化学療法後の樹状細胞ワクチン接種 腫瘍は、骨髄由来サプレッサー細胞 (MDSC) などの抑制細胞の腫瘍微小環境への動員を含む、抗腫瘍免疫応答を抑制するいくつかのメカニズムを利用します 13。 抑制性腫瘍微小環境における MDSC の存在は、DC ワクチン接種やイピリムマブを含むいくつかの免疫療法の有効性の低下と相関しています 14,15。 治験責任医師の研究室で得られたデータは、MDSC がプラチナベースの化学療法剤で標的にされる可能性があることを示しています。 シスプラチンを週 6 回投与された頭頸部扁平上皮癌患者では、MDSC の頻度と抑制能力が最終投与の 2 週間後に有意に抑制されました。 したがって、カルボプラチンによる6サイクルの化学療法後に患者をDCワクチン接種で治療すると、DCワクチン接種の臨床結果にプラスの影響を与える可能性があります。

シングルアームの探索的、単一施設研究

私たちの研究集団は、カルボプラチン/パクリタキセル化学療法を週1、2、3、および5、6、7週に毎週受ける8人のmEC患者で構成されています。
8 週目に、骨髄および形質細胞様 DC (nDC) に腫瘍ライセートと MUC1 およびサバイビン PepTivators をロードし、節内に注射します。
すべての患者に対して広範な免疫運動が行われます。
安定した疾患、部分奏効、または完全奏効を示す患者は、17、20、および 23 週目に nDC の節内注射による 3 週間の延長化学療法レジメンを継続します。
