patiëntencontact krijgen patiënten instructies over gedragsverandering op het gebied van stoppen met roken, lichaamsbeweging en voeding. Angiografische studies zullen ook worden uitgevoerd als onderdeel van de follow-up van tien jaar. De vier centra die deelnamen aan de eerste fase van deze BARI-activiteiten zullen opnieuw de angiogrammen uitvoeren. Patiënten zullen bestaan uit het gevestigde cohort getrokken uit de gerandomiseerde deelnemers die angiogrammen van één en vijf jaar hebben voltooid.
Een substudie van BARI, Study of Economics and Quality of Life (SEQOL), werd aanvankelijk gefinancierd door de Robert Wood Johnson Foundation om de impact van een specifieke revascularisatie op de kwaliteit van leven, functionele en economische (ziekenhuis- en artskosten) resultaten bij patiënten te beoordelen gerandomiseerd naar BARI, en om andere factoren dan behandelingen te onderzoeken die deze uitkomsten beïnvloeden. Vanaf juli 1997 werd de steun voor SEQOL overgenomen door de NHLBI onder subsidie R01HL58324. De subsidie, die afloopt in juni 2002, verlengt de follow-up van het gerandomiseerde SEQOL-substudiecohort (752 proefpersonen overleefden vanaf 1 mei 1996) tot tien jaar om de langetermijndeterminanten van kosten en kwaliteit van leven te bestuderen en een model om de impact van technologische veranderingen op de resultaten en kosten van CABG en PTCA te projecteren.
