Het effect van diabetes type 1 op de pan-arteriële vasculaire functie en insulinegevoeligheid bij mensen (EJB046)

Arteriële vasculaire ziekte is de belangrijkste oorzaak van morbiditeit en mortaliteit bij Type 1 diabetespatiënten (DM1).
Metabole insulineresistentie (metIR), zelfs bij afwezigheid van hyperglykemie, brengt een 1,5 tot 3 maal verhoogd risico op hart- en vaatziekten met zich mee in de algemene bevolking.
Er is herhaaldelijk aangetoond dat metabole insulineresistentie (MetIR) voorkomt bij volwassenen en adolescenten met DM1.
MetIR bij obesitas en DM2 gaat gepaard met vasculaire insulineresistentie (vasIR), die wordt gekenmerkt door verminderde vaatverwijdende werking van insuline op resistentie of microvasculaire vaten.
VasIR is niet systematisch onderzocht in DM1.
Onze hypothese is dat DM1 bij jongvolwassenen zowel de baseline als de op insuline reagerende vasculaire functie in het gehele arteriële vaatstelsel schaadt.

In onze studie worden 20 gezonde controlepersonen vergeleken met 20 DM1-patiënten (18-40 jaar).
We zullen de functie in conduit (pulsgolfsnelheid-PWV, flow-gemedieerde dilatatie-FMD en augmentatie-index-AI), weerstand (post-ischemische stroomsnelheid-PIFV) en microvasculaire hart- en skeletspiervaten (contrastversterkte echografie-CEU) beoordelen voor en na 2 uur van een euglycemische insulineklem.

Beschrijving
Inclusiecriteria:
- Leeftijd 18-40
- BMI ≤ 25
- Gezond zonder chronische ziekte (alleen controlegroep)
- Normale screeningslaboratoria of geen klinisch significante waarden
- DM1-patiënten zullen minimaal 5 jaar insuline hebben gebruikt en een HbA1c <9 hebben gehad
Uitsluitingscriteria:
- Eerstegraads familielid met diabetes type 1 of 2 (alleen voor controlegroep)
- Roken op dit moment of in de afgelopen 6 maanden
- Medicijnen die het vaatstelsel aantasten (behalve ACE of ARB bij DM1-proefpersonen, hoewel ze deze medicijnen 2 weken voorafgaand aan het onderzoek moeten gebruiken).
- Verhoogd LDL-cholesterol > 160
- Bloeddruk <100/60 of >160/90
- Pulsoximetrie <90%
- Zwanger of borstvoeding
- Geschiedenis van hart- en vaatziekten, cerebrale vasculaire aandoeningen, perifere vasculaire aandoeningen, leveraandoeningen
- Aanwezigheid van een intracardiale of intrapulmonale shunt (we zullen hierop screenen door middel van auscultatie tijdens het lichamelijk onderzoek).
- Bekende overgevoeligheid voor perflutren (opgenomen in Definity)
Voor DM1-groep:
- HbA1c ≥ 9
- Microalbuminurie
- Retinipathie
- Ketoacidose in het afgelopen jaar.
