概要: 現在のパイロット研究は、ランダム化された行動運動介入です。
手順:
研究フェーズ I: 家族 (n=60) がベースライン調査訪問を完了します。 その後、青少年はランダムに運動グループの 1 つに割り当てられ、UMMC 統合健康センター (CIH) で直接、指導型の運動セッションを 1 回完了します。 指導演習セッションの後、青少年は 3 日間の在宅外来モニタリングプロトコルを完了します。 研究フェーズ II: 次に、青少年は 8 週間の遠隔医療自宅運動プロトコルを完了します。 青少年は、第 4 週と第 8 週に 3 日間の外来モニタリング プロトコルを完了します。青少年は、運動プロトコル完了後 1 週間以内に UMMC CIH に戻り、アンケートと臨床評価を繰り返します。 保護者もアンケートを繰り返します。
研究フェーズ I:
ベースライン調査訪問: 登録後、RA は親の連絡先情報と他の 2 つの関係の連絡先情報を取得します。 青少年とその保護者は、Apple iPad 上の REDCap 電子データ収集を使用してアンケートに回答します (10 分)。 アンケートには、人口統計、痛み、身体機能、否定的な感情に関する項目が含まれます(うつ病と否定的な感情は、SCD11、63、112、113の若者の痛みの最も確実な予測因子の1つであるため)。 採血: ベースラインの炎症マーカーを測定する目的で、血液サンプルが収集されます (10 分間)。 予定された臨床採血中に、局所リドカインクリームを使用して血液を採取するためにあらゆる努力が払われます。 臨床的および体力の評価: 次に、Actigraph GT9X-BT デバイスを青少年の手首に装着し、ベースライン調査訪問中に HRV を監視します。 その後、青少年は臨床および体力の評価を完了します (30 分、詳細は下記)。

研究フェーズ II:
8 週間の遠隔医療運動プロトコル。 被験者内および被験者間のデザインを使用して、青少年は自分自身の対照(前後)として機能し、(1)低全身筋力トレーニングまたは( 2) 適度な全身運動トレーニング。 指導演習セッションの後、すべての演習セッションは自宅で完了し、遠隔医療を通じて研究演習担当者が監督します。 各運動セッション中は、保護者も家にいなければなりません。 保護者がいらっしゃらない場合は、練習の日程を変更させていただきます。 運動担当者は、各セッションで運動の有害事象を記録します。 すべての有害事象は直ちに医療責任者および主任研究者に報告されます。
3日間の外来モニタリング。 遠隔医療運動プロトコルの第 4 週目と第 8 週目の間、青少年は 3 日間連続して毎日の日記とアクティグラフィーによる外来モニタリングに参加します。 手首装着型アクティグラフィー デバイスは、パッド入りの封筒に入れて普通郵便で返却するか、再診時に返却します。
フォローアップ訪問。 家族は、8週間の遠隔医療運動プロトコルを完了してから1週間以内にUMMC CIHに戻ります。 青少年は、痛み(主な結果、目標 2)と HRV および体力の評価(副次的結果、目標 2)に関するアンケートを繰り返します。 保護者もアンケートを繰り返します。

8 週間 (週 3 回、セッション 45 分) の自宅ベースの遠隔医療強化運動プログラム

低全身筋力トレーニング。
青少年は45分間の運動トレーニングを完了します。
カーフレイズ、ハンドグリップ、スクワット、シーテッドロウ、チェストプレス、プランク、ローバックエクステンション、クランチのすべての主要な筋肉群をターゲットとする8つの特定のエクササイズが実行されます(表1)。
各エクササイズには中程度の負荷での収縮が含まれますが、持続時間は最長 5 分間となります。16、17
各エクササイズセッション中、青少年は 5 分間のウォームアップ、5 分間のクールダウンを行い、必要に応じて休憩をとることができます (最低 30 秒)。
青少年は、最初は休憩時間の数を減らし、次に負荷を増やすことによって進歩します。
青少年には、2ポンドのメディシンボール、レジスタンスウェイトバンド、セラパテ、自宅でのエクササイズ用のヨガマットが提供されます。
青少年はこの家庭用運動器具を保管します。

中程度の全身運動負荷グループに割り当てられた青少年は、予備心拍数 (HRR) の 40 ~ 59% で 45 分間、全身の有酸素運動を行います。76,111
青少年は、早足で歩き、腕を振りながら歩くことで、中程度の全身有酸素運動を完了します。
運動担当者は、ビデオ会議を通じて 5 分間のウォームアップと 5 分間のクールダウンを監督し、ウォーキング運動セッション中は常に青少年と電話を保ちます。
青少年は、所定の時間が達成されるまで、または疲労するまで運動し、その時点で強度を下げるか、回復するまで停止します。
すべてのトレーニング強度は HRR に基づいているため、個別化されます (HRR の 59% を超えない)。
初期のトレーニング要件を満たせない青少年は、8 週間かけてゆっくりとトレーニングを進めていきます。

この研究における潜在的な懸念は、12~17歳という幅広い年齢層であり、子供の免疫系の変化と思春期のタナー段階の違いが運動に対する生理学的反応に影響を与える可能性があることである。
