動脈血管疾患は、1 型糖尿病患者 (DM1) の罹患率と死亡率の主な原因です。
代謝性インスリン抵抗性(metIR)は、高血糖がない場合でも、一般集団の CVD リスクを 1.5 ~ 3 倍に高めます。
代謝性インスリン抵抗性 (MetIR) は、DM1 の成人および青年に蔓延していることが繰り返し示されています。
肥満における MetIR および DM2 は、抵抗性血管または微小血管に対するインスリンの血管拡張作用の障害を特徴とする血管インスリン抵抗性 (vasIR) を伴います。
VasIR は DM1 では体系的に研究されていません。
我々は、若年成人ではDM1が動脈血管系全体のベースラインおよびインスリン反応性血管機能の両方を損なうと仮説を立てている。

私たちの研究では、20 人の健康な対照被験者を 20 人の DM1 患者 (18 ~ 40 歳) と比較します。
導管(脈波速度-PWV、血流媒介拡張-FMD、増大指数-AI)、抵抗(虚血後流速-PIFV)、心臓および骨格筋微小血管(造影超音波-CEU)血管の機能を評価します。正常血糖インスリンクランプの 2 時間前と後。

説明
包含基準:
- 18~40歳
- BMI ≤ 25
- 慢性疾患がなく健康(対照群のみ)
- 通常のスクリーニング検査機関、または臨床的に重要な値がない
- DM1被験者は少なくとも5年間インスリンを服用しており、HbA1c <9です
除外基準:
- 1型または2型糖尿病を患う一親等親戚(対照群のみ)
- 現在または過去6か月以内に喫煙している
- 血管系に影響を与える薬剤(DM1被験者のACEまたはARBを除く。ただし、試験前に2週間これらの薬剤を中止する必要がある)。
- LDLコレステロールの上昇 > 160
- 血圧 <100/60 または >160/90
- パルスオキシメトリー <90%
- 妊娠中または授乳中の方
- 心血管疾患、脳血管疾患、末梢血管疾患、肝疾患の既往
- 心臓内または肺内シャントの存在(身体検査中に聴診によってこれをスクリーニングします)。
- パーフルトレン(Definityに含まれる)に対する既知の過敏症
DM1 グループの場合:
- HbA1c ≧ 9
- 微量アルブミン尿
- 網膜症
- 過去1年以内のケトアシドーシス。
