外傷は、年間 580 万人の死亡原因であり、これは世界の死亡率のほぼ 10% を占めています。 15 歳から 59 歳の成人では、死因としての負傷の割合はさらに高く、22% から 29% の範囲です [1]。

病院到着時に以下のパラメーターの少なくとも 1 つ:
- 呼吸数≧30/分または≦10/分
- 脈拍≧120/分;
- -収縮期血圧≤100 mmHg
- 推定外部出血量≧500ml
- グラスゴー昏睡スコア≦13
- 異常な瞳孔対光反射。
または、次の診断のいずれかの臨床的疑い:
- 少なくとも 2 つの長骨の骨折
- 複数の肋骨骨折、フレイルチェストまたはオープンチェスト
- 重度の腹部損傷
- 骨盤骨折
- 不安定な脊椎骨折または脊髄損傷の徴候。
または、次の損傷メカニズムのいずれか:
- 高所からの落下 (10 フィート以上)
- 車両からの排出
- 同乗者死亡
- 同じ車両に重傷を負った患者
- 胸部/腹部がくさび状になっている、または閉じ込められている。
除外基準:
- 年齢 <18 歳 (わかっている場合)
- 既知の妊娠
- 他院からの紹介患者
- 鈍的損傷メカニズムによる明らかに低エネルギーの外傷
- 明らかに孤立した外傷として、身体の 1 か所の刺し傷 (銃創を除く)
- CTスキャンを受けるには不安定すぎると判断され、死が差し迫っているため(心肺)蘇生または即時手術が必要な患者。
