このプロジェクトでは、内因性オピオイドを (SMT または MT を介して) 強化することで、オピオイド鎮痛薬の投与量を減らし、副作用や乱用に関連する薬物効果を減らして、望ましいレベルの鎮痛を達成できるかどうかを判断します。 この X 年間のプロジェクトでは、240 人の慢性腰痛患者のサンプルで研究仮説をテストします。 この研究には 2 つの重要な要素があります: 1) 慢性疼痛 (CP) 患者における無作為化され、制御された SMT または MT で、毎日の自宅でのエクササイズを完了すること、および 2) EO 機能の定量化を可能にする、治療前および治療後の実験室誘発熱痛プロトコル治療による EO 機能の変化の調査 (プラセボ対ナロキソン)。

- -少なくとも3か月間、毎日の重大な慢性疼痛強度(10ポイントスケールで4以上)および痛みによる日常活動の妨害(10ポイントスケールで3以上)
- 18 歳から 75 歳まで
- -毎日、または各実験室セッションの3日以内にオピオイド鎮痛薬を使用していない(尿薬物スクリーニングで確認)
- 無傷の認知状態とインフォームドコンセントを提供する能力
- 調査アンケートを理解して完了するのに十分な英語の読み書き能力
除外基準:
- アルコールまたは薬物乱用の問題の基準を満たす
- 過去または現在の精神病性障害または双極性障害の基準を満たす
- アンケートに回答したり、治療に参加したりするのに十分なほど英語を理解できない
- 悪性疾患による痛み、関節リウマチ、片頭痛または緊張性頭痛、複雑な局所疼痛症候群、または線維筋痛症症候群
- 過去6か月以内の腰椎手術

治療セッションの直前と直後、および治療の途中(4セッション後)に、参加者はプラセボ/ナロキソン投与介入を受け、SMTまたはMT関連の変化のメカニズムを評価します。

セッションごとに 2 つの SMT 技術が管理されます。どちらも、SMT を提供する理学療法士 (PT) と被験者との間のスクリプト化された対話です: 1) PT が約 6 フィート離れている間、参加者は SMT テーブルに 20 分間横になります。
スクリプトの一部として、PT は対象者と共に包含/除外基準を表向きにレビューし、何も変更されていないことを確認します。

各 MT セッションは、(a) 横になった状態での呼吸やその他の身体感覚の認識を伴うボディスキャン瞑想、(b) 呼吸、身体感覚、思考、感情の認識に焦点を当てた座位瞑想、(c) 穏やかな動きで構成されます。運動中に意識(マインドフルネス)を発達させることを目的とした演習。
