この研究の目的は、バングラデシュの第 1 レベルのプライマリケア施設で急性呼吸器感染症を管理するために IMCI サービスにパルスオキシメーター (PO) を導入することの実現可能性、受容性、および運用上の課題を評価することです (フェーズ 1)。 研究者はまた、第 1 レベルのプライマリ ケア医療施設 (フェーズ 2) で IMCI サービスに PO を導入することの有効性と費用対効果を評価します。

バックグラウンド:
- 重荷: 肺炎は小児死亡率の主な原因であり、バングラデシュおよび世界の 5 歳未満の死亡者数の 16% を占めています。 これらの死亡者のほとんどは開発途上国で発生しており、WHO はこれらの貧弱なリソース設定で、肺炎を含む一般的な小児疾患の外来管理戦略として小児疾患の統合管理 (IMCI) を採用することを推奨しています。

方法:
この研究では、IMCI サービス (フェーズ 2 の目的) に PO を導入することの有効性を評価するために、クラスター無作為化比較試験デザインを採用します。 実現可能性評価 (フェーズ 1 の目標) は、内部パイロットとして、より大きな有効性トレイル内にネストされます。これは、堅牢なフェーズ 2 試験を設計するための証拠を生成するのに役立ちます。 IMCI サービスを提供する第 1 レベルのプライマリ ヘルスケア施設は、クラスターおよび無作為化の単位と見なされます。 24 の第 1 レベルのプライマリ ケア医療施設が無作為に比較および介入施設に割り当てられます。
フェーズ 1 では、IMCI サービスに PO を導入することの実現可能性、受容性、および運用上の課題が介入施設で評価されます。 フェーズ 1 の目的に対処するために、IMCI サービスにおける PO の使用の構造化された観察とコミュニティの事例追跡が実施されます。 厳格なプロセスの文書化とデータ収集の定性的な方法を採用して、実装プロセスを文書化し、実装結果を評価します。
フェーズ 2 では、比較施設と介入施設の両方でコミュニティの症例追跡を通じて有効性が評価されます。 治療失敗率が主要な結果となり、分析では「治療の意図」アプローチが考慮されます。 また、原価関連データの収集には、活動基準原価計算法が採用されます。

実験的:介入(パルスオキシメータ)
介入施設は、既存の IMCI ガイドラインに従うことに加えて、PO で IMCI サービスを提供します。
IMCI サービス プロバイダーは、病歴と臨床徴候に基づいて、咳や呼吸困難を呈する子供を分類し、治療します。
さらに、PO を使用して、病気の子供の SpO2 ステータスを測定します。
臨床的に「肺炎」と分類されているがSpO2が90%未満の小児は、入院管理のために上位施設に紹介されます。
臨床的に「肺炎」と分類され、SpO2>90% の子供のみが、経口抗生物質 (アモキシシリン、1 日 2 回、5 日間) による在宅管理によって治療されます。

SpO2 は、低ノイズ ケーブル センサーを適用したハンドヘルド PO デバイス (この研究用に選択) を使用して測定されます。
体重が 10 kg の患者の場合、センサーは人差し指に装着されます。
最初の試行で失敗した場合は、別のつま先または人差し指が試行されます (2 回目の試行)。
最初に、センサーをつま先または人差し指に置き、PO の電源を入れます。
SpO2 測定値が少なくとも 10 秒間 ±1 % 安定しており、デバイスの信号強度が適切であると表示された場合 (緑色の信号)、安定した SpO2 測定値が得られたと見なされます。
1 ~ 2 分以内に安定した読み取り値が得られない場合は、2 回目の試行が保証されます。

NO_INTERVENTION:比較
比較施設は、既存のガイドラインに従って定期的な IMCI サービスを提供し続けます。
通常の IMCI サービスでは、IMCI サービス プロバイダーは、病歴と臨床徴候のみに基づいて、咳や呼吸困難を呈する子供を分類し、治療します。
バングラデシュの通常の IMCI サービスでは、PO は導入されていません。
したがって、比較施設では、臨床的に「肺炎」と分類されたすべての子供は、経口抗生物質(アモキシシリン、1日2回、5日間)による在宅管理を通じて治療されます。
